source of my problems, possible solutions?

Post by lookouth »

I have pretty much located the source of all my triggerpoints and referred pain- the rectus abdominis muscles and the pyramidalis muscle. I will be seeing a pain specialist at a PM clinic in the coming weeks to investigate tirggerpoint injections. My question here is two part. One, could there be an underlying mechanical problem with my pubic symphysis that is perpetuating these trigger points/groin pain. Two, what exactly are 'muscle energy' techniques in relation to Pelvic Pain, I see them discussed frequently- Wise mentions them and so do various PT's online who talk about treating pelvic floor dysfunction. Can anyone show me a link to them, can they be done alone? Thanks
Age:22 | Onset Age:16.5 | Symptoms: Pain with extended sitting, Pain shortly folllowing urination, mostly above pubic bone, Pain after defecation mostly around external sphincter, Pain after ejaculation mostly in Urethra | Helped By: Progressive and Moment to Moment Relaxation, Stretching, Bed Rest (laying down and relaxing muscles), Hot Baths, Hot Showers, | Worsened By: Alcohol (variably), Ejaculation, Urination, Defecation, Stress, Extended Sitting, Sleeping in the wrong position, tensing my muscles while writing or doing anything else stressful/intense, Progress: 65%, basically only dealing with post-ejaculatory pain, onset after about an hour, peaks for 3-6 hours and then remains sore for a day.

For the first 12 months of treatment my physio always found my pelvis out of allignment and that certainly seemed to be flaring my symptoms. Every session I had with him started with some manipulation of my back/pelvis to reallign it.

Might be worth getting checked out by a physio.

On the trigger point injections, I have had two visits to get injections. First appointment he injected something like lidocaine into my sensory nerves, left and right of my lower abdomin and got him to dry needle a trigger point on left just below belly. No real side effects apart from strange feeling in abdomin for a couple of days. Trigger point went away but reappeared. was hard to locate trigger points at appointment

second appointment

brought my Theracane (i recommend doing this if you have one) I felt a bit stupid walking around with it, but if anyone give you any hassle whack them with it  doctor actually got me to write it down for him so he could pass it on to the physio department. Got three points in my abdomin injected with small amounts of lidocane. Result have no more trigger points close to abdomin but still have a couple on abdomin further down. still treating abdominal area with Theracane . Tempted to go back for a third appointment but can't afford it at mo.

so from experience bring a Theracane with you and get your trigger points either dry needled or lidocaned don't bother with the sensory nerves as to me this only had a short term effect

Am I right in thinking you brought your Theracane to help you identify your trigger points and more accurately show them to doctor? If so, I was actually thinking of using my Theracane shortly before a visit and marking with a sharpie pen the spots of my triggerpoints. I plan on asking my doctor to dry needle but also use phenol, a substances apparently used like lidocane but for longer term relief of trigger points in the ab-wall.
I had 20+ Stanford-trained PT sessions, after pulling on one leg a few times for a few sessions my physio assured my pelvis was pretty much aligned. But the spot I was talking about seems like it could be hard to diagnose without an x-ray or scan, but may be easily treatable with 'muscle energy' pt, e.g http://www.painschool.com/case1.html

Yeh you got it, I guess either would work...good luck, post anymore if you find anything about muscle energy techniques, couldn't find anything significant myself
