I have this dull ache in the perineum along with the weakened ejaculation. Whenever possible I sit when urinating which relaxes the pelvic floor muscles. When i'm finished, i'll gently massage the perineum starting from the anus toward testicles. This also evacuates any lingering urine. I also do this in the shower and before bed.

That's good stuff. I also sit when I pee now which seems to improve flow. Is your pain on your entire perineum or does it originate from one side more than the other?

Almost all my pain radiates from this specific area. I hope there is no issue with one of my Cowper's gland (Inflammation) because I really only feel discomfort on the left side , and this also seems to be the area where the gland is located. After massaging this area I can feel the discomfort radiate down my leg. Can an issue with the Cowper's gland cause weakened ejaculation?

I also don't know the relationship between chlamydia leading to complications with cowpers gland.

Just my latest thoughts, I continue on with my usual therapies :/

I doubt you have Cowperitis. You don't have the symptoms:
The bulbourethral or Cowper's glands originate as evaginations of the epithelium that cover the urogenital sinus and contribute about 0.1–0.2 ml of the ejaculate; their secretion neutralizes urine in addition to lubricating the urethra prior to ejaculation. Cowperitis is an inflammation of the gland which can be either chronic or acute; acute Cowperitis presents as fever, malaise and severe pain in the perineum with frequency, urgency, painful defecation and sometime acute urinary retention

I doubt I do as well. I am trying so hard to pinpoint the exact location of my discomfort.... no matter how hard I poke around internally and externally, I can never pinpoint an exact location. It's exclusively to my left side and I can feel it run all the way down through my leg, obviously originating from my pelvis. Masturbated today and the orgasm gave a little burning in urethra, which was different... also very little semen came out and it was very gel like. As I touch my prostate, it still doesn't feel tender, but the lack of semen/ gel likeness of it leads me to believe it must be aggravated.. ? Prostate fluid looks normal though when milked.

I always ask this, do you find yourself involuntarily tensing your pelvic floor muscle? Like the muscle that's used when urinating to squeeze out that last bit

I do... but I think this is normal for everyone to some extent.

Trying to relax my pelvic floor hasn't really helped me... and I can almost guarantee that it won't/ hasn't "cured" anyone from doing this alone. I'm fairly certain my pain comes from muscles/ nerves I have 0 control over.

Hey @webslave or anyone else,

One of my more alarming symptoms is when I ejaculate, I get a feeling of incomplete emptying. I also have to milk my penis to get a decent amount of semen out of my urethra that didn't seem to make it out. Any idea if this is a muscular issue or an organ issue? Or potentially something else? Any input would be great! Thanks!

Well I have a pretty strong ejaculation force, but when I "milk" my penis after ejaculation, there is a decent amount of semen that comes out of it as well. This has always been the case for me, as long as I can remember, so maybe it is just a normal thing?

I would normally agree with this but its way more than how it was prior to all this. The amount of semen that actually comes out during ejaculation is very little.

Quick update...
Not really much improvement lately... ejaculation still weak and incomplete feeling. Pain is localized to in between my thigh and perineum.

I've been doing internal massage at least once a day, the area between 8 and 11 o'clock.

Can internal massage make the area more sore? I really don't feel much tension release, I just massage the sore spots, but it feels sore now.

During my last flare like 4 month ago I also didn’t shoot out any semen when ejaculating. It was more like drooling out and it wasn’t much neither. Now with my hardcore regimen (stretches, massage, sauna, diet, meditation) I orgasm almost normal again. So its reversible imo.
Age: 32| Onset Age: 30 | Symptoms: “Cured“ now, no symptoms. Symptoms were: 8/10 pain, couldn't sit, bleeding anus, ED, HF (tinnitus too from stress), Insomnia or CFS | Helped By: Stretching, foam roller, hot baths, sauna, tennis ball, antidepressants (acupuncture and fasting improved the tinnitus a lot) | Worsened By: stress, sitting, chronic masturbation but yeah mainly stress | Other comments:

Oh I am only taking supplements like magnesium. And Quetiapine (Seroquel) sometimes when I can’t sleep 25mg.

But I meant meditation as in meditating :)
