Excessive Prostate secretions/Prostatorrhea

Post by mapletop »

I'm new here. The problem I'm having is that my bladder feels full or when my penis feels like theres something in it, I can go urinate and at the end of the stream I can bare down or push with my pelvic muscles and milky/clear secretions will come out. They will string in the toilet and they are slippery. They also occur when when I have a bowel movement. The fullness sensation in my bladder and symptom urethritis goes away after I do this. I have to do this daily sometimes more. I have searched the web and books for this symptom and the best I can come up with is Prostatorrhea. Has anyone else had this happen to them.

More info: I have had flare up of enlarged prostate/prostatitis for about a year now. I've had a scope done and my doc said that my gland was enlarged. neg for STDs. I also have regular ejaculations. Taken countless antibiotics. Uroxatral.
Age:33 | Onset Age:on and off since 16 | Symptoms: Semen Leakage when having a bowel movement, perineum pain, urethritits symptoms, Itching urethra, NO Bacteria ever found. | Helped By:reading A Headache in the Pelvis. | Worsened By: sitting, worrying about my problem (Antibiotics don't help, no bacteria in prostatic secretions).

Please fill out your signature. The excessive secretions are probably due to the swollen prostate occluding ducts and preventing the normal escape of secretions. Combat the prostatitis and the other problem will disappear.

Hi,

The same thing happens to me now and again. Every once in a while, particularly after a constipated/difficult bowel movement, I'll pass fluid similar to what you describe. I seem to be especially prone after arousal or long periods without ejaculation. However, this gives me the opposite effect, flaring up rather than providing relief.
I am not a physician. This is not medical advice. Consult your doctor!

Age: 26 Onset Age: 17 Symptoms: Shooting, nerve-like pains throughout the penis, which abruptly hit and leave. Testicular pain, perineum pain, burning/irritative urination, extended pain after ejaculation. Occasionally, some allodynia or ache in the coccyx/sacrum/thigh/buttocks/legs. Diagnosis: Pelvic floor dysfunction, degenerated lumbar disk, and mildly herniated lumbar disk. Helped By: Physical therapy, pain management doctor, hot baths, therapy pool, stretching regimen, breathing exercises, relaxation, distraction. Worsened By: Arousal/ejaculation (worst), constipation, panicking/obsessing, other triggers depend upon current symptoms. Tests/Prior Treatments: Too many antibiotics to count, multiple urine tests (all normal), testicular ultrasound (normal), bladder and renal ultrasound (normal), lumbar and pelvic MRI with and w/o contrast (revealed disk problems), Elavil 25mg (caused retention), Flomax 0.4mg.

Thanks guys. Jay. I have researched this on the internet and most of what I have found about prostatorrhea was published in books in the early century from urologists. No updated material I have found about this specific symptom of prostatorrhea. What I did read was that it was caused by sexual excess, delaying/holding back orgasm, sexual excitation without ejaculation. This fits my case upon arrival of symptoms. Do you have any input about this?
I also pass jelly like gelationous pieces the size of a grain of rice upon muscular effort from time to time, but this occurs more often when I do not ejaculate for a couple of days is this normal? They are clear/white with the consistancy of cooked egg whites.
I also have more questions in reguard to webslaves input.
How can blocked ducts cause the secretions to flow out with muscular effort? Is the blocked ducts filling up with secretions only to be expelled with voluntary muscular effort? Or is the gland not blocked but the opposite where as secretions flow out with minimal effort due to enlargement? Can this be a result of years of sexual excess and the gland is enlarged and hyperactive in secretion production?

Bowel motion presses against the prostate and that typically expresses some of the inspissated, sequestered secretions. Equally when you put a lot of pressure on the pelvic floor, the congested gland will react similarly. It's not recommended. The ducts are occluded because the uroepithelium is inflamed. No, I don't believe this is due to sexual excess, but then what do you mean by excess? It's possible to have an overuse issue if you ejaculate often enough...

