Here's my story, the long and short of it.
I'm a 24 year old man. At 20 I started getting urethral burning after peeing less than 2 days after a brief episode of unprotected sex. The girl claimed she had nothing, tests found nothing, and antibiotics did nothing. The burning became relatively constant. My penis was sensitive in strange places along the shaft, but nothing acute. The only burning or discomfort seemed to be at the tip. No discharge.
I had a varicocele removal operation in both testes a year and a half ago. When the bandages came off, I had a new discomfort, in the left hand base of my shaft. It would feel like a small cut there after I masturbated. It was just a slight discomfort though. It eventually escalated to the point where the entire base felt sore after a session, and the left hand in particular was very uncomfortable. It almost felt like there was a sore muscle running to the top of the shaft. I was afraid the operation had gone wrong. I switched hands masturbating, and soon felt it on the right side too.
A urologist told me he was positive there was nothing bacterial going on, and this was nonbacterial prostatitis. He felt my prostate and said its tightness confirmed his suspicion. He said the operation had nothing to do with it. He wrote me a prescription for pelvic therapy.
I went for about 2 months before insurance ran out. I did abdominal rolling, and used the recommended wand for pelvic stimulation. Sometimes it seemed to help, other times it just seemed to make it hurt. In the months since, I've really noticed the "clenched fist" quality of my lower abdominals. I have very minor hemorrhoids which I attribute to it. After masturbating, I feel a sensation of general pelvic soreness like somebody kicked me. When I eat a big meal, and the pelvic area is sore, it seems like the food weighs down on the painful area.
I'm a generally tense person, and there is a ton of anxiety in my life right now, jobless, trying to move out of home, and trying to foster a sex life. I worry that my penile discomfort will threaten my ability to satisfy a girl. And, of course, I worry, that this will never go away. I don't know anyone that's had it. And I don't know how to alleviate it in any lasting way.
Please advise me the best you can, or point me to someone that can. It weighs really heavily on me. Much appreciated!

The pelvis is a highly innervated area, due to the tie-up between reproduction, evolution and sexuality. This makes it prone to neurological problems like wind-up, in which small insults or injuries can become large over time. That's the basis for what's been happening to you. Reversing the process is not simple, especially when you have an anxious disposition that tends to maintain the neurological state.
Good News! The great ProstaQ is BACK at last! You must try it.

My best recommendations are the following:
- Try to control your anxiety: Use meds if necessary. They gave me a leg up in my recovery process.
- Engage in stretching, light exercise: in addition to mitigating anxiety, this can help your physical symptoms.
- Try a diet that will be easy on your intestines/bowel movements. I quit coffee immediately and never regretted it.
- And most of all, don't obsess over it. Sometimes trying to do too much can make things worse. Change your lifestyle. Have faith that your body is working on recovering naturally and that can help a lot as well. I know it's tough with the pain, but this is your body's way of telling you do change your ways.
Think baby steps - there is no complete overnight cure for this. The collective effect of small changes can do wonders.
