I’m new to this forum and grateful for the wealth of information as well as the personal experiences shared here.
I’m 43 yo. On a Friday night three weeks ago, I began experiencing nocturia coupled with the feeling of not being able to empty my bladder. By the following Monday, I was already so miserable I made an appointment with a top urologist in LA, who gave me a PSA (my score was low) and a dígital rectal exam and concluded I had bacterial prostatitis, even though the culture came back negative. He put me on 10 days of doxycycline.
My symptoms went away for a few days but then quickly came back. Specifically the feeling of not being able to empty my bladder at night. It seemed to be triggered by spicy food and alcohol.
I reported this to my doc and he doubled down on the antibiotics and put me on a 20 day course of Cefdinir. This time I only had one day of symptoms relief and then the nocturia and not being able to empty my bladder symptoms returned. I should point out that it’s strange he didn’t put me on Cipro, since the docs consider it first-line.
Anyway, having read this forum, I’ve begun to wonder if I might have either CPPS or overactive bladder. The issue is I don’t have a lot of the excruciating symptoms (pain in the penis or feeling like I’m sitting on a golf ball) described by many of you. I continue to feel almost completely normal all day and then suffer from frequent urination and not emptying bladder at night. When my bladder is full after drinking a lot of water, I do have some discomfort in my pelvic area (on the right side), but it goes away once I urinate.
Based on what I’m describing, does anyone think this is CPPS? Or is possible the doc is right and this is bacterial? Would love to hear any of your reactions. And I completely understand that most of you are not medical professionals.

Unless you have a recent history of unprotected intercourse with strangers, I would discount the bacterial prostatitis theory entirely. Your reactivity to spicy food and alcohol, frequency, discomfort when bladder full all point to UCPPS, probably the IC/BPS variety. Have you experienced unusual stress recently?

I have been dealing with some next level career stress. Rather than have my urologist continue to poke and prod me, I’m going to try PT and see if that breaks the spell. Any other advice is greatly appreciated.
