Weekly update since I had a PT visit today. My symptoms remained high for most of last week. On Saturday I woke up and was much better. 80% symptom free. Then Sunday came and I was 99-100% symptom free. On Monday during the day I was back down to about 70% and in the evening I was back to like 10%. Today I am probably around 30%. It's kind of frustrating that my upswings only last a few days. The overall trajectory of recovery isn't that good at this time.

PT mentioned most things were much better though I had some hamstring and lower back tightness left over from building a fence this weekend.

I also read Ending Male Pelvic Pain (EMP) and while I liked A HEADACHE IN THE PELVIS I found EMP more useful when it comes to recovery techniques. They actually reference A HEADACHE IN THE PELVIS in the book which is kind of weird.

I haven't quite figured out what is causing the flares up to come up. I thought last time it was decaf coffee. Over the weekend I drank some caffeine free diet soda but I didn't really have any symptoms until 48 hours later so I am not convinced that was the cause. Possible I have some crystal light packets at work that I used on Monday. I threw them away already and will stick to regular water.

Current Treatment Plan
  • Stretches x2 a day
  • Foam Roll everyday
  • No caffeine or decaf coffee/tea
  • No Spicy Foods
  • Pilates routine outlined in EMP twice a week
  • Yoga routine outlined in EMP twice a week
  • Core workout from EMP once a week
  • EPR once a day
  • Aloe vera supplements twice a day
  • Magnesium once a day
  • Practice diaphragmatic breathing as much as possible.

This week has been great. Aside from Monday evening and and Tuesday morning my symptoms have remained at a 2/10 or less. Last night I decided to indulge in a diet root beer. I ended up waking up to pee many times at night and my frequency is up to a 5/10 today so looks like diet soda is likely a trigger. Not a big deal since I rarely drink soda anyway but I am kind of hoping when I am clear of everything for a few months I can go back to the occasional diet soda.

Weekly Update: Symptoms were mostly mild, say 1-4 Monday to Thursday. Thursday evening things went up to about 5-6. Back down to maybe 4 this morning. I can't think of a trigger since my diet was more or less the same all week.

I am moving paradoxical relaxation down to 20 minutes a day. It just isn't feasible to do all the required self-care, work a full time job, get the mundane chores/house things done, have some recreational time and fit an hour + of meditation in.

Do triggers ever go into remission?

So the list of things that seem to make my problems worse is getting rather hard to swallow. Has anyone experienced that maybe after 6 months to a year symptom free they can reintroduce some triggers?

Finding out cycling is out of the picture sucks.

Well I cannot go to hookers anymore as this scares me and therefor causes me CPPS. But I think anything physical you should be fine once your muscles recovered.

I thought I posted my last comment in it's own thread, guess not.

Weekly Update: been up and down but haven't crested above a 5/10 so that's good. Right now I am probably around 3-4. I am thinking about quitting my current PT. She is great but it's super expensive and it's across town so counting appt time and commute time we are looking at it taking 3+ hours. There is a place that is in between my home and my work that I went to for one appt that is much cheaper. I am going to give them a go again. I am also going to do either every other week or maybe even once a month.

Also my urologist finally got back to me and said he would do a microgendx auth form so I ordered that this morning. What prodded me to get it is... and if you are queasy stop here btw, is that I am producing yellow semen.

Yellow semen can be normal, usually just a sign of inflammation (pus cells, aka white blood cells or WBCs, aka leukocytes). Cows milk has quite a few of these too, but we drink it, go figure (cow's milk contains an average of a million pus cells per spoonful of milk)

Well, not really 'normal' for it to be a thing. The real question is whether the inflammation is caused by an infection or not.

Yeah, but enough of a thing to let me rationalize spending 200 bucks on some more advanced testing. I am 80% it will come back with nothing but it will at least help 100% write off what's going on. I would feel stupid if I am doing all this self-therapy and have it simply helping out with the symptoms of an infection.

Weekly update is I am hanging out at a 2-4/10. I have't had PT in like 3 weeks and am considering cancelling it completely since I can pretty much do everything she does now.

I saw my primary and she thinks testosterone replacement therapy (TRT) has given me an enlarged prostate which kind of upset me since I had already told her the urologist didn't find anything wrong with my prostate and he took over my TRT. I guess the FDA's issued a warning about TRT possibly causing enlarged prostate issues but urologists and studies don't seem to support this. Just another way men's health is in such a shitty place.

Got my results back. Medium bacteria load (105 - 107) 99% of the bacteria was bacillus cereus. That is odd since bacillus cereus is commonly associated with food poisoning and is found by testing the food, stool or vomit. Instead they found it in my semen and I haven't had food poisoning in many years (maybe never, actually). I booked a follow up with my doctor.

Bacillus cereus is an interesting bug, and one I've had problems with through following a gluten free diet. Turns out that the go-to food for replacing glutinous foods is rice, which is full of B. cereus spores. These spores can survive cooking and once in your gut, start multiplying, eventually causing tenesmus (cramping) and diarrhea. I had repeated mysterious bouts of painful diarrhea until I eventually worked out that the delicious rice flour pancakes my wife was making me were infecting me with this bug.

To kill the spores, you need to follow some guidelines:
While B. cereus vegetative cells are killed during normal cooking, spores are more resistant. Viable spores in food can become vegetative cells in the intestines and produce a range of diarrheal enterotoxins, so elimination of spores is desirable. In wet heat, spores require more than 5 minutes at 121 °C (250 °F) at the coldest spot to be destroyed. In dry heat, 120 °C (248 °F) in a sterilizer for 1 hour works for rice, for instance.

Just in case anyone wonders what the results look like from MicroGendx below is a screenshot of my results. I talk to my doctor on Thursday but I suspect she will refer me back to my urologist. I kind of hope they just don't do Cipro again.

Actually doctor called me instead of an appt and is putting me on levofloxacin but wants me to make an appt with my urologist.
