coldness from the penis extending to legs and feet.
I have this too, on and off. It seems my erections are worse when I have the cold feeling. Almost never have any ED-issues when the coldness is not present. I've had this symptom all the way from the start, and always believed it was caused by the muscles being so tense reducing blood-flow.

I have been meaning to ask here about it. Any veteran suggestions on how to reduce the effects of this symptom?

I've been trying to massage the bulbospongiosis and other perineal muscles, but that only has limited success. When I stretch my gluteal muscles and hip muscles (piriformis) it seems to raise the temperature in my pelvic area... Does the blood vessels pass these muscles?
Age: 41 Onset Age: 27 (dec 2006)Symptoms: Urethral burning, muscular pain and soreness around the base of my penis, perineum and muscles around my sitbones. Frequency, urgency, dribbling, general discomfort in the pelvis, decreased libido, tight levator ani (according to PT) and occasional levator- and sphincter spasms. Sometimes ache in penis, especially when erected. Latest addition is testicular pain (and epididymis). Also excretions of clear (prostatic?) fluid throughout the day as well as secretion in my urine. Varying power of urine stream. Redness on tip of penis and ballsack.Helped By: Positive thinking and controlling anxiety Worsened By: Stress, lack of sleep, heavy training (especially abs and legs), catastrophizing over this condition

The symptoms listed here are largely what I am experiencing now.

My issues are in the urethra (feeling of trapped urine, irritation, warm and cold feelings, etc.) along with some occasional testicular pain. I also have frequency (but not usually urgency).

My urologist did a cystoscopy a couple weeks ago and thinks this is all from primary bladder neck obstruction. The strange thing is, I have had two appointments with the urologist, and after both appointments (in which tests were done), I felt great for 3-4 days--then symptoms start coming back.

I have had symptoms completely vanish for 48 hours, just to return with a vengeance later. I can't determine what is "setting me off"--diet doesn't seem to have anything to do with it. Sex? Unclear. Ejaculation? Either makes it better or much worse.

I am pretty frustrated, because if it is chronic prostatitis / chronic pelvic pain syndrome proper, the only possible solution is spending tens of thousands of dollars going to special physical therapists thousands of miles from my home. If it is primary bladder neck obstruction, the solution is taking Flomax or surgery--and Flomax hasn't done much. I sometimes think this whole thing is a "throw away" diagnosis like Fibromylagia. Once you have it, you are SOL.
Age: 38 | Onset Age: 32 | Symptoms: Frequency, the feeling of having to urinate right after voiding, pain in the penis—especially on the tip, dripping, urine retention, mild burning. | Helped By: Hot baths, exercise. | Worsened By: stress, caffeine, possibly diet.

That's not true. Not everyone can afford to go to Stanford or even to see their local physiotherapist/PT. Such people usually get by with a copy of A HEADACHE IN THE PELVIS and a wand of some sort, combined with various medications, and various relaxation techniques (from books, from CDs, from techniques taught by Wise), and some even make lifestyle changes to avoid stress.

I do have the book--maybe I should start seriously reading it!

The frustrating part of this is that lifestyle changes don't seem to be doing anything. I can go on a macro-biotic vegetarian diet and avoid alcohol and caffeine and feel miserable. The next week I can eat Mexican food and booze it up and feel better.

I'm sure stress plays a part--so I need to work on that as well.

Booze make some men relax, so keep that in mind. It's a muscle relaxant, but not worth the side effects for most men.

Have you really made lifestyle changes? A mere dietary change is not a lifestyle change.

I am so afraid!

Post by Alphalifestyle »

Hey everyone, I wrote here before about my penis problems, but now they got so much worse, I am really afraid of losing my penis!

Alright that's what happened:
I suffered from painful ejaculation, pain after ejaculation, feeling of tightness in my lower belly/pelvic probably since I am 14. I am pretty sure I suffer from CPPS. My main symptom so always was, what I would describe as reduced bloodflow to the penis, because my penis was often shrunken and hard when it was flaccid, looked white, I didn't had a lot of erections and it just felt like my muscles were so tight and rigid, that I just didn't get enough bloodflow to my penis.

But that was all something I could live with.
Fast forward to approximately one year ago, my condition changed dramatically. It all started with a "harmless" zipper injury. I injured the tip of my penis on a zipper. It didn't bleed, but since that time, on the right site of my penis tip the skin seems to be loose, there is like a part of the skin that seems to be easily moved around, kinda like "extra skin" when you are to fat. Difficult to describe, but since than I feel kinda numb and painful at the penis tip. For some reason the skin of my glans and shaft also feel kinda sticky all the time, now this soft skin on the tip always sticks to my undies or something and when I undress looks really weird or the hole of my penis stands wide open...
Sometimes after sex, the skin there looks swollen.
The skin there seems also to be so sensetive now, that it already started bleeding several times just from walking around, when the skin sticked to my underpants.

