One of the most troubling symptoms of this condition for me is the pain that sets in about 1/2 an hour after ejaculating, and sometimes lingers for several hours. Having read some posts where this is brought up I see that this is something a few other guys are troubled with as well, in varying degrees. As some of the postings have suggested, I have been making conscious efforts not to 'tighten up' when ejaculating and that seems to have lessened it, somewhat, though it always kicks in at some point. But lately I've noticed that if I have an orgasm before falling asleep at night (a regular practice) and I wake up a few hours later that night to pee (also something that occurs pretty regularly, at least once), I don't notice any pain, in the penis, testicles, nowhere. Nothing. So, I'm wondering if that maybe has to do with falling asleep after the ejaculation and the muscles entering a completely relaxed state once I'm sleeping so that they don't have a chance to get all agitated.
I was just wondering if perhaps others had noticed anything similar. And, of course, would be more than happy to hear any further thoughts, strategies, anything, on how to further reduce or, eliminate, this part of the condition.
Post ejaculation pain
Post ejaculation pain
Age: 35. Age of diagnosis: 33. | Symptoms: dull pain in testicles, usually the left side, dull pain in perineum, occasional 'golf ball in the rectum' feeling, general tension/tightness in the whole pelvic region, pain/discomfort after ejaculation. Helped by: acupuncture (slightly); exercise; stretching. Worsened by: ejaculating; alcohol, somewhat; anxiety & stress
I think you answered your question. Going to sleep after sex and being in a relaxed state, I would think gives those muscles a break from the strong contraction from ejaculation. I think a good idea is stretching after sex and take a hot bath and then go to sleep would be something to consider. I know this has helped me as well.

I encourage anxiety prone UCPPS people to consider L-Theanine
Age, 44 onset age 37 Feb 2006 Freq. need to urinate. Sensation of having to urinate soon after going. Perineum discomfort/burning/tightness, pubic area discomfort @ times, poor urine stream, post urine dripping/spray. All symptoms have improved with my protocol. At the worst I give it a 1 to 2 on irritation and discomfort and frequency. Helps: Elavil 5mg for anxiety and mast cell protection, (will only take it as needed) self internal PT as needed, stretching, walking, stairmaster cardio workout and light weights, reducing stress, moment to moment relaxation, deep breathing relaxation and using a Theracane. Makes worse: sitting for long periods, stress, over focusing on it. Currently 95%-98% recovered. Stay positive, relaxed and control your anxiety.

although I've had difficulty mastering it, webslave will advise you try to relax the pelvis during orgasm. Its easier said than done though.

Re: Post ejaculation pain
I ejaculated yesterday after a long hold off due to the fear of increased pain.
The ejaculation itself wasn't to horrifying-I experienced some minor anal pain as well as the tip of my penis that lasted for and hour or so. I also urinated 15 minutes or so later.
But 5 hours after the event I started getting lower back pain. It started on the right side and now on the left. It's a dull pain and I'm hopeful it's not my kidneys. I don’t plain on voluntary doing it again for a while.

Started: 15 Aug 07 "Birthday present" Onset extremely swollen prostrate problems following a sinus infection, which developed into burning urine- 50% better, relaxing, diet. Pain after ejaculation. Worsen by:stress, beer.
Agree with Carl. Follow a regimen after sex that includes a hot bath, peeing, and stretching. Then head to bed to sleep. If possible, do some internal massage/stretching when you stretch. You may notice the muscles are tensed up due to the "activity". Try to gently get them to relax using a gloved, lubed finger.

Make sure you keep you muscles relaxed leading up to the ejaculation and don't try to force the semen out.MikeinCA wrote:I ejaculated yesterday after a long hold off due to the fear of increased pain.

Age: 32 | Symptoms: Twingy pain/muscle spasms in groin area (gone), tingling sensation at tip of penis (has resolved as of now), urinary frequency (resolved as of now), thicker/decreased amount of semen (resolved as of now). Makes better: Stretching, Relaxing pelvic muscles, moment to moment relaxation, not sitting with legs crossed, Elavil, quercetin. Makes worse: Sitting, stress, more than one orgasm/day, constipation, manual labour
Out of curiosity, I did once do some [careful] internal work, briefly, following orgasm to see if that might alleviate the anticipated pain. I don't think it did much to help, but it really was a fairly short exercise. What I did notice, however, is how much twitching, spasms I guess, was going on inside that I wasn't even conscious of. No pain had set in yet, and lying there on my back I really did feel as though the pelvic floor was relaxed, but once I slipped a finger in and just let it rest for a moment, I could feel the muscles spasming all around it. And this wasn't anything I could actually feel anywhere in the pelvic region.
So, just interesting to note how intense an experience orgasms really must be for the muscles involved and, as most guys have stated, how important it is to try not to tense up during the experience - all easier said than done, as we know.

kegals
This was my worst symptom because it was the most scary and most severe. It brought on some "I want to kill myself pain" and was 100% consistent. I dont remember the sleeping afterward affect but I was in pain everyday so it was hard to tell. I have yet to find anyone on this site that agrees with me but I did a Kegel regime for over 6 months and noticed I no longer had this symptom. Any type of exercise should smoothen the neuromuscular connection, so maybe they just gave me the ability to relax. Others say the wrong type of exercise will increase tension and I see there point. I had used biofeedback machines and the whole bit so it wasn't a typical kegal regimen. Also I should note it totally sucked, I hated doing it.
Age: 26 | Onset Age: 20 | Symptoms: Used to be pain around the base of the penis, now I have golf ball in rectum feeling that can be very painful. | Helped By: kegal exercise/biofeedback (fixed ejaculation pain), antiinflamatory drugs, warm baths (intermediate term relief), hamstring stretching (very short term relief), the biggest being stress management | Worsened By: Stress, Standing for long periods, ejaculating used to make it worse, but not anymore