I can get this sort of discharge following urination - but it only happens if I've not ejaculated for around 10 days. Doesn't happen these days as my sex life is back to normal, as I don't get an increase in chronic prostatitis / chronic pelvic pain syndrome symptoms post orgasm, but in the bad old days I minimised ejaculating and this happened - so I made sure I ejaculated at least once a week.

Thanks everyone. Scotsman, yes mine are worse too if I do not ejaculate for awhile. However mine occurs daily. When you had to cut back on ejaculation, may I ask if there was a reason for this?
Webslave if muscular effort relieves pain and symptoms how can it not be recommended? What else can you do? Also thanks so much for the medical term for the small rice shaped globules. I've searched for that for months. Its hard to explain to the uro. By excess I mean delayed orgasm and mutiple ejaculations in a day. I've read that too many and not enough ejaculations can inflame the gland.

You should not be straining the pelvic muscles to expel prostatic secretions. That alone could cause a pelvic pain issue. If your gland is unusually congested you could try to drain it manually, but be careful

That's too much. Try to stick to a few times a week.

Ejaculation increased pain and frequency feels for me - it was minimised by relaxating my muscles when ejaculating. Triggerpoint work got rid of the frequency feelings and my general chronic prostatitis / chronic pelvic pain syndrome symptoms decreased enough over time (with relaxation, stretching and triggerpoint work) so that ejaculating these days has no adverse effect.

Thanks guys. I have tried manually and the secretions that I expel are minimal. If I don't push out these secretions then my prostate has prickling pains and I have a feeling of fullness of bladder. I have thought about trying Proscar for the enlarged gland. Has anyone used homeopathic remedies? I have used everything from zinc, saw palmetto, prostate pills and tinctures, Quercetin, quercetin. Anyone heard of Staphysagria. I've found that it has been used for symptoms like mine but it comes from old books.

The secretions come out of my penis. Mucous is normaly found in stood its just not always seen. I notice a lot of guys on the board have bowel problems along with chronic prostatitis / chronic pelvic pain syndrome or prostate problems myself included. Is there a relationship?

Hi,

Gastrointestinal issues, particularly Irritable Bowel Syndrome (IBS), frequently present among people posting on the boards. I have IBS-C, myself. In the past, the question has been raised as to whether IBS might be a precursor to chronic prostatitis / chronic pelvic pain syndrome in some patients.

Misdiagnosed with prostatitis

Post by mapletop »

For many years I took antibiotics and had urologists palpate the prostate and say yeah its inflamed. A few years back I had a TRUS and found out that my seminal vesicles were enlarged, thick and inflamed. The years I went without proper diagnosis, I feel has caused damage to the vesicles. I have had a bad flare up recently and have taken prednisone for the inflammation and it has not done any good this time. I wish I could find a doctor that could help me.

Contact a urologist for seminal vesicle removal. I know Dr Shoskes had a special interest in this.
http://my.clevelandclinic.org/staff_dir ... torID=3631
Dr Daniel Shoskes wrote:The indications for seminal vesiculectomy (surgical removal of seminal vesicle) are limited to very unusual and specific cases of infertility or chronic infection. In the FEW patients in whom it may be indicated however, there is justifiable reluctance to proceed due to the difficulty of the standard surgical approach (which often involves division of the bladder neck or prostate) and the high complication rate.

Along with my colleague Dr.J Rajfer (who specializes in infertility), we have developed a new surgical approach to remove a seminal vesicle that avoids the above mentioned problems completely. It is done through a lower abdominal incision (called a "Gibson", which incidentally is the incision I use for a kidney transplant) and leaves the bladder and prostate untouched. Between the 2 of us, we have done about 20 of these operations now, although only one was in my own patient with documented obstruction and infection who failed antibiotics and massage. It was a technical success, but we are too soon after to assess changes in symptoms or EPS.The other 19 operations, mostly infertility related, were unassociated with any major complications such as impotence or incontinence.

In any case, we are planning to write up the operation for publication, but the lag time for journals even if it is accepted right away can be several months or even a year. Therefore, if anyone has been considering this operation for a valid, documented indication, I would be glad to discuss this approach with their own urologist. Please have them email me, or they can leave a message for me at my prostatitis clinic.
