Just to contribute some hope to this incredibly awful condition, I'd like to update my progress.

After the typical run of things that almost everyone here has experienced: urinary problems, pain while sitting, ineffective meds and treatments, I'm happy to say that I am continuing to improve and currently at about 80% recovery after 1.5 years. I can still get a flare up here and there by ingesting the usual exacerbating suspects (e.g. coffee and soda), but it's not nearly as debilitating as before.

What's helped? Actually, time has helped quite a bit. I'd say that sometimes doing too much can actually make it worse - it's best not to worry too much about this if possible and try your best to keep a positive attitude. Believe me, I know what kind of panic this condition can cause as you convince yourself something is seriously wrong and continue to become frustrated that there are few proven methods or practitioners out there to treat it. Most of us are not even sure what caused it in the first place which makes it difficult to figure out what exactly is 'broken'. Discerning what is a 'cause' and what is an 'effect' as far as symptoms go is what confounds us supremely.

Decreasing anxiety is 100% the best weapon in combating this condition. I've received some help from Lexapro and Lorazapem to give me a leg up on the anxiety thing while I work on changing my lifestyle both mentally and physically. Learn to relax - exercise, yoga, meditation, whatever. The benefits from these changes will enrich your life way beyond treating CPPS.

This condition may never totally be gone, but do your best to find a way to manage it and not let it manage you. Best of luck everyone.

I encourage anxiety prone UCPPS people to consider L-Theanine
Age, 44 onset age 37 Feb 2006 Freq. need to urinate. Sensation of having to urinate soon after going. Perineum discomfort/burning/tightness, pubic area discomfort @ times,poor urine stream, post urine dripping/spray. All symptoms have improved with my protocol. At the worst I give it a 1 to 2 on irritation and discomfort and frequency. Helps: Elavil 5mg for anxiety and mast cell protection, (will only take it as needed) self internal PT as needed, stretching, walking, stairmaster cardio workout and light weights, reducing stress, moment to moment relaxation, deep breathing relaxation and using a Theracane. Makes worse: sitting for long periods, stress, over focusing on it. Currently 95%-98% recovered. Stay positive, relaxed and control your anxiety.
