Hello everyone!
Let me introduce myself. My name is Alexander, I live in Moscow, Russia. I'm 24 years old. My story lasts for 2.5 years. To be honest, I came here by advice (stevo, thank you again) from another forum where I was discussing surgery as a last resort for my case. Hope this forum could help me to find more options.
Here's a history of my case.
I'm almost sure cause of my condition was overdoing sex 2.5 yrs ago. Till then I hadn't regular girlfriend for a year or so - masturbated maybe 3-4 times a week, sometimes 2 times in succession. And few days after I met my girlfriend, she was living at my place for 3 or 4 days. And we had sex sometimes 2 or 3 times in a row. And one more thing - usually it was hard for me to reach a climax, I was used to - so sex could last for about a 30 minutes. 3 days later in the morning I went to the bathroom and felt burning pain when was peeing. When I finished there was burning in urethra and feeling that I still have urine left in my bladder plus discomfort and irresistible urge to urinate despite of empty bladder. This was reaally horrible feeling. Few minutes later pain left me, but uncomfortable itchy sensation in the back of my urethra and in perineal zone became persistent.
Worst of all - 2 or 3 days later symptoms returned and new added. That's the symptoms I have now. I have burning pain in perineum especially when sitting on a hard surface or driving, wearing tight pants. Burning in the back of urethra - sometimes visiting toilet I feel like I peeing out barbed wire, feeling of "last drop" staying in urethra after visiting toilet. Sharp shooting pain (like electric discharge) through urethra to the tip of my penis. Sometimes heavy discomfort in perineum like my bladder is going to blow, but when I'm near bowl there are just few drops. Ejaculation sometimes painful, and is happening much faster than before. Burning after sex is sometimes unbearable. . . So now I often try to avoid sex and this makes me really depressed. Lifting heavy things makes my symptoms worse, lying on a back gives me relief - I'm going to the bathroom in night very seldom because of it.
Now about my tests and treatments I have tried. You'll see that I'm considering surgery for a reason. . .
I have high WBC count (30 in moderate) and enterococcus faecalis (10koe4) in EPS, DRE is painful. No STD. TRUS shows area of calcification in the periurethral zone in the right lobe of prostate. Nothing special on CT and MRT. Myography and uroflowmetry are ok. Flow is good, no intraprostatic reflux. And few weeks ago I had a prostate biopsy done - calcified zone samples has signs of inflammation and fibrosis, also presence of bacteria. PSA is 0.4.
I've tried antibiotics (all quinolones and much more), alfa-blockers, heat therapy, magnetic therapy, SANOS protocol, prostate massage, broccoli thing, changings in diet - no caffeine, alcohol etc, cauterization of colliculis seminalis with AgN (that's hellish thing, only some old soviet school docs uses this), biofeedback, Russian analog of saw palmetto, pollen bee, hot baths, detralex and troxevasin (I have dilated (varicose) periprostatic veins - does anyone have it also? seems important to me), vezikar ( (SOLYFENACINUM - anti-hyperactive bladder med), diclofenac, other anti-inflammatory meds, enzymes, allopurinol, also katadalon, grandaxin and sirdalud - this is a muscle relaxant, painkiller and neuroleptic from a neurology specialist. . . Even hirudotherapy and homeopathy. Not sure this is a complete list. Everything failed or at had very short-time effect.
I found uro who was giving me intraprostatic shots using same cocktail of abx and steroids as Bahn - I found recipe on Bahn's page and gave doc a link. He gave me 3 shots with 10 days interval - after last injection my symptoms reduced maybe by 50%, but came back with revenge within a month. He is expensive, I can't allow doing injections for the rest of my life every 2-3 months, so I quit.
Now I'm going to pool and swim 2-3 times a week, exercise at tai-chi class, doing Kegel exercises - all for about 4 months. Still no help and I'm loosing heart because of it. There are no PT qualified in trigger point release in Russia, and I don't have a possibility to go to Stanford clinic, but I'm going to order a copy of "A Headache in the Pelvis" tomorrow. I decided to put off surgery for a few months at least and look for other possibilities. But if nothing helps. . .
So I'll be glad to hear any suggestions and advices. Anything.

Re: My case (newbie from Russia)
Just a few observations:
- The heroic measures you have sought out and tried tells me you are a hypervigilant Type-A personality. This could be part of the root cause of the entire condition.
- You may have had an infection at the start, but it's very unlikely that you have one now.
- Prostate removal is NOT the way to go.
- Kegel exercises will make the condition worse.
- Stop doing everything you are doing. Nothing is helping.
- Get A HEADACHE IN THE PELVIS and (free) Quercetin.
- READ, read, read this forum. Keep off the other forums or you will get lost. The people running the other forums are seriously mistaken in many of their assumptions.
Good News! The great ProstaQ is BACK at last! You must try it.