Now over the last few month my glans penis got softer and softer. It feels so soft and mushy it scares me. It feels like the slightest force could rip my glans apart.

It feels painful on the tip and when I press it. I think the pain is still from the injury. All urologists and dermatologists I visited dont find ANYTHING. They say it looks a little unusual but its basically nothing.

Right now my penishead feels basically empty, i can almost not feel it, like there is almost no more feeling, its super soft and almost feels like the skin on it can be moved around. It burns, is numb and the tip hurts. Its often cold and feels like there is bad circulation.

I am afraid that all these symptoms are caused by tight muscles and prevent this small injury from healing and that now the tissue is kind of damaged. It feels like my penishead is dying of it gets worse and worse.

PLEASE help me. Is there anything I can do to increase the bloodflow? Can this really be CPPS? Could chronic prostatitis / chronic pelvic pain syndrome weaken the penis through restricted bloodflow so much, that its easier damaged/can not heal anymore?

This is like a nightmare, I feel like a get a panic attack (and I wasn't like this before), because my penis never felt so weak like today. I am almost afraid of moving...

Please, if someone has an Idea, what I can do I NEED HELP. I think maybe internal massage would be a good idea, but I live in Germany and I dont have any money right now. I cannot afford any Physical Therapy.

Here is a picture of my penis from 4 month ago, maybe you can see the loose looking skin at the tip:

Please help me.

I'm not a doctor but it looks pretty normal to me and you are stressing and making your condition worse, painful ejaculation and post ejaculation are signs of chronic prostatitis / chronic pelvic pain syndrome and I have them both so your not alone there

Loss of sensation and erection dysfunction are also common you will find many answer on the this board search all key words for questions,

Webslave will be be able to answer your questions a lot better, he's the man :agree:
Age:33 | Onset Age:19 | Symptoms:burning urethra, deep sore stinging around the perineum, dribbling | Helped By:when flare starts deep relaxing over a toilet to let out dribbling urine, hot bath, rubbing a heat cream on the perineum, Tens with a pad on the perineum, running, medication (solpadol) for pain. Current treatment: PT. | Worsened By:sitting, stressing over it, ejaculation, sometimes alcohol but I risk it. Been feeling a lot better last 6 months about 85% with a few flares lasting about 5 days. No longer having PT. Stretching every other night and I believe time is also a big factor for me

Yeah, I know, the picture doesn't look spectacular, but that's the problem, the urologists also don't see anything...

My main problem is not painful ejaculation, erectile dysfunction or something. BUT SEVERE softness of the glans, it feels as week as a piece of sponge or something, almost total feelingness, but pain when I touch it, especially the tip. Its cold and retracted. It just feels completely strange. I am really afraid, that it is dying off or something. It got extremely worse in the last few weeks.

Could TMS really create these symptoms:
- extremely (!!!) soft, mashy penis head to the point of being afraid that it gets hurt easier
-coldness in the penis
-numbness in penis head
-sticky skin on penis and penis head (even after a shower)

I also feel like my muscle tensions got worse recently: my nightly teeth clenching got worse, I feel more and more out of breath, my anus and the skin around it feel extremely irritated.

Is there anyway to make my glans feel alive and health again? Is there any triggerpoint known to restrict bloodflow to the penis tip or creating numbness in the penishead?

Hi,

I am going to defer to Webslave on the bulk of your questions, but I wanted to quickly address a few points.

Firstly, try to calm down. Anxiety is not going to help your muscular tension. I realize that you are very alarmed by your symptoms, but take a deep breath and realize that what will be, will be. No way around it. I do this when I have lightning bolts in my genitals/rectum and start thinking about crazy causes!

Secondly, I am not a doctor, but I really doubt that your penis is dying. That would be highly unusual and, I think, borders on a catastrophic thought. Coldness and loss of sensation are reported symptoms, in the literature and on the boards. Contraction is a good sign of muscular activity, too. Much of the penis is spongy (including the glans), but it's often forgotten that a sheaf of muscle does extend a fair distance up the shaft.

Finally, the fact that urologists can find nothing wrong is pretty much par for the course with CPPS. They rarely do. I've even had very good MRIs done, without result. Whatever else may be wrong with me, the only thing I've ever tested positive for is pelvic floor dysfunction.

You may be in the throes of a flare, but that's a guess. As I said, I'd like to defer to Webslave's knowledge for your case. Of course, go to the doctor or emergency room if you feel that your condition warrants it.
I am not a physician. This is not medical advice. Consult your doctor!

