Thats funny you say that, because I engaged in a extended edging session before my condition was aggravated a lot. I already suffered from CPPS, but I suddenly had the great idea to bring more blood flow to my genitals... I did it by massaging my penis with oil for like an hour without ejaculating, I did that like 2 times that day and next morning I woke up with a almost lifeless penis, it was totally shrunken, bloodless and almost without feeling to the touch. I did not get an erection for several weeks. I think I must have totally fucked up a already strained pelvic muscle that day. Since than I have problems with premature ejaculation and decreased feeling on the penis. Its just weird because I did not use force or anything, I don't really think its a structural damage, more like a severe muscle strain or something like that.

Anyway I of course regret it in hindsight, was a stupid idea, but than again, it did not seem like something like massaging your (erect) penis for a really long time could do any damage. Anyway the pain along the urethra pathway was there before I did that. I only got the additional penis numbness and premature ejaculation.... not that I did not have enough to worry about.

So what is your theory why do you think this practice specifically could cause CPPS?
Age: 26 | Onset Age: 16 (but got much worse around 22) | Symptoms: hard, cold flaccid penis; reduced sensation/numbness; diminished erections; premature ejaculation; urinary frequency; burning urination; urgency (bladder and urethea inflammation); painful ejaculation | Helped By: Exercise, cold shower, internal massage, massage of inner tights, abstinence, stretching | Worsened By: sex/masturbation; working on the computer too long | Other comments: I am always worrying it is nerve-related and cannot heal naturally.

Because edging involved getting right up to ejaculation without going all the way through with it, and that is where those muscles are tightened, and other muscles tighten to hold back. However, I don't think you'd acquire chronic prostatitis / chronic pelvic pain syndrome from just doing what you did once, unless the groundwork had already been laid before somehow.

Alpha, try long, hot baths rather than cold showers, just to see if it helps. Don't worry about a cystoscopy. They very rarely cause any problems other than transient discomfort, and can be extremely useful in ruling out other conditions, such as stricture.

Oh yeah, I was going to mention, don't worry about the cystoscopy. I had one, and it was no big deal. A little sensitive when it goes through through the prostate, but just take a deep breath. If it's the flexible cystoscope, no worries. Easy.

Thats going to be a challenge. My urethra feels really inflamed and painful most of the time. I am scared as hell that it gets worse or I will feel unbearable pain if it gets irritated by the cystoscopy. But on the other hand I have those problems for such a long time now and so far nothing helped to get on the healing track. So IF a stricture or some bladder or urethra related condition is causing me all this pain, discomfort and dysfunction I guess I have no choice but to the cystoscopy.

Are there any documented cases of guys with CPPS, where a stricture or urethra injury of some kind was the cause of the symptoms? Because according to wikipedia the only symptom would be a weak flow.

I mean I have this thing, if a child or my girlfriend accidentally hit me or brush my penis I flinch, because its very uncomfortable and sometimes painful if I get touched like that on the penis. It feels like some kind of injury. And like I said, when I touch along the urethra rectally, the whole area feels inflamed and painful. Thats basically the only area I feel painful. Most of the traditional triggerpoint locations internally or externally I dont feel any massive pain or discomfort when massaging it.

Also my stream is relatively weak compared to healthy individuals and twisted. Can this ONLY be explained by stricture or also be a "normal" chronic prostatitis / chronic pelvic pain syndrome symptom?

Well, that's a tough call without doing specific examinations, and that's what the doctors are supposed to do. Seems like it could be either chronic prostatitis / chronic pelvic pain syndrome related OR stricture related. I don't know about "only".

The sensitivity to touch sounds like allodynia to me, nerves that are far too over-sensitized. My nerves are messed up in that area too. Nerves are strange and it's so difficult to find the smoking gun that began the problem.

Some guys have referred pain for years from hard to find trigger points that are stuck in the Charlie horse, chronically. And I'm a strong believer that trigger points and chronic strong muscle tightness can really goose the nerves. Even after the trigger point has been resolved, the pain can last for quite a while before it slowly fades away. There are so many factors that could be influencing your issue, so I think doing the scope procedure is smart to rule out any physical obstructions, growths, or deformations along the urethra. But urethra pain can be referred pain from the bladder and prostate tissues and muscles, I believe. I hope you get some answers and some relief!

The weird thing is that I guess I have some form of allodynia, because like if someone else suddenly touches my penis or surrounding area I wince. Also my ass and upper legs are super sensitive to touch, its unnatural ticklish.

