Bladder neck question

Post by soulway »

Hello All
I've had "prostatitis" like symptoms for the last nine months initially they consisted of suprapubic pain, a burning throbbing tennis ball like pain behind the scrotum, very weak flow, low back pain and burning on ejaculation.
Over the months these have changed or moderated to left sided rectal and lower back pain and I still have the weak flow and ejaculatory pain.
Have had all the usual tests multiple blood/urine, PSA, ultrasounds, X-rays, cystoscopy et al. These have shown nothing apart from a "high bladder neck" revealed by the cystoscopy.
The urologist suggested alpha blockade which I've been on for around four months. This has improved the flow somewhat but its still poor at around 9-10mls.
He seems to be of the opinion that this problem with flow could be the origin of my trouble.
I gather that one option may be a BNI however I am very unsure about going down this route as I have read both good and bad accounts of this procedure. If there are any guys on this forum with experience of trouble from a high bladder neck or any who have undergone surgery of this type I would be most interested to hear their opinions.
I should add that I've read "Sleeper service"'s posts on this subject but it sounds like the surgery didn't necessarily resolve his issues.
My current protocol (if you can call it that) is the A HEADACHE IN THE PELVIS stretches and internal work, 50mg tramadol x2 p. d. and occasionally valium 2mg.
Regards
Jeff

Uros will often try to turn your frustrating and difficult to cure problem into a surgical opportunity. I would forego all invasive treatments at the stage you are at.
Good News! The great ProstaQ is BACK at last! You must try it.

My surgery didn't resolve all my issues but it did remove those connected with poor flow and obstruction. As such it reduced my symptoms by 50% off the bat and over the last couple of years I'm at the stage where I woud say I was consistently 80% better, have a good number of virtually symptom free days and few bad days. In short life is good again.

However...

A high bladder neck is not in itself an indicator that you require surgery and I would under no circumstances recommend that you have such a procedure without first undertaking a thorough videourodynamic study as this is the only really reliable indicator of whether or not you have a bladder neck obstruction.

Hi Sleeper,
Many many thanks for your response.
As the man here with practical experience with this issue I respect your opinions.
As I mentioned in a previous post my symptoms have changed over the months and currently the most bothersome are a left sided rectal/low back pain, the very poor flow around 8-9mls and some burning post ejaculation.
Curiously I've never had any frequency or nocturia as such.
Originally the symptoms came on literally overnight, woke up one morning and couldn't pee and it went on from there.
I guess my dilemma is are the "prostatitis" symptoms caused by the high bladder neck or vice versa.
The urologist is of the view that some residual urine (my PVR after voiding a really full bladder was around 50ml) may be acting as a irritant to the prostate.
As I am a tad phobic around hospitals I've been hoping to avoid any more invasive testing but its looking like I will have to bite the bullet and consider the videourodynamics as the way forward.
Regards

I would recommend that you do. As you may know a BNI carries some risks and side effects and you need to be absolutely sure that it will benefit you before having it. I must also be clear that it may not clear up any pain or irritative symptoms (mine improved but did not resolve).