Age: 26 Onset Age: 17 Symptoms: Shooting, nerve-like pains throughout the penis, which abruptly hit and leave. Testicular pain, perineum pain, burning/irritative urination, extended pain after ejaculation. Occasionally, some allodynia or ache in the coccyx/sacrum/thigh/buttocks/legs. Diagnosis: Pelvic floor dysfunction, degenerated lumbar disk, and mildly herniated lumbar disk. Helped By: Physical therapy, pain management doctor, hot baths, therapy pool, stretching regimen, breathing exercises, relaxation, distraction. Worsened By: Arousal/ejaculation (worst), constipation, panicking/obsessing, other triggers depend upon current symptoms. Tests/Prior Treatments: Too many antibiotics to count, multiple urine tests (all normal), testicular ultrasound (normal), bladder and renal ultrasound (normal), lumbar and pelvic MRI with and w/o contrast (revealed disk problems), Elavil 25mg (caused retention), Flomax 0.4mg.

You've been assured by medical men that there is no obvious organic problem, and the rather poor photo certainly does not show anything obviously wrong.

The vague nature of your complaint (mashy, soft glans, moistness, etc) and the normal appearance of the organ, coupled with your clear psychological distress and panic lead me to believe that while there is definitely an element of long-term CPPS, most of the problem is now psychological. If I had to put a name on it, I'd say it could be a case of body dysmorphic disorder. This is an anxiety spectrum disorder not too far from OCD, which itself is common in chronic prostatitis / chronic pelvic pain syndrome patients.

Your next visit should be to a psychologist or psychiatrist.

@webslave: No I dont think so. I don't care about how it looks at all or what others think about it, the pain, numbness and discomfort are my main symptoms. I know that some of my symptoms are strange, but aren't many symptoms of chronic prostatitis / chronic pelvic pain syndrome sufferers rather strange and could also easily labeled as psychotic, were they are clearly not?

There is a clear visible change in the appearance and I have a constant mild pain. If the tip on the penis is touched or irritated during sex in becomes an strong pain! I think most of the other symptoms which you label as of vague nature are probably caused by the over-irritated nerves. I just dont know in which relation these symptoms stand with my chronic prostatitis / chronic pelvic pain syndrome problems.

My panic resulted from an increased numbness and softness of the penis, yesterday, after I hurt again at the tip from walking, it became cold and almost white, as if there was a severe lack of bloodflow...

Furthermore... I have real functional problems now, the tip of the penis feels so irritated during sex that it sometimes hurts so much that I loose my erection, I can loose my erection if my girlfriend touches my penis or stimulates me orally, because for the most part that creates only pain now. NEVER had that problem before. Also, it seems like there is some nerves involved, because as soon as the tip get stimulated during intercourse I feel like getting an ejaculation, but it feels wrong, its not building up, it also doesn't feel like normal premature ejaculation, I just come without even an orgasm.
Walking is painful because its painful if the tip touch the pants and the tissue there often sticks to the undies and even bleed just from walking 2 times... still the urologist insist there is nothing unusual... thats what creates my mental distress: MY PENIS feels completely different, sex is very difficult and frustrating, my penis tip bleeds from walking and no one is willing to look into it!

I had pictures where the whole tip of the penis locked swollen after sex, my urologist confirmed that it looked not normal, only in the normal condition, it looks pretty normal. Unfortunately I dont have the picture anymore....

You don't understand body dysmorphic disorder. In most cases, the sufferer is obsessed with what they themselves think about a body part, not what others think.
...the pain, numbness and discomfort are my main symptoms.
As I said, you do seem to have CPPS, and you say you cannot afford treatment, which means you must treat yourself (and we have lots of advice here on that. Have a look at posts by CPPSdad).
I know that some of my symptoms are strange, but aren't many symptoms of chronic prostatitis / chronic pelvic pain syndrome sufferers rather strange and could also easily labeled as psychotic, were they are clearly not?
Your psychological symptoms are not "psychotic". Please don't use terms like that without knowing what they mean. But I strongly suspect you have an anxiety spectrum disorder in the region of obsession and fixation. I urge you to visit a professional and have my suspicion verified or disproved.
I had pictures where the whole tip of the penis locked swollen after sex, my urologist confirmed that it looked not normal, only in the normal condition, it looks pretty normal. Unfortunately I dont have the picture anymore....
The updated picture you provided above does look a bit unusual. A protuberant urethral meatus accompanied by bleeding could, at worst, signify urethral carcinoma. Have you had a cystoscopy?

Re: Odd glans penis pain/ CPPS?