I agree with the surgery part. It would be insanity to remove anything out of your pelvic area because there isn't just one organ to deal with. It's a pelvic problem that encompasses everything in there. The people who resort to surgery either have no change in symptoms or additional symptoms they wouldn't have developed if they would have not had surgery. I've also heard of people who after having one thing removed, in their desperation to get better, had something else removed, and just got worst.
Stay away from the surgeons knife.
I commend you for what you have done thus far in your quest for wellness, but be careful that in your haste to get better, the tension and anxiety of not making significant progress exacerbates your symptoms.
I have been doing a lot of Yoga and daily meditation, which has really helped me a lot. I'm not well, but better and when it comes to this disease better is better than nothing.

Webslave, Strobers, thanks for reply.
I was considering surgery, because of thoughts that I have a prostatitis, not chronic prostatitis / chronic pelvic pain syndrome - I have high wbc count (30 on average) in eps and prostate is painful during DRE and massage. Also, I have nidus of calcification and fibrosis according to the biopsy. And sometimes my semen consistency is changed to jelly-like and colour to yellow. Could it be just prostatitis?
And one more thing - I was wondering about stretching exercises I used to do, i mean tai-chi. Are they recommended for people with condition like mine? looks like they are helping a little, not doing worse at least.

Re: My case (newbie from Russia)
That does not mean that you do not have CPPS. The high WBC count means there is inflammation in the epithelium lining the urethra and/or prostatic ducts. Are you aware that nerves can create this inflammation? Read this page in Russian:sabulon84 wrote:I have high wbc count (30 on average) in eps and prostate is painful during DRE and massage.

None of this is unusual in CP/CPPS.Also, I have nidus of calcification and fibrosis according to the biopsy. And sometimes my semen consistency is changed to jelly-like and colour to yellow. Could it be just prostatitis?
Shouldn't hurt, but not as useful as the proper stretches.I was wondering about stretching exercises I used to do, I mean tai-chi. Are they recommended for people with condition like mine? looks like they are helping a little, not doing worse at least.

Re: My case (newbie from Russia)
I got my condition exactly the same way you did (read my story). My symptoms were also similar. Your calcifications are probably the result of inflammation. My symptoms have improved in just a few days with the use of Librax, a muscle relaxer. Most of the burning is gone. My new Uro told me my prostate now feels normal with no boggyness. I take a hot 45 minute bath each night, refilling part of the tub as the water cools, I am in day 6 of the master cleanse fasting program of which will do 8 days. I am also doing a parasite cleanse. If this isn't enough, I will try PT. Also, I am going to try to go 14 days between any sexual activity to give things a rest; it's hard to do.
Age: 52 | Onset Age: 52 | Symptoms: fairly constant groin "burning", not really related to urination (it feels better after I do it), some post ejaculation soreness.Helped By: hot baths, relaxation, Xanax | Worsened By: too many orgasms, anything but really gentle sex, driving and other stressful activities. Tried: Multiple antibiotic, although I never went more than 2 weeks on any particular one. Did I need 4 to six weeks on Cipro, for example? Prosta Q, PEENUTS, experimenting with Chinese herbs now. Suggestions are welcomed.

Does it sounds like a trigger point?
I've tried massaging lower abdominal below, as one member of the board advised me to - I mean I press firmly with my first 3 fingers from my belly button down to my pubic bone, and to the right and left sides of the middle line.
And I noticed that besides referred pain in urethra I have very uncomfortable compressing feeling coming from somewhere in the right lower part of my perineum, closer to anus. Feeling like a small but heavy boulder is in my butt, and when I was pushing my abdomen or contracting my pelvic muscles - urge to urinate, pressure and pain appeared.
So I've tried to understand - where this pain is coming from and reach this spot. So I began palpating perineal area deeply and slowly, trying to find the source of pain. Couldn't do it while massaging pubis and through lower perineum - so had to use a back door.
And looks like I found it. Pain is coming from a small spot close to prostate, bit closer to anus, inside perineal muscles. Feeling like a muscle cramped, and when I pushed, pain have increased. So I've tried to massage this point firmly, pushing and releasing it again and again. Pain didn't go away completely but lessened. Improvement in my usual symptoms was quick and remarkable. Next day (today) is almost pain-free. Could it be a trigger point - thing that i have found?? Or is it just referred nerve pain from prostate?
P.S. Remembered - I had a little thickening of a muscle (don't remember which one) in the right side of pelvic floor - according to pelvis MRI last year. But doc just ignored it and said this is normal - but now I think maybe it was a muscle spasm I found at least?
