Background: Now 26 years old, good health. 170 LBS. 6 Feet tall. Non-smoker, drink socially. Been in relationship with same women for over 2 years.
In 2011, after intercourse (I think it was unprotected if I remember, not 100% sure), I went to pee and nothing came out except a few red blood drops, scared the crap out of me. Took an hour to finally relieve myself. Several large blood clots (looked like leaches) came out. Please keep in mind I was single at this time. Went to the family doctor, was told I had a UTI and given antibiotics. A few weeks later, I had oral sex with a different girl after a night of drinking and went to pee and the same thing happened again. Went to a doctor on call as the next day was a Saturday, and was having issues passing the clots (couldn’t urinate). I passed the clots there and thankfully avoided the catheter. He referred me to an Urologist. Went to the Uro, and had the following done: CT scan, DRE, and then a cystoscopy. The CT was fine, and the cystoscopy was fine and the only notes were veins were venomous. The DRE showed the boggy and inflamed prostate, they said I had prostatitis after originally thinking it was seminal vesiculitis. I was given Bactrim DS for 4-6 weeks, and an anti-inflammatory for a week. I was not sexually active during this 4-6 week time with just a few masturbation cycles in between. I had sexually intercourse a few months later and never saw any blood. I figured problem solved. Keep in mind, this only happened during sex and never while masturbating. I never saw blood in the semen, only urine after intercourse.
Now, from mid 2011 until December 2014, I never had a single issue of blood clots after sex. I felt like I was fairly sexual active with the last two years as I have been in steady relationship. (I now live in a different city than I did in 2011, so new docs.) In December 2014, I have a serious stressful work quarter and had not been sexually active. I had sex one morning and went to pee and it happened again. I about passed out…. This was the first time in say maybe 30-45 days we had intercourse. I masturbated around 1-2 times a week. But passing this clot was very difficult, took a shower, sat in bath, finally passed it. Scary to see something that big come out of your penis.
Went to the doctor the day it happened, felt prostate—felt inflamed. Urine sample was very bloody. Put me on Cipro and referred me to Uro. Saw the new Uro next week. Wasn’t crazy about the new Uro Doc, seemed rushed and didn’t listen to me out, however urine was clean. Think this is where I made a mistake and should have seen someone else. He put me on a low dose of doxycycline, for 6-8 weeks. This didn’t really help. Gave up, and tried the all natural thing—Zinc, Bee Pollen, Cranberry at one time. Then tried Vitiamin K2, liquid chlorophyll, turmeric. quercetin with bromelain. Nothing worked. I did a 8 day dose of Levaquin, as I couldn’t finish the full 10 days, did not like that stuff, very strong side effects.
A few things different happened this time vs. 2011: Now I have prostate pain especially when sitting. I even made a standing desk at work it was that annoying. That helped with the pain some but it’s still there, standing or sitting. Some days there is no pain, some there is. I usually wake up feeling fine pain wise. I have started to keep a log of what I eat, change diet, seeing if that changes, tried cutting out dairy, acidic foods. Nothing. I haven’t had caffeine or coffee in 6 months. Limited alcohol. I live a healthy life, eat well, run 4 days a weeks. I am in good shape. Something else that is different this time is blood in the semen. This has happened several time, both during masturbation and sex. In March they did a CT scan and cystoscopy with bladder biopsy. All tests were fine. I felt like I was getting better, until 2 weeks ago I had sex and looked in the condom and was very bloody. Went to pee and large clot passed (even took a photo to show the doc). The next day, I called the practice and requested a different Urologist. I really liked this guy, we talked for a while, he did a DRE, said it felt inflamed (larger for 26 year old). He asked what medication I used back in 2011. So, here I am on Bactrim DS and now Flomax as a alpha-blocker. Currently on day number 9 of medication. Today, I felt prostate pain throughout the day, and I tried masturbating. There was lots of blood in the semen. Followed by blood clot in urine, after the clot passes, urine is pretty clear. I am aware I’ve only been on this medication for 9 days, and he is suggesting 4-6 weeks, which I agree with.
Is something triggering this? I do eat gluten daily, I have read about that being an issue. I have spent hours online over the past 6 months researching this topic. This issue is really affecting my life and is very depressing. Standing at work is tough, can’t have sexual intercourse, limited alcohol, spicy foods, no coffee (if that is even a contributing factor.) I have never had a PSA test done. The new Doc said a semen culture could be a option however the results are often not truth telling. I don't have the normal symptoms of a lot of folks. I don't go the bathroom that often, can make it an entire night without going. Stream Is a bit weaker on first pee in the AM. I have peed clots after waking up with an erection and sleeping on my stomach. Bowel movement can be touch, perhaps prostate related. I only had burning after ejaculation 2 times during this 6 month period of this issue.
What I know is the prostate is enlarged, inflamed, and / or infected and is causing a vein in the prostate or urethra to tear. Sorry for the long post, just looking for some help here. Been reading the forums and it’s great that this is here for support. Any thoughts here or positive thinking? Something I haven't tried is just taking break...
Thanks for taking the time to read my post.

Re: New to Forum - Not New to Prostate Issues- Blood in urine & semen
Firstly, blood in ejaculate is not a typical symptom of prostatitis / CPPS. Bloody ejaculate can be meaningless, and is more likely meaningless in a young man like you. However, the large clots you describe are very unusual and make me think there is a developmental flaw in the vasculature of your genitourinary tract, somewhere. This could mean that it's relatively easy to provoke bleeding during intercourse, especially if there is pressure applied by your partner with her body to your pelvis or if you slam against her too vigorously.
I think you should see your CPPS as something separate to the bleeding. The only thing about CPPS that could cause someone with a congenital flaw in vasculature to bleed excessively is the increased blood flow to the prostate in CPPS patients, as seen in Doppler studies.
Secondly, it's very unlikely, given your history, that you have an infection.

Hi webslave,
Thank you for the response and your feedback. Very helpful. I agree with on my blood clots in semen & urine are not the typical symptoms of CPPS, and it being a different issue. Now any idea on how to treat & healing of whatever vessel / vein is breaking during sexual activity? That's the million dollar question I guess
. I do however plan to lay off sex / masturbation for 1 month, as this is something I have not done since this started. I usually ejaculate 2 times a week. I am considering a semen culture so see if that shows anything.

Have a semen culture if it will lay your mind to rest. Dollars to donuts nothing is found.
I've already suggested a way to cope with the venous weakness, by careful lovemaking.