Post by Alphalifestyle »

Thank you, this was the more differentiated answer, I was hoping for! ;)

Actually I think my problem is a mix of the 3 components you already noticed:
  1. Some form of Post Traumatic Stress Disorder or Anxiety with a tendency to hypochondria. I am pretty sure this originates from an injury I had when I was 14. I had a Pneumothorax, a accident were one of my lungs collapsed and my life was in danger. Until that event I thought "I can not become ill, serious stuff happens to others, not to me". From that event on I felt that my body wasn't reliable, I felt so vulnerable and with every little twitch or problem I was afraid I have something life threatening. From that time on I got more and more "little" problems which are on the border to psychosomatic problems like: breathing problems, visual disturbance, tinnitus, muscle tension, muscle pain, knee pain without reason, digestion problems and worst of all: My chronic prostatitis / chronic pelvic pain syndrome problems which manifest in a broad spectrum of symptoms.
  2. The chronic prostatitis / chronic pelvic pain syndrome disorder, with probably has it origin in my anxiety, but maybe not. I definitely have extremely tight and painful muscles in my pelvic, which not only cause painful and frustrating sexual problems, but also make my whole body core feel tight and restricted. Probably cause digestion problems and difficulties to breath freely. Even if the origin is the anxiety/PTSD, I probably have to treat the tight muscles, because trigger points won't disappear by them self (or would they?).
  3. Is probably a real, but small injury of the skin at the tip of my glans. I am pretty sure that in a normal healthy male this would self correct or not be so troublesome, but because my penis feels irritated already from the chronic prostatitis / chronic pelvic pain syndrome and maybe there is some problems with blood circulation or over sensitive nerves or SOMETHING, this small injury produces very painful and annoying additional problems now. For me it really feels like a piece of the skin on the tip got separated from the tissue under it and is now kind of loosely lying on top, like a air-filled blister, that collapsed, but the skin doesn't die of, but instead is just lying on top of the tissue without being really attached to it... if that makes any sense.

My plan:
  • I will go to 1 more urologist and dermatologist and let them look at the "injury" again. If they also don't know what to make out of it and can at least guarantee me there is nothing serious, nothing to worry about I concentrate on 1 and 2.
  • I will then look for a good therapist to finally treat my anxiety/trauma disorder.
  • Next I will read the 5th edition of HITP and search the forum for method of treating the triggerpoints by myself.

That seems like a good plan. Trigger points won't go away on their own unless you completely remove the cause, and since that is usually psychological, it's usually impossible. So treating them is a good idea.

Self-Treatment: Resources and Routine

Post by Alphalifestyle »

Hallo everyone,

After having what seems to be Pelvic Pain Syndrome for many years - which in my case shows itself mainly through numbness and pain in the penis, chronically irritated, dry and changed skin on the penis head, frequent urination, pain during and after intercourse and a general feeling of burning and tightness in the anus region - and a worsening of the symptoms in the last 1½ years - I read the newest version of Headache in the Pelvic and I am more determined then ever to take the treatment of this frustrating condition in my own hands now and make healing happen.

Well, like probably many others I am not able to visit the Clinic in the US for the education in Triggerpoint Release and Paradoxical Relaxation so I want to start treating the whole condition on my own now.

I need some advice and guidance of people who are already experienced in the whole process so.

My plan was to treat the condition in 4 ways:

1. Going to a good Cognitive Behavioral Therapist to get treated for Anxiety and in my case especially Health Anxiety.

2. Start doing regular Feldenkrais Sessions to increase Body awareness and relaxation.

3. Buy the new Wise book on Paradoxical Relaxation and try as got as possible from a book to follow the protocol.

4. Most importantly: The Self Massage:

This is were I still feel most clueless, even after reading the book. I experimented with using my own finger and some oil to do internal massage and found that especially just a few cm inside around the anal sphincter muscle there is a lot of soreness and painful muscles.

I couldn't find any Triggerpoint yet, but almost every spot inside feels uncomfortable to touch, inflamed and sore. I also found several places were I found hard muscle tissue, like long strings or pieces, not just little knots, almost 2 or 3 cm long. Could this also be from chronic tensioning? Even after weeks of massaging them they would not become soft.

Sometimes I have a little blood on my finger, even so I massage not to hard. Feels like the tissue inside is super sore and sensitive, but I don't know if a little blood is normal? I also sometimes have a little blood after wiping my ass.

I seem to also have tight muscles higher up the pelvic floor, but I can't quiet reach there with my finger alone. Should I use a tool?
I am afraid so to use a tool inside my body, because I don't know how deep I should put it in, where I can and can't apply pressure, etc. Any guidance?

So please recommend me one or two tools I can use for internal massage. And which kind of oil can I use for internal massage?

Any routine for the actual massage or do I just have to experience and find my own method?

Because I hold so much tension in my Anus region, so much that my anus feels almost numb, but also super sure, I thought maybe I can use some kind of vibrational tool like this, to loosen the region up a bit: http://www.amazon.com/gp/product/B001M5 ... ag=cp58-20

Looking forward to your responses.

Yours,
Ken
