Hey all,

Went to GP on Monday for some prescription renewal stuff. Talked to him about my CPPS issues and discussed amitriptyline but he recommended I try Lyrica since it interacts less with my sertraline medication.

I'm on day 4 and I definitely notice the pain has gone down. Will keep updated to see if improvement continues, flatlines, or if I flare up.

Had sex this weekend and the orgasm was very intense, been in a flare up since...the Lyrica may or may not be helping. If it is, its not very noticeable.

At this point I've tried:
  1. Antibiotics
  2. Prednisone
  3. Physical therapy
  4. Self therapy, stretching, internal massage - ongoing
  5. Quercetin - ongoing
  6. Pollen extract - ongoing
  7. Magnesium
  8. NAC
  9. Cystoscopy
  10. 2x PCR of semen/ prostate fluid
  11. Mastubating more
  12. Masturbating less
  13. Prelief
  14. Lyrica - ongoing
The list is more to help me keep track..

Thanks

I had quite bad experience with Lyrica last year. Brain fog and strange feelings of weirdness/depersonalisation/anxiety...and of course hell in the pelvis. So after 4 weeks I withdrew...and started combination of Atarax/low dose Amytriptyline which helped a lot. Summer was great. But everybody is different, you have to find your way. But do not mix Lyrica and Amytriptilin. I have also pain in penile base right side+groin pain. I think the most probable trigger point are in high pubococcygeus+adductors. Difficult to treat, especially puboccocygeus, sometimes I almost feel some clamp 2 cm behind my penis, on the right side. This clamp is going back again and again (there were periods when I did not feel it and I was significantly better).

Yep I've experienced some of that brain fog already. I'll discontinue use if I don't see any benefit. Yep my pain 95% of the time is on the left side of my pelvic floor. Its almost like a nerve or “line” of pain that runs along the left side of my pelvic floor through my bulbo and shaft, only left sides. The same spot on my right side feels perfectly fine. But the main climax point if pain isn't really near the penis at all.

Did you have any weight gain on Lyrica? Just out of curiosity ...

No I didn't. But overall reaction on Lyrica was very bad. Amytriptylin was walk in the park comparing Lyrica...
My pain is strikingly similar, but on the right side. During heavy flare-ups it is also transposing to left side.
Anyway, during the flare-up (like now) I can feel the structures attaching the penis to the body, these "structures" on the right side are very painful, pain is coming from the body "behind the penis" on the right side going into shaft, like some guitar string...It might be pudendal nerve, irritated by the spasms of pubococcygeus. Terrible feeling...

Good to know about Lyrica, thanks. Sounds similar to my left side. Are you ever able to pinpoint the exact location of your pain? For me its very enigmatic. I do agree a nerve is irritated, dont know which, and I think its more a victim than the root cause.

I sometimes clearly feel a clamp on that right side...it is like small fist or something pulling my penis to the side...I call this spot "mother of all trigger points" because this was my first symptom...it is probably huge trigger point (or maybe a group of several trigger points) which is located in pubococygeus (most probably) or ischiocavernosus (less probably). There might be contribution from adductors as well. I agree with all that you wrote, pudendal nerve is a victim (but when it is irritated, the hell begins).

Well Lyrica really didn't too much for my pain, nothing consistent. I asked to try alfuzosin, mainly just to knock it off the list, we will see where it goes...I'm on day 2.

So I've started dating a woman and the orgasms I've been having from sex are a lot more intense which seems to lead to increased pain. Last night when I orgasmed I could literally feel a sharp sensation run along my left bulbo area. There isn't pain immediately after BUT I will wake up in the night a couple hours later with a lot of pain radiating all along my left side. This pain will stay with me for 24 hours or so then the pain will return to normal levels.

I've been stretching after sex and also trying to relax during orgasm, easier said than done. Doesnt really seem to help...

These post sexual pains have left me very depressed the past couple weeks.

Even though I'm massaging internally and externally, and stretching daily, I'm still in constant pain. I'm thinking of trying to go back to a different PT (the first one I went to sucked) and see if they can offer and new techniques/ revelations.

Any advice would be great but I'm also here to vent as I am at a low point.

My post-ejaculation pain pattern started very similarly. Never immediately but 24 hours later, sometimes even 2 days later...Horrible experience.
Pain trace also very similar, along the shaft into the body and then direction perineum. It indicates that nerves are main players in this havoc. Try some diazepin immediately after sex and anxiety plays surely also an important role...

Thanks European, I'm going to see my GP this week and am hoping for a referral to a pain specialist, and I'll ask for a diazepine rx.

How is your pain in the morning when you wake up? Also, did you ever go see a pain specialist?

Yes, I did. Pain specialist suggested various collections of painkillers and relaxants regimen, pretty useless. Urologists the same.

Generally, pain is better morning and progressively worsening, but not always. Approximately 30 weeks in year I have "remission" period when my symptoms are somehow at bay. That means pelvis is rather "achy" than neuropathic pain, I am able to relax and somehow enjoy the life. Still mild ED (but not severe), dysaesthesia (an abnormal unpleasant sensation felt when touched, caused by damage to peripheral nerves) is much lower in all problematic areas (penis, scrotum, pubic area, groin). Rest of the year I have flare-ups with vigorous pain in penile shaft, groin, perineum. Sometimes stingy, nasty pain especially in perineum/right groin. Feeling like penis is constricted with something inside. No or only minor urination symptoms (dribbling, weak stream, but no pain associated with urination). Feeling of having clamp behind the penis, on the right side (this was my first syndrome ever), probably big trigger point in pubococcygeus or bulbospongiosus.

Its weird how you have these periods of remission, you'd think that your body would much prefer to continue with the trend that has less pain. I dont really have penile pain unless you count dysuria. I have had it though in the past, felt like someone was pumping air into my penis, only lasted a few days before symptoms returned to normal.

The only pattern I have observed is that it is seasonal, first half of year is always worse, flare up peaking in April and May and from July till Christmas I was last three years in remission (which is far from optimal but bearable).

Hey Webslave, or anyone,

Any thoughts as to why my symptoms, especially pain, increases a couple hours into falling asleep? And upon waking? When I start walking after waking, I instantly have my pain drop by a significant amount. I even woke up the other night, and like a minute after waking I had a sharp pain radiate from my inner left side perineum, it quickly subsided to normal pain levels. This has never happened while I was awake.

Also, if I don't really have to pee and do pee or toward the end of the urination when the stream is weaker, burning on urination is a lot more noticeable. If I have a strong stream, I rarely feel burning.

Thanks men!
