I have a question regarding internal self-treatment: whenever I’m having a flare-up I’m experiencing pain in the perineum radiating into the testicle.
I do self treatment alternately lying on my right and left side but I’m having a hard time reaching the muscles around the perineum. I now try to perform tp release lying on my back because of the better reach in some parts of the pelvic floor.
I’m a bit weary of digging in too deep because of the proximity of the prostate.
Although, following the diagram on http://ucpps.men/1/male-pelvic-floor.jpg the perineal muscles lie a bit upstream of the rectum and the only way to reach them would be lying on one’s back.
Any suggestions on what might be the best technique to look for perineal tps?

Re: Question on the best position for internal self-treatment
Hi Benzo...
How ya doing these days?
This is what I do...
I sit on my toilet and with my gloved right middle finger, I go through the front palm down or towards the body. Put your left hand on the ground as you go in and drop the pelvis as you go because you naturally want to clench as your finger is inserted. Try working the area around the anus as well. 1 or two inches in.This is a trouble spot for me and the area bellow the prostate closer to the anus carefully
It helps to have your bowels empty to go higher up as well and go from there. Hope that helps...
Man, the sh*t we have to go through to keep the monster at bay
Cheers my friend...

I encourage anxiety prone UCPPS people to consider L-Theanine
Age, 44 onset age 37 Feb 2006 Freq. need to urinate. Sensation of having to urinate soon after going. Perineum discomfort/burning/tightness, pubic area discomfort @ times, poor urine stream, post urine dripping/spray. All symptoms have improved with my protocol. At the worst I give it a 1 to 2 on irritation and discomfort and frequency. Helps: Elavil 5mg for anxiety and mast cell protection, (will only take it as needed) self internal PT as needed, stretching, walking, stairmaster cardio workout and light weights, reducing stress, moment to moment relaxation, deep breathing relaxation and using a Theracane. Makes worse: sitting for long periods, stress, over focusing on it. Currently 95%-98% recovered. Stay positive, relaxed and control your anxiety.

Re: Question on the best position for internal self-treatment
Hi Carl,
Not so good at the moment, I'm having a massive flare-up while in the middle of renovating my new flat. It's so predictable.
Although I must admit I came out of a very good spell, but as I already stated, I kind of slacked and didn't keep up my game in terms of tp release. So as soon I think I have this thing under control it hits me square in the face again.
So I dove into tp release again, the worst pain comes from the perineum these days and that is one hard spot to reach internally, hence my question. I always mimicked the sideways position I'm in while at my PT's but this might not be ideal because the range of motion is limited that way. That's why I'm looking into alternatives so thanks for the feedback. Also started on a new supplement mix: I'm now doing Quercetin + Zyflamend + Barleans Omega Man which contains a flax seed oil compound. I must break this flare ASAP because it's driving me insane with the renovation and all. And yes, the way I explain it, it's a cycle really.

It just goes to show that stress again is such a factor here...What about evening relaxation to wind down after all the construction hell...Make sure you give yourself some down time as well...

It sure is for me. Thanks for posting the instructions for self-internal massage. I will probably start trying that soon.carld wrote:It just goes to show that stress again is such a factor here...
Did you ever go to the Stanford Clinic or get physical therapy from one of the people in So. Cal?

Re: Question on the best position for internal self-treatment
I think it's important to remember that if it is in fact a trigger point and not just muscle tightness you will find that the pain is being referred from a TP that is in a different spot much higher up then the perineum itself. Perineum pain and tightness is one of the most common areas of pain with CPPS. I had primarily perineum issues for a long time and massaged the hell out of the perineum the whole time with no real results. I found that when I finally went into the rectal cavity and massaged out the rectal walls about 7 inches in that I finally hit the area that referred pain and symptom to my perineum. I know it's not what most people want to hear because they'd rather simply massage 2 inches inside the anal sphincter in hopes of releasing trigger points but I suggest you go much much deeper with the use of a sturdy tool to find the culprit. Just my suggestion though. ;)

Ya, the slender 7 is what's been used with the best results from what I have read here...I have one...I tried one attempt, but with out the sensory of my fingers...it just seemed risky...but non the less, at some point I will have to give it a careful go...because perineum discomfort is still an issue with me when I have it...

Re: Question on the best position for internal self-treatment
I bought the Slender 7 too some weeks ago. I don't like it being so sharp at the tip, I am afraid of poking around to much with it. Think I will have to buy a Crystal Wand. I did try using the Theracane too, and it worked better, but lacks the curvature.
Age: 41 Onset Age: 27 (dec 2006)Symptoms: Urethral burning, muscular pain and soreness around the base of my penis, perineum and muscles around my sitbones. Frequency, urgency, dribbling, general discomfort in the pelvis, decreased libido, tight levator ani (according to PT) and occasional levator- and sphincter spasms. Sometimes ache in penis, especially when erected. Latest addition is testicular pain (and epididymis). Also excretions of clear (prostatic?) fluid throughout the day as well as secretion in my urine. Varying power of urine stream. Redness on tip of penis and ballsack.Helped By: Positive thinking and controlling anxiety Worsened By: Stress, lack of sleep, heavy training (especially abs and legs), catastrophizing over this condition

Re: Question on the best position for internal self-treatment
I hope I read your post wrong Hacksquat, but you attempted to use the Theracane for internal work? Yikes!!!
I hope it is early and I am reading that wrong. I have both the wand and the purple slender. I prefer the wand while laying on my back with my knees bent and feet flat on the ground.. My body is at it most relaxed position I could find. For the first few months, I stayed within the length of the wand. My pain diminished a little, but I decided maybe I needed more distance. I purchased the slender and have only experimented a few times. I am definitely not comfortable with the distance especially after Dr. Dimitrakov warned that damage can be done easily in this area because the tissue is very soft and weak. I think he was directing his comments towards area near the prostate.
The fingers have more sensitivity but even at 5'10", 170 and having taken yoga for the past 3 months, I find it tough to hit certain areas using my fingers without putting my body into a state of tension.

Yep, I used the Theracane internally, no problem
. No seriously, I tried it once and won't do it again. It's too big so you have no control of what you are doing in there.

Check out posts by ex-moderator cppsdad. He used a Theracane internally with success.
