A little different situation, my case
Hello, I'm 35 yo, and, 9 months ago, I started having a burning feeling in my urethra and reddish tip of penis. In 2 or 3 days evolved to a strange feeling in the perineum area, a bit swollen, some pain and also a feeling of swollen and sometimes painful testicles, and severe anxiety about it. The usual, I believe.
Contacted a friend urologist, who talked to me for a while and told it was probably prostatitis and prescribed me azithro and ofloxacin. Symptoms didn't improve with the antibiotics.
Never had fever or felt feverish, never had running/dripping/secretions, nor augmented lymph nodes but had reddish, irritated glans sometimes, had burning in the urethra sometimes, not related to urination or ejaculation, ejaculated normally with a healthy appearance and no strange smell, urinated normally, and had a strange tingling on the tip of my fingers (strange, maybe anxiety).
Later consulted a prostate specialist, who also thought it was chronic prostatitis and prescribed urine exams and cultures, all normal, no growth, no blood, no leukocytes, normal pH, nothing abnormal, but symptoms persisted. Took 2 more different AB's for some time to no improvement. Symptoms came and went independently of AB, always with a lot of anxiety, specially about the prognosis.
I was examined often, he never found any clinical signs of bacteria. I also developed an upper pelvic mild pain, doctor told me there he didn't see the need for me to take any more AB, he prescribed alpha-blockers and over the time the prostate feeling of swelling mostly disappeared and as a side effect of medication I stopped ejaculating.
This week, at the doctor's appointment, since the feeling of swollen prostate was mostly absent for some weeks and I had taken alpha-blockers for a few months, urine flux was normal (made a urofluxometry test, not sure this is the correct name), the doctor told me to stop the medication and keep going with my life, taking only serenoa repens (saw palmetto, permixon is how it is called here) and hopping it would keep the progress and go away, also told me he didn't see a reason for me to return unless something relevant changed for worse, since I was on the right track.
Now I started ejaculating again, but it is strange, since it appears to have more volume than it used to have, is less gooey than I remember, more liquid, and the last part that comes out has a different color, something more of a discreet yellowish white that is worrying me, not real yellow, but not pure white either, somewhere in between. Normal smell. Could it be related to the fact that I didn't ejaculate properly for months? Or something of a medication withdrawal effect?
The remaining symptoms appear to be actually better, but could just another phase... I still feel the upper pelvic pain, some groin pain, nothing too bad, the occasional urethral burning, no testicle pain, and the anxiety about it is still here... I don't really feel cured...
Truth is, there was never actual evidence of bacteria, there where mostly symptoms that I described, nothing that could be measured or detected, not even the swollen prostate, there was only what I felt, because the doctor didn't think it was augmented to the touch, and PSA values were normal, (although he believed every word I said I felt) and his opinion is that, if there were any bacteria, that started the process, there isn't anymore, and the symptoms are being triggered by neurological reasons, due to stress and other related motives.
Any thoughts? Anyone had the same symptoms or have something to add here?
Would really appreciate opinions. I'm kind of lost here...
Thank you, and sorry about my poor English, it's not my native language.
Cheers

Re: A little different situation, my case
The yellow in the semen / ejaculate is white blood cells, and it denotes inflammation, which can be symptomatic or asymptomatic. Many normal men have a yellow tinge to their ejaculate, although the most common color is white to grey-white. So this is not really a significant feature. The color appearing late in the ejaculate may indicate an origin in the seminal vesicles, possibly.
I suggest you stop the saw palmetto and take quercetin instead. SP is not a recommended medicament for this condition.

Forget about bacteria.

Thank you for the answer, I'll look for quercetin then, I also don't think saw palmetto is doing anything really. Cheers!

Exact same situation for me. Can't point to anything specific that has been helpful in my case. Pain seems to be reducing very slowly over time. Any specific event you attribute to the onset in your case?

Re: A little different situation, my case
Well to tell you the truth, the pain has been reducing, haven't felt any pain on testicles lately (a week or so) only on the groin and upper pelvic area, but I think the psychological part has been decisive, I'm actually convincing myself it will go away and it actually is going away. Still feel some pain/swelling but I'm fighting that feeling, if you know what I mean, trying to ignore it and keep doing everything normally and not think a lot about it, not letting myself be affected by it. Yesterday, for instance, I was totally asymptomatic, today, more or less, but since I'm a bit tired, the strength of will is a bit affected.

Also, ibuprofen seems to help, although I'm not taking it anymore

Re: A little different situation, my case
Just to give some update on my situation, for the ones who have similar symptoms.
It's been over a year now, I have asymptomatic periods, sometimes a few weeks, and some symptomatic periods, that usually have their worst phases during 1 or 2 days, start with heavy anxiety, usually unjustified, and then start to get better slowly, over the days.
Most of the time I have mild symptoms, like some urethra burning on random days (again not related to urination or ejaculation), some upper pelvic pain/stiffness, and recently some testicular tingling (feel them moving, like when you have goosebumps, no pain or swelling at all, very weird
).
Never had real relevant pain again, and the perineal area stopped feeling swollen or stiff for good.
Stopped most medication, but sometimes I take 0.5mg diazepam, for the muscle relaxing effects and the anxiety relief, mostly in SOS, when I have worse episodes.
Mostly it became a tolerable condition, that I can live with, but still very unpleasant in worse phases, specially when I am alone or less busy and notice the symptoms with more attention.
The awareness of the somatic part, and that it is not bacterial helped a lot to improve the condition overall.
Hope this helps.
Cheers

My semen has been yellowish ever since all this started for me. Never any infection found. No clue what causes it. Must be inflammation somewhere, not sure how to reduce it!

Re: A little different situation, my case
I'll add another development, an unpleasant one, for which I would appreciate some opinions and feedback.
As mentioned before, my symptoms have been mostly absent and I now realise they are closely related to anxiety.
Yesterday I started the day a bit anxious and when I went to take my morning shower, I realised 3 or 4, distinguishable, very noticeable, about 1mm or less, raised red "pimples" on the right side of my penis glans, along with the, not so rare, reddish urethra opening and a discreet but constant burning feeling.
My anxiety rose up to the sky, and some other unspecific symptoms came along.
It's not something I can spot from afar, but it is easily distinguishable when I look directly.
I don't think it is herpes, because they are spread out on the right side of the glans, not in cluster, don't seem to be round "fat" or full like the usual herpes lesions, and some seem to have a little white spot on the middle.
Today I noticed they are a bit more discreet than yesterday but there a few more to count, still red urethra opening and a discreet but constant burning.
I though about a stress rash or fungal infection, but I also found weird for it to be only on the right side. I'm applying fungal cream, just in case.
Also I'm not sure, but I think I had something similar one year ago, when the whole thing started, and it slowly disappeared, not sure it was the same.
I'm really really worried that I overlooked something during my prostatitis treatment or that I and the doctor misinterpreted the symptoms and it is something else that is getting worse now...
Can you guys me some opinions on the matter please?
Thank you in advance.

Re: A little different situation, my case
Probably nothing. Worrying about it so much may be a sign of hypervigilance

Thank you for your response, I believe I may be hypervigilant, due to worriness, but at the same time I can't help to notice 5 or 6 very distinguishable persistant red raised pimples. They are actually there and after 2 days with fungal cream are very similar... No improvement.

Could be bullous impetigo.

Re: A little different situation, my case
It's an hypothesis, but I don't think the pimples are filled with liquid or resemble the usual impetigo ones, I'll wait a few days and give further feedback.

I'm currently leaning more toward thrush, candida or an erythema of some kind... Not sure...
