Get asked a lot what my management plan has been with this so thought I would do a post so its up here in the future. Disclaimer: one size does not fit all, this is what I did and do and seems to have helped me. Secondly, my pain was penis pain, not rectum. The basics: I do stretches from Headache in the Pelvis and Amy Stein. Video by Uptown Mike covers the basic ones to do
I try to do the stretches everyday. Every second day I do 20 to 30 minutes on Crosstrainer. If your in a flare, avoid weights and ab work. Mental Health: I took Duloxetine for a while to help with my mental health, took about 4 weeks to kick in but it reduced my anxiety over this. I'm not saying anti-depressants are for everyone but get on top of your mental health. Mediate every day for about 10 to 30 minutes - lots of good mediation videos on Youtube. Change your mindset, don't let the pain and discomfort dictate you. This is probably the hardest part - to ignore the pain and get on with things but you will find most people, the moment they accepted the condition and got on with life, the pain reduced. Flares: Work out what flares you, for me it was sitting, so a donut cushion was essential. Any donut cushion will do. Sex: for me reducing porn, masturbation and sex helped. I reduced sex/masturbation to once a week or fortnight, avoid porn when flaring as it stimulates the pelvic area and those muscles. When having sex, don't overdo the thrusts as this can cause a flare, just try and relax. Have a hot shower immediately after sex and if you flare after, have a bath. Supplements: I take Magnesium Glycinate and Ashwagandha every day. Time: don't set a time period that you expect to heal, don't set yourself up for failure. You may need to accept that you will have this issue for the rest of your life but it will fluctuate and can be managed. Other: others talk about trigger point therapy and other methods including hours of physical therapy and exercise but the above is what I did and seems to have helped as currently in no pain, I mean NO pain. I can't stress enough that you need to find a management plan that works for you. It a bit trial and error but you will find it. Once you have exhausted the medical field, try to look at this as a muscle issue that you need to work on and stop trying to find a medical solution as this will most likely send you down a dark hole and obsession over the condition. Hope this helps.

Re: My Management Plan
Great post. I do a lot of the same things including magnesium, ashwagandha, stretching. I have found CBD oil to be excellent for calming anxiety. In my particular case, I know exactly what set-off the cascade of events. It's been 1 year now and as my nervous system calms down, my pain calms down. I'm starting to feel like myself again, whereas for the past 10 months I've been borderline obsessive compulsive regarding this condition. I now find my mind wondering and thinking about other things beyond my CPPS which to me is a sure sign that I'm headed in the right direction. I'm sometimes hesitant to revisit this site for fear that I'm in some way supporting my condition, but I feel its important for current sufferers to hear from those of us who are making positive strides. I'm currently reading John Sarno's book called Divided Mind and it makes a lot of sense. Just like Headache in the Pelvis does. However, Sarno connects strong emotions, such as rage, in the unconscious mind as the culprit for conditions like CPPS vs. just unconscious tensing. I am planning to see a psychologist to locate any hidden emotions that might be contributing to this. But, by in large, as my mind and anxiety has settled, my condition has settled.

Re: My Management Plan
Good to hear, rjdemarx. I'm confident to say I am in a remission hopefully. As most know there ain't a cure and I still get minor flares and redness to the penis head but they get less and less. The above seems to be working for me but I can't stress enough that this issue is insidious and different for everyone. I truly believe the most important aspect is your mental health, everyone needs to get on top of it. The more I focused and obsessed over the pain, the more I felt it. I know that this is the hardest part and it is certainly not in your head - the pain is real - but I say to anyone to get on with life for 3 to 4 weeks and try and push the pain out of your head. Find a way to deal with your stress and anxiety and I would recommend not internet searching or compulsively going to forums etc in that time. Take Magnesium Glycinate and Ashwagandha and do the stretches. Don't set yourself up for failure and think this will cure it but its important for sufferers and newbies to know that it can and will get better.