But on the other hand my glans and penis when I touch it or let it be touched consciously I have very diminished feeling. The good, sexual feeling is almost completely absent.

So I am over-sensitized and numb at the same time, kind of. :S

By the way, obitoo I found a clinic in Germany which by the looks of it, now tries to treat patients according to the Wise-Anderson Protocol. Its a clinic in Cologne, Germany, the EBZ, Rectum- and Pelvicfloor Center Cologne: http://www.ebzkoeln.de/pages/en/disease ... m-CPPS.php

Did you hear anything about this? Treatment is 2000 Euro. I am really considering saving for that.

Don't be fearful of a cystoscopy. I have had 4 (some due to kidney stone recently), no problems. Really impossible to decide if you have chronic prostatitis / chronic pelvic pain syndrome without having it to rule out other issues (stricture, etc)

Hallo guys,

Finally I made some progress with my condition!

I took the plunge today and after a lot of hesitation and fear I finally DID the cystoscopy! I was REALLY afraid, but its good I finally did it.

Well I do not have a stricture, BUT the doctor told me that my urethra and my bladder are both chronically inflamed and changed. The bladder has more blood vessels than normal.

I told me, its no wonder that I feel pain during sex and during urination.

So after 10 years of pain and 10+ urologists telling me its in my head or I should relax more, finally a diagnosis.

Now, he told me to take a medication for 3 month, 1 Uro-Tablinen (Nitrofurantoin) every evening. He told me that should get the inflammation out of the tissue. But, according to wikipedia this active component is an antibiotic used to treat bladder infections, why is he not giving me a anti-inflammatory drug? I mean I trust the doctor, but he never found any bacteria, so I wonder if this is the right treatment.

What do you guys think about this? Could this chronic inflammation still be part of chronic prostatitis / chronic pelvic pain syndrome or could I be save to say now, that the chronic prostatitis / chronic pelvic pain syndrome treatment will not work for me anyway?

Nitrofurantoin (Furadantin, Macrobid, Macrodantin) is an antibiotic used to treat bladder infections. He is just covering his ass here.

Yes, it could be chronic prostatitis / chronic pelvic pain syndrome or IC (or a bit of both, because they are more or less the same condition). And yes, stress can cause it. Stress can inflame the bladder in cats, did you know?

So what do you think, how should I proceed here? First go with what my doctor says? Are there many guys with chronic prostatitis / chronic pelvic pain syndrome that also have Urethritis AND Cystitis? Any medication you could recommend to get the inflammation down? I feel like if I get this painful burning out of my urethra and bladder, than it would also be much easier for me to relax down there...

You guys told me it would be a good idea to do the cystoscopy, so what should be the next step after this?

CPPS can encompass pain and inflammation in bladder, prostate, urethra, even testicles. In the bladder it is called Interstitial Cystitis.

Yes, take the antibiotic, but I doubt you have a bladder infection (you've had the condition for 10 years, more or less). But don't allow the medics to give you any further antibiotics after this.

The cystoscopy has proved there is no stricture, which makes a chronic prostatitis / chronic pelvic pain syndrome diagnosis more likely (although the absence of pain is a contraindication, or to what extent do you have pain? Your sig does not mention it)

Come back after the antibiotic and we can look at the situation again.

I feel a lot of empathy for you, because I am partially struggling with similar conditions, mainly the general allodynia. The problem is that, there must be some underlying trigger points which are the "mothers of all trigger points", my problems started in that way that a day or two after prolonged sex/masturbation I felt a "lump" somewhere "behind the penis". This resolved after several days. But repeated again, before it became chronic and now sometimes the general allodynia of the penis/scrotum surface is bigger problem than that original big trigger point in bulbocavernosus/ischiocavernosus area. The edging definitely massively contributed to my condition. Despite having that, I had this year some good 6-7 periods, which are always ended by huge flare-up.

Well I and the therapist I visited never found a triggerpoint. My main symptom is really inflammation in my bladder and urethra and a inflamed feeling in the anus, pelvic region and legs, probably caused by the cystitis... I think in my case the allodynia is also to blame on the inflammation, I feel like my muscles in the region tighten as a reaction to the inflammation in my bladder, not the other way around. :S Anyway I also feel like sex/masturbation triggered the problem in the first place, but I guess its not something the doctor would agree with. ^^
