Ejaculation Pain

Post by treynor33 »

I know a lot of people have it.....how many have it right away and how many have it within 8-24 hours or longer....another new symptom for me. I seem to have acquired pain not immediately but within 8-24 horus later. Appreciate any replies. This latest new set of symptoms and flare is just really puzzling me.
Age: 45 Onset: 43. | Symptoms: Tightness, spasms, burning and pain in the perineum. Occassional mild pain after urination in the perineum. Helped by: Cardura, quitting caffiene, relaxation, fiber, stand up workstation and some PT a year ago. Makes Worse: ANXIETY, ULTRAM, going to work, sitting or squatting too long, constipation (big problems), stress, and sometimes unexplainable.

I have similar symptoms- no pain immediately after but then followed (starting 1 or 2 hours later lasting 8-10 hours) by bad nerve like pain in my urethra, along with tightness and soreness in my pelvic floor- perineum, supra pubic, etc. As far as I can tell what happens is when you ejaculate you tense up your muscles in your pelvic floor, including those around your prostate, and those throughout your abdominal cavity- when these muscles contract for ejaculation they are contraction approx 10 times stronger than when you urinate or defecate (a fact quoted to me by a SP trained PT) because of muscle dysfunction underlying chronic prostatitis / chronic pelvic pain syndrome these muscles do not immediately revert to their totally relaxed state and can stay hyper active and tense for a period of time thereafter- causing your symptoms. Additionally the quick tight squeeze during ejaculation on all your nerves in the abdomen/pelvic floor can cause them to feedback for quite a while even when the noxious stimuli has stopped, think about hitting your funny bone and feeling the nerve type pain for a while after- with CPPS, as far as I can tell, our nerves are so hyper active already that just a small pinch (from contracting muscles) can trigger them into a feedback loop that to create pain for days on end- my recommendation is work on moment to moment relaxation to make sure your nerves get as much rest as possible throughout the day, so their threshold can be raised- and along with stretching and deep breathing I would try to limit your movement (walking, or even sitting) after ejaculation- try to just lay down and relax or sit in a squat stretch for a while. A hot bath after can do wonders as well- the main thing is to avoid any activity which normally causes involuntary muscle clenching right after ejaculation (for example I can't sit down and write or play guitar right afterwards because I unconsciously tense my muscles during that period). The good news, in my opinion, is this means your are improving I remember during my worst months having testicular/nerve pain right after ejaculation lasting up to two days- now the pain comes on later and stays for shorter periods of time.
Age:22 | Onset Age:16.5 | Symptoms: Pain with extended sitting, Pain shortly folllowing urination, mostly above pubic bone, Pain after defecation mostly around external sphincter, Pain after ejaculation mostly in Urethra | Helped By: Progressive and Moment to Moment Relaxation, Stretching, Bed Rest (laying down and relaxing muscles), Hot Baths, Hot Showers, | Worsened By: Alcohol (variably), Ejaculation, Urination, Defecation, Stress, Extended Sitting, Sleeping in the wrong position, tensing my muscles while writing or doing anything else stressful/intense, Progress: 65%, basically only dealing with post-ejaculatory pain, onset after about an hour, peaks for 3-6 hours and then remains sore for a day.

Thanks so much for the advice....I've had this crap for 20 months....I just can't believe that after all this time this is now an issue. It used to actually help me on most occasions....I have been in a flare for about 2 weeks and yesterday it seemed to break....woke up this morning and things were good. Had sex and about 4 hours later....it is just like it was a few days ago again.

Sometimes I do get this, but it's mostly when I'm having some perineum pain in any case. Those muscles get rock hard when a man becomes erect anyway....it's strange though, because for me, it hurts like hell when I first start becoming aroused and kinda during, but after, it usually subsides.

In any event, it does not stop me from getting down to business
Age:23 | Onset Age:22 | Symptoms:Urinary hesitancy, moderate frequency, urgency and retention, slight perinneum, rectal and leg pain, frequent abdominal/bladder pain. Red/white spotty irritation on penis | Helped By:hot baths or showers, walking and moving around, pain medications | Worsened By: sitting for a long time, holding urine too long. I'd say I'm about 75% better naturally so far.

I have had this - it was really bad. There appear to be two key trigger points which caused to this pain for me. The first one was internal, just to the left of my prostate. Interestingly, when this TP was flared up, I would have small orgasmic sensations throughout the day which was (surprisingly) an extremely unpleasant experience. The second TP which related to this pain was external on my rectus abdominal (what used to be my 6 pack before chronic prostatitis / chronic pelvic pain syndrome struck!) two inches below my navel and slighty out to the left. This sent severe pain in and around my pubic bone/urethra when it was treated.

These TP's also seemed to relate to weak erections and ejaculations.

The good news is that I'm now 100% free from post ejaculatory pain following treatment of these TPs'.

Age:39. Age at onset:31. Symptoms prior to treatment: Golf ball in rectum, severe urinary frequency (2-3x/hr; 5-10x/night); weak stream; painful ejaculation; coccygeal pain; tip of penis pain; general pelvic pain on left; testicular pain; supra-pubic pain. Current | Symptoms: Urinary frequency 1x every 2-3 hrs and 1-2 x a night; mild pelvic pain on left hand side (all symptoms still improving!)
Helped by: Trigger point release; avoiding exercise; pelvic floor relaxation; Neurontin decreased bladder sensitivity somewhat. Worsened by: Exercise; frequent ejaculation; ibuprofen irritates bladder. Made no difference: Diet; biofeedback; quercetin; Steroid anti-inflammatories; Elavil.

****UPDATE*** I am now able to sit again at work all day, and can perform moderate aerobic exersise again for the first time in 8 years!!!

Mine occured 5 mins after, and usually urethral pain. The pelvic pain occurs perhaps an hour or two later.

The more ejaculations I do in a row, the more pain I will have, and the earlier the pain will occur.

How about it starting about 4-12 hours later an lasting for 2-3 days? Anybody have anything like that? I am just trying to figure out if it is what is bringing this crap on lately. I never had an issue with it before the last few weeks and am not sure at all if I do now :?: ....in fact before the last few weeks, the more sex I had (within reason) the better things were for the remainder of the day and even the next day.

I guess I am asking what is the longest period before someone can say with realitive assurance that they feel pain started after the ejacuation.

Wow...I guess under that scenario I'd have to just go without for a few weeks, hope things are better, then give it a shot and see if it makes things worse.

Mike, how long do you think the flare up lasted for you even if it started 2 days later?

I haven't done anything since Saturday and things still are not right.

My flare-ups, though minor these days compared to 18 months ago, can last anywhere from a few days to a few weeks of ongoing annoyance. None of my pain needs painkillers any more, but the flare-ups can be a bummer, even though they are low level. Typically though, they are on the shorter end.
