Injury to Testicle & Premature Ejaculation

Post by villager »

Hello

Thank you for taking the time to read my post. Let me give you some background about my condition. I am a 37-year old male. Ten years ago I did something reckless and got myself injured on my left testicle. I did not know then but can only guess now that, as a result of the injury, the pelvic floor near my left testicle was also damaged. The condition was exacerbated by varicocelectomy procedure performed on the left side of my groin in order to correct varicocele developed on my left testicle just a few months after the incident. Since then I gradually developed CPPS. And you probably can guess, life was hell.

A few years ago, I came across Headache In The Pelvis and have been following the advice in the book with some success. In general, I go through my day without pain except when stress sometimes hits me and I start tensing my pelvic floor. Most of the time, when I catch myself starting to tense up, I would relax and the pain usually goes away. I am optimistic and diligently working towards a pain-free life.

You might start asking: "So what's the problem?" Well, recently I started to notice that it is almost impossible for me to control my ejaculation. I do not have any problem getting an erection. As a matter of fact, getting an erection is the easiest part but the problem is that after about 45 seconds of action I am ready to shoot! After much thought, I can only attribute the symptom to the weakness of my pelvic floor muscle. In fact, even after almost 10 years after the operation, my left testicle and its proximity still feel "funny". It's not pain I am talking about but more like a dull, mild and constant ache that emanates to my perineum and rectum. The dilemma is that I can't do Kegel because it will make the pain worse but somehow I urgently need to do something with my inability to control before my girlfriend files a complaint against me which she is now thinking about from the look on her face after my consistently disappointing performance.

Any thought? Thank you.

Have you tried using condoms to reduce the sensation?

Age: 31 | Onset Age:30 | Symptoms:Burning/raw pain in perineum, occasional penis pain, burning pain in inner thighs, down back of legs, anterior thighs to the knee. | Helped By:Not sitting, Not obsessing, used to be helped by pelvic floor massage and sleep, but seems less effective lately, anxiety has been significantly worse | Worsened By:Sitting at work, catastrophic thinking, doing things I really don't want to do

Let her get on top. Condoms are a good idea. Think of other things, non-sexual things. Relax your pelvis as she does all the work. Make her stimulate herself with her clitoris on your erect penis without penetration for quite a while before intercourse. That way she won't last long either.

Just some tricks from the male playbook

Yes, I have. It effectively increases the time span from 45 seconds to 1 minutes and 30 seconds!

I also tried having her on top and I did last longer. However, after a while, it got boring and I'd like to take control. I have also tried thinking about all the horrible things imaginable while performing but I couldn't focus at those things for long because strong sensation down there calls for my full attention. I have come to realise that one way or another I need to exercise my pelvic floor to build up lost muscle mass partly due to the injury.

Any suggestion on stretching exercises that are less likely to aggravate the pain?

If Villager is still around, I wonder if he could give us an update on his condition and maybe, if possible, more details on what the "something stupid" he did to first cause it was. I ask because his symptoms and progression (minus the premature ejaculation and surgery, though I do think I may have a mild left varicocele) sound very similar to my own. Thank you!
Current age: 30; Symptoms began: 29; | Symptoms: dull, burning pain in scrotal, perineal and rectal areas, focused on the left side, dull stomach pain when sitting; Improved by: stretching, yoga, Theracane massage (external and internal, though it causes flare-ups), diazepam (valium), hydrocodone (vicodin), hot baths, sleep, relaxed sex, alcohol (a little); Worsened by: SITTING, frequent masturbation/ejaculation, lots of caffeine (but a little's okay); May help a little: Doxazosin (but caused heartburn), Prosta-Q, nerve block injection (provided symptomatic relief for a few days but likely no long-term benefit); NO help: Amitriptyline (Elavil), Cernilton

I saw some people a while back talking about reverse Kegel helping a lot on this front. Kegel itself is a terrible idea for chronic prostatitis / chronic pelvic pain syndrome patients (although I would like to be wrong on this) and from my experience it creates PE rather than correct it (will go away with time).

As for reverse Kegel: Now Mark might correct me but it seems that there is a fine line between relaxing your muscles and pushing them (slightly) in the opposite direction of Kegel. While some people have reported improvement in PE I wonder if reverse Kegel could be also a bad idea for this patient group.

I personally tried Reverse Kegel for the last 5 days, very gently, and it gave me pain. As for PE when I am in pain I won't even try.
Age: 33| Onset Age: 24 | Symptoms: dull ache in pelvic area, tension, feeling the need to urinate, frequency, dribbling after urination, ED symptoms started 6 moths after the onset wrecking my life since, abdominal tension, tension in my thighs. | Helped By: stretching/massage , benzos | Worsened By: Mainly sex, but also sitting and anxiety| Other comments: I have seen periods of substantially less flare-ups, but now I am at a steady state where it comes back almost always after sex.

Reverse kegels involve an initial tightening of the pelvic muscles, before the relaxation phase, so I'd avoid them.
