hey:
As I've mentioned (probably too often now), my wife and I are trying to conceive. The hot baths that I was taking ran roughshod over my sperm count. But to make sure that that was really the case, I had a TRUS done (trans-rectal ultrasound), to see if I had any blockage in my seminal vesicles.
The vesicles were fine, but the urologist found I had "a slightly inflamed prostate." Now, other docs had said my prostate was fine -- and even this doctor says that the prostate wasn't THAT inflamed (or rather, it wasn't inflamed enough to account for the severity of my urgency/frequency problems -- let alone the perineal burning and testicular pain I used to have). But I wondered: Is it possible that I don't have chronic prostatitis / chronic pelvic pain syndrome if I truly do have inflammation in my prostate? Or is my weekly PT session inflaming it?
I know that the stretching, baths, relaxation, TENS machine have all helped, and that I used to have a split urine stream for years, and that -- until this started -- I used to urinate by pushing it out un a strained way.
Any help answering these would be, as always, very much appreciated.

My understanding is this...
I have an inflamed prostate. My EPS and at the time soft boggy prostate strongly indicated that it is my prostate that was causing my discomfort, frequency etc...Now how did my prostate become this way? Many reasons can add to it. From stress to diet to injury like sitting to much.
So in your case which is similar to mine, you have not had the major pelvic pain that is very much a large part of this forum. Like myself you have an inflamed prostate. Your CP is not related to infection, but most likely a host of many things mentioned above. So over time, your muscles in your pelvis have contributed to your problem. So do you have CPPS?, based on what I have read, you did have some pain, but you no longer have it? But you have the LUTS end of CP/CPPS. Attack this problem with all the information this website throws. Quercetin, relaxation, diet change to check for food irritatives, stretching, basically everything you already know about. Something is causing your prostate to inflame. Those nerves are all connected to each other and cause havoc.
Just think about the fact when a prostate is removed, you have the side effects from the prostatectomy, leakage, ED, etc... So what I'm trying to say is your prostate is inflamed and causing your problems. The scan confirmed that! Your doctor feeding you the bull**it that He doubts the slight inflamed prostate would cause the havoc in your life only confirms to me the lack of understanding that doctors have on the wall nut shaped gland that is our prostate and how huge of a roll it has on comfort vs. discomfort when the prostate is not healthy! I'm not a doctor, just a dude with an inflamed prostate. Regards...

I encourage anxiety prone UCPPS people to consider L-Theanine
Age, 44 onset age 37 Feb 2006 Freq. need to urinate. Sensation of having to urinate soon after going. Perineum discomfort/burning/tightness, pubic area discomfort @ times, poor urine stream, post urine dripping/spray. All symptoms have improved with my protocol. At the worst I give it a 1 to 2 on irritation and discomfort and frequency. Helps: Elavil 5mg for anxiety and mast cell protection, (will only take it as needed) self internal PT as needed, stretching, walking, stairmaster cardio workout and light weights, reducing stress, moment to moment relaxation, deep breathing relaxation and using a Theracane. Makes worse: sitting for long periods, stress, over focusing on it. Currently 95%-98% recovered. Stay positive, relaxed and control your anxiety.

Thank you Webslave, made some changes to my thoughts.
I do have a question? If I'm dealing with neurogenic inflammation which is thought to be triggered by muscle spasm, then can you feel the muscle spasm happening in your pelvic floor? I have not felt or at least been aware of any spasms in my pelvic floor muscles? I have been told they are tight. I will say that early on I felt my perineum muscle spasm like crazy for months on and off and then I would feel my discomfort? So is this what were talking about with neurogenic inflammation?
Regards, Carl and sorry about the confusion...

It's not a spasm like a calf muscle spasm, it's more like hypertonicity, as if you were slightly tensing your muscles on a permanent basis. It's not easy to notice.
