If the prostate is slightly swollen, it crimps the urethra, and you can get leakage/discharge of retained urine, as you correctly speculate.

Urge to urinate, pressure feeling, can suggest prostatitis.

Are you in the early stages of CPPS? Time will tell. But you could abort the syndrome if you follow the recommendations we discuss here, which are too numerous to list in this one post.

Does that mean the urge to urinate and pressure feeling cannot be related to an irritated prostate due to an earlier infection? I was hoping that all symptoms would clear up after a while with the irritation or inflammation going away. That’s what the two docs suggested.

In fact, I think I’m feeling better, at least yesterday and today during morning until early afternoon. I was almost completely without symptoms during that time. However, I must add that I didn’t drink that much water during that time, only some glasses in the morning and a small bottle for lunch. I didn’t have to pee urgently during that time.

During the afternoon, once I was at my desk with a water bottle and started to drink more, the feeling of having to urinate would start faster and more often (sometimes three times within 60-90 minutes and without drinking additional fluids after having gone to the toilet).

It made me think whether I should space my water intake more evenly throughout the day, such as a glass of water every hour or so only.

I have to say that I am normally a person who doesn’t drink much throughout the day unless I had/have an UTI. So not sure if my bladder just isn’t used to it (but before I could drink lots of fluid without causing such immediate urge to pee).

An infection can be the first step towards CPPS, even if it is cured. Anything that causes pain or disruption in the pelvis can start a nerve process that escalates. The actual biochemical and neurological explanation is very complex, but that's essentially what happens.

Liquids: as long as you get a litre or two a day, you'll be okay. Urine colour is more important: make sure it stays straw colored, not orange and never brown.

Hi,

I am almost in the same boat as you are.
First somekind of nsu (altough negative tests) which was cured with azithromycine, got it back after a few weeks and azithromycine doesnt work anymore...

I also have this ‘discharge’, but can't really see it, like there is moisture left in the urethra and glans becomes sticky after a while.

Maybe just do mdx test or proper std test (you did mycoplasma genitalium and ureaplasma?) and if negative let the std illusion go..

Did you have the same symptoms during last uti/sti?

I already ordered the MDX test. I did two multiplex PCR urine tests that test for 7 common STIs including MG, MH, UU, UP etc. Both came back negative. Was wondering if it could be false negatives since the first one was taken just days after I took Azithromyzin and the second one some weeks after, but doctor told me antibiotics won’t affect the realibility of the test. Several normal urinalysis also didn’t show any signs of an infection, same as for semen test as well as for swab.

During some of my last UTI/STI I think I had similar symptoms. At least the discharge deep inside the penis that I can see sometimes but cannot squeeze it out. Not sure for the urinary symptoms of frequently having to go pee. I think normally during UTI/STI I would feel there’s discharge/wetness inside the penis, so I would drink a lot and therefore go pee more often. But when the UTI/STI is gone I wouldn’t have this feeling of discharge/wetness anymore and no urine leakage, so I wouldn’t drink that much anymore (I normally drink not enough during the day). I might just experiment with drinking less or spacing it throughout the day more evenly to see if that improves the urinary symptoms.

Two urologists say there’s no infection but just an irritation that will go away. It’s what I hope for still, but it’s already two months now after the Azithromyzin (and 2.5 months after the infection).

The only thing that worked for me was azithromycine, I had a lot more antibiotics, supplements, anti depressants, pelvic therapy, ... without any success.
Also I had a lot of urine analysis, penile swabs, 5 microgen tests, a lot of std tests and 3 uro’s.... all negative.

How did you cure previous UTI’s?

Just a short course of antibiotics, normally two different ones for a week. In case of suspected Gonnorhea an injection. Until this time I had never heard of Prostatitis or CPPS. UTIs/STIs for me were something not to worry about because I could just throw antibiotics at it and after a week it would be all good. That’s why I had lot of unprotected sex and a fair amount of infections though I never actually tested for it (I would just get the antibiotics from the doc whenever there was discharge).

When you say Azithromyzin is the only thing that worked for you, do you mean it cured you completely? Or just the infection?

Not sure how I can work on those pelvic floor muscles. I don’t have access to PT etc. where I live. I’m working in an exotic country which has excellent hospital for traditional common things but nothing that goes beyond surgeries and pills.

So far I’ve started to:
- change my diet to less sugar, less carbs, less red meat, more veggies
- take supplements for the prostate (saw palmetto etc.)
- play soccer again

I will also try yoga.

What I need to work on most is anxiety and worry. Don’t know how to do that.

Only urinary symptoms - what conditions to rule out first?

I had unprotected sex, took antibiotics, all tests turn out negative, but the urinary symptoms still persist. It’s 2 months now since I took the last antibiotics. Two urologists keep telling me there’s no infection but just an irritation that needs time to clear by itself. I should drink plenty of water and maybe take symptomatic relief meds such as UriSpas or Pyridium. After reading this forum (and others about Prostatitis) I am worrying that’s it’s something worse, such as kidney stones, a stricture, a chronic bacterial infection that didn’t show so far, or ultimately CPPS/IC caused by anxiety, guilt, stress.

My question is: Before concluding it’s CPPS/IC, what other conditions should I rule out first?

Done already:
- several urinalysis, all negative
- 2 multiplex PCR urine tests checking for 7 STI, both negative
- 1 semen test, negative
- 1 swab, negative

Should I check for the following?
- full urine culture, maybe even 1 of first void and 1 of midstream urine?
- MDX test kit (ordered already)
- Prostate fluid culture
- another semen test
- Kidney stones
- anything else?

One reason I am specifically asking for kidney stones is because I used to drink not enough water my whole life. However it never caused any symptoms other than my urine typically being more on the yellow than light yellow side. I only started to drink plenty of water whenever I have an UTi/STI but this time I feel like it’s making things worse (ie. it makes me have to pee a lot).

You do not have kidney stone symptoms (and I've had 2 of them, so I should know).

You've had extensive testing for infections, more than is reasonable. Give it up. It's becoming obsessive.

A cystoscopy to check the bladder for inflammation would be interesting.

Isn’t the standard test procedure to rule out an infection first by doing the 4 glass approach? Also, I’ve never done a full urine culture, only urinalysis and PCR tests checking for 7 STI.
A cystoscopy to check the bladder for inflammation would be interesting.

Meares Stamey testing is defunct. Researchers have known for nearly 20 years that the Stamey test is useless and called for it to be abandoned. https://www.medscape.com/viewarticle/494378

Hey,

Same situation as you. All test negative but I got the prelim from MDx and I shall update my post history when I get home.

I agree that you should leave no stone unturned just to be safe (not necessary STDs/STI but some infections can be caused by commensal bacteria) but also invest your time in a possibility that could be CPPS.

Don't forget that the "wet" feeling in the urethra could simply be allodynia, not CPPS. That can fade with time. Allodynia makes something feel weird or uncomfortable that normally is not noticed.

There’s still discharge. I can see it during the day deep inside and more clearly in the morning. Clear and sticky discharge.

How reliable are those multiplex PCR urine tests that check for 7 common STIs? Any information on that?

This doesn’t sound too much different from my situation. Mainly urinary symptoms plus pain in the testicles. And I haven’t done any prostate fluid culture so far like the person in your article.
