My prostatitis (urinary) symptoms started very abruptly at 4 am. But my pain did not develop until about week later.
Curious what experience others had.

Mine started the same way, suddenly one early morning when I woke up...with the beginning of the most severe pain of my 19 months about two or three days later...and then about 45 days later I started having spasms, like lock-ups when I urinated. Now it is mostly mild to moderate pain and still the urinating difficulties, but no bladder issues at all since some external PT near the bladder.

Age: 45 Onset: 43. | Symptoms: Tightness, spasms, burning and pain in the perineum. Occassional mild pain after urination in the perineum. Helped by: Cardura, quitting caffiene, relaxation, fiber, stand up workstation and some PT a year ago. Makes Worse: ANXIETY, ULTRAM, going to work, sitting or squatting too long, constipation (big problems), stress, and sometimes unexplainable.

Urinary dribbling after attempting to stop urination started for me roughly 3 weeks before the major flare kicked in. This coincided with increased periodic pain at the tip of the penis. Moderate pain while sitting was occuring in my tailbone region for the prior 6 months. Once the flare started, everything hurt, including the prostate. I did not see any muscle twitching until the second week of the flareup, with the first week being dominated by burning pain all over the place. My urinary symptoms got to their worst (couldn't pee at all) after the urologist pushed hard on my prostate, which was clearly boggy and irritated.
This is not medical advice, and I am NOT a doctor of medicine or a related field.
* Age:33 Onset: February 2004.
* 99.9% IMPROVEMENT in 2.5 Years with the first year being the really hard part
* Current Symptoms: Mild irritation of perineal muscles on occasion. Relieved for days at a time by a specific stretch (see below).
* Initial Symptoms: Terrible penile, urethral, rectal, and perineal burning/aching with addition afferent sensations.
* Current Treatments: Deep stretching of the legs and pelvis. Most effective: Deep psoas and levitar ani stretch using the first phase of the "pigeon pose" from Yoga. When a deep pulling is felt in the middle of the pelvis next to the upper rectum, symptoms are completely alleviated for several days.
* Past Treatments Hyperprotection of the perineum for 1.7 years, Walking, Rectal biofeedback, Stanford/Wise-Anderson Protocol, Conditioned deep relaxation practice, Men's Multi-Vitamin and an Extra B-complex pill, all seemed to help.

hmm...my pain started in each testicle, then to the area where the abdomen meets the left groin area...almost where the top left point of the bladder would be. It was very intense pain...then the urinary symptoms started, but I had those many years before, which went away, but have always dealt with a bit of hesitancy since my mid teens.
I think it's just different for everybody on what signs they start to see first. I think though that I'd rather deal with the pain than the mind numbing urinary problems.

Age:23 | Onset Age:22 | Symptoms:Urinary hesitancy, moderate frequency, urgency and retention, slight perinneum, rectal and leg pain, frequent abdominal/bladder pain. Red/white spotty irritation on penis | Helped By:hot baths or showers, walking and moving around, pain medications | Worsened By: sitting for a long time, holding urine too long. I'd say I'm about 75% better naturally so far.

My was sort of abrupt. A couple of months before the major one started I was peeing like crazy for 48 hrs and then it went away. A couple of months later it started with frequent urination and got to the point where I was peeing like as if there was no tomorrow for two weeks, it got so bad that I ended up in hospital with dehydration (Yes I was drinking lots of water but when I did it just came out just as quick). The other pains like burning feeling in the penis and on the perineum came later. Strange thing was that when the doc checked my prostate and pressed on it didn't hurt. The doc said that the prostate felt ok. The only feeling I have now is slight pain just below either side of the belly button and this constant mild urge to urinate and slightly sore legs, like as if they are tired.
Age 43: Onset June 2005, Initially: perineal pain, discomfort in penis, very sensitive, sometimes burning. Red around the urethra on penis. Increased urination during night, problems with stream, sometimes constant urge to urinate. Exercise and moving around and laying down helps. Sitting down and squatting makes it worse. Improved to about 85-95%. Symptoms usually mild pain near belly button and slight urge to urinate.

I also have the mild urge to urinate that seems to settle down for weeks, days or even hours during the day only to come back....it's almost as if I am consciously aware of my bladder filling....because it goes away when I do pee. I've been told alpha blockers help this but I'm still very hesitant to use them.
