Question about inflammation and diclofenac

Post by User-CPPS »

Can someone explain to me how there can be inflammation without bacteria? How can you treat inflammation? Why is diclofenac not helping?
Age: 41 Onset Age: 14, 22 got really bad
Symptoms: pain and discomfort perineum, a little bit while sitting in the right half of the scrotum. Sometimes dual stream while urinating. While ejaculating, not all shoots outside, some stays and flows outside with time (Without medication very painful).
Diagnosis: have seen several urologists in my life, have done any possible and pain creating diagnosis could not help me ever. Life was hopeful after googling the Stanford/Wise-Anderson Protocol in early 2008, then every ache started making sense./ Hemorrhoids: they stress my prostate when I have a Hemorrhoid flare!
Helped By: Physical therapy, keeping myself warm, hatha yoga, stretching, walking, relaxation, distraction, Tadalafil 5 mg
Worsened By: Constipation, Fear or OCD "flare up", sitting and not doing any kind of "motion", too much tension in media or life, caffeine and acidy food
Prior Treatments: Antibiotics, Tamsulosin, Lyrica, Gabapentin, Tylenol

Nerve endings can cause inflammation, through overstimulation. It is too complex to explain, but look up a condition called 'complex regional pain syndrome' (CRPS) to see how bad it can get. Allergies can cause inflammation. There are also many other causes, including stress, injury, chemicals, etc. The treatment of inflammation depends on the cause. In the case of men with UCPPS, the cause is psychoneuromuscular mostly, so those are the things we target.

How long have you used the diclofenac? Diclofenac oral usually begins to work within one week, but in severe cases up to two weeks or longer may pass before you begin to feel better. The topical gel can take up to a week to start working. It only helps arthritis (joints) and pain in muscles and ligaments, not discomfort in organs or genitourinary structures.

Thanks for the heads up. So the muscular tension leads to inflammation in the organs.

I have to give a semen sample regularly at the moment and there is inflammation every time. I took diclofenac capsules daily for 4 weeks, still I have inflammation and now that my CPPS symptoms became worse so much I am afraid the inflammation is even higher.

Admin comment: As I said, diclofenac / Voltaren will not help organs and genitourinary structures.

Dear Webslave, how can we beat the CPPS inflammation? Is it so? Beat CPPS and thus beat inflammation?

One point though: If diclofenac treats the muscles which cause inflammation in the organs wouldn’t the Diclo help in that way? Or is the pelvic floor "immune" to diclo?

Voltaren is simply not good at treating UCPPS. Search the forum and see what others have experienced. It is a NSAID and mainly treats pain, but is ineffective for UCPPS pain, for most men. It's not good for treating trigger points. NSAIDs are only slightly effective in CPPS. But there are some positive reports on the forum, so try it out, is the best advice I can give.

Agree that Voltaren, Ibalgin etc. are pretty useless. After 2 really good years (almost no pain) I am struggling with another very nasty flare up, where the neurogenic inflammation plays dominant role (neuropathic stinging/burning pain in scrotum, groin, base of penis etc., more right side than left). Sometimes I think that supressing this upregulated nerve system is even more important than the eradication of trigger points. Sometimes in the past I felt that my pelvis has some trigger points inside (tightness) but it was not manifested as a real pain, only as minor discomfort. But once this mess overreach certain threshold of neurogenic inflammation, the hell breaks out. Needless to say that anxiety is always a fuel. Relaxation, meditation, rest, and maybe Amytriptilin (which I resumed these days) might help.
