My Pelvic Muscle Assessment & Uro Visit

Post by Makaveli »

Hi guys,

As promised, I am reporting my visit with Uro Dr. Zietlin and Physical Therapist.

I saw Dr.Zietlin on Monday morning. He was a great doctor. He was very friendly and listened to what I said. He did a complete evaluation. Checking everything from my throat, to my feet. Including the heart and testicles. He eventually checked my prostate and said perfect! He said that I was in good shape. I told him about my slight discharge and told him that one of the previous UROs recommended it, and it happens less often when I do this. He said that I am making too big of a issue out of this and that it is a normal condition. I just create a lot of this fluid. He said some ppl create less and some ppl create more. He said to stop with the tests and to relax and take it easy. Stupid me, I forgot to have him check pelvic muscles and things like that, and answer some questions. I sometimes get intimidated and forget. so mayble ill see him once more later on.

Just today, I saw a physical therapist. I have her card in my walet and if Mark or anyone else wants it, ill be happy to give her information. Her name is Angela (dont know last name off top of head) and works at Telesis Physical Therapy. She was wondering how I knew she knows about Pelvic Floor Muscles and the Dysfunction of them... suprised, I said dont all Physical Therapist know of this? She said, that she was the only 1 of 2 in Los Angeles that knows of it and is trained in it. She said I was either lucky or someone told me. I told her that I guess I was lucky. Anyway, she was very professional, not to mention hot!!!

She did all sorts of checks on muscles near my hips, stomach, butt region, and all sort of excercises clenching my pelvic muscles and rectum.

She found that I have pelvic misalignment? or hip misalignment? something like that. She said my muscles were very tense. She said my (excuse the way I say this) operative (?) muscles were tight. My levantor (?) muscle was tight.

She said this could definitely be the problem. She wants me in for at least 3-4 more sessions and wants me to practice some excercises.

So is it tense tight muscles which are putting pressure around tissue surrounding the prostate which is causing it to leak some fluid into the urethra or is it that I am not ejaculating enough? and have too many dirty thoughts in my mind.

I dont know. Guess I ll try both.

I'm really baffled. :pensativo: If I remember correctly, you have no pelvic pain and no urinary symptoms. There is no evidence that you have any urological disease. Why are you seeing so many urologists and spending hours and hours posting to prostatitis newsgroups? Why don't you go out and enjoy your life? I think you're getting a bit obsessive about this slight discharge thing, and it might be time to consider seeing a psychiatrist.

Yes, Mak, I also seem to remember that was one of the reasons we banned you long ago: you had no pain, just this fluid thing. I have to agree with MastCells: if this is your only problem and you cannot stop worrying about it, you may have a form of Obsessive Compulsive Disorder (subtype Body Dysmorphic Disorder) and need to take a SSRI medication like Lexapro to help your brain escape these tight, imprisoning circles of thought and worry.
Good News! The great ProstaQ is BACK at last! You must try it.

I think leakage is normal anyway. I always remember getting some clear discharge back before I had CPPS. I never was concerned about it at all.

If Makaveli knew how much the people of this forum suffered from pain hopefully he would realize that posts like this about a very minor issue are cruel. It's like going to an arthritis message board to complain about a paper cut on your finger.

Mast Cells,

Well the discharge is a urinary problem, it happens at the end of urination. Also, in the morning the very first session when I urinate, I cannot always get it out, I have to really relax myself or stimulate my belly or bladder a little and shake it around for it to release the rest.
I do agree that my stress and me thinkn about it is causing it more, cuz when I am out and hardly think about it, its not around.

Webslave,

Yes I figured that much, but it would have been nicer if you would have spelled it out to me like you did here instead of kicking me out. Cuz I just dug into things deeper. Maybe I will see the psych but I dont like taking medication.

Really? I didn't know you had that problem. Thats interesting.

Listen, I am sorry I dont mean to offend any of you. Although it may seem very minor to you guys, it is very major to me, and its disabling my life, its constantly on my mind, and because I dug too much into the prostatitis google groups on all that bs info I went even crazier. I dont mean to hurt or offend any of you. Trust me I have my problems with this. If I am offending you and you would rather me not post then I won't. I just that this was a support group with ppl with urinary/prostate issues.

sorry

Mak, Body Dysmorphic Disorder (BDD) is a very serious mental disease, and can cause people to commit suicide. In my opinion, you are a very likely candidate for BDD and should lose no time in seeking help.

hey,
Sounds to me like you want to undergo psychological treatment indeed. A problem like you have should not take over your life - that isn't a healthy response to a problem of that magnitude. I can certainly sympathize, i've been known to behave somewhat similarly myself.

It seems you do have a real physical problem tho. I dont think its a huge deal, or that you should think about it often, but that doesnt mean you shouldnt get treatment for it. To the best of my knowledge, its likely that your discharge and urinary difficulty are related to pelvic floor dysfunction (PFD) - a condition which you have been diagnosed with by a qualified person. I dont see why you shouldnt be treated for it. PFD may not be causing you grave problems, but it is not an asset. I recommend you read the short book "A Headache in the Pelvis" and follow that protocol. Physical therapy is only one part of the deal. A major part (for most people anyway, and certainly for you and me) is changing the way one relates to stress and stressors, which psychological treatment should help you with.

I hope you still feel welcome here; I know I'm willing to take a shot at any ?s you might have.

I think my point was that I don't think that leakage is a problem. I think most guys get this, particularly when they are horny.

Ya, I'm starting to believe that. I usually get excited, produce a lot of this fl uid, then dont take care of it, and it eventually comes out on its own during urination.

maybe not a problem, but it can drive you crazy, I've been on these boards for like 6 months now

Hodologica, this forum was created for the discussion of Chronic Pelvic PAIN Syndrome (CPPS). Here are the definitions of CPPS:
https://ucpps.men/what-is-chronic-prostatitis-CPPS

What symptoms does Mak have that are consistent with CPPS?

Also, please read our posting guidelines:

Hey thanks buddy.

Mast Cells,

https://ucpps.men/what-is-chronic-prostatitis-CPPS states:

"A sensation of not emptying the bladder completely after finishing urinating"

Which is a symptom I sometimes have the first session of urination in the morning. I also was diagnosed to have my operative (sp?) and levantor (sp?) muscles being really tight, by a physical therapist.

If thats not enough. and you guys dont want me here, then I will no longer post.

How can you have Chronic Pelvic PAIN Syndrome, if you don't experience any chronic pelvic pain? I don't think Hodologica or anyone else would be doing you a favor by encouraging you to obsess over this discharge thing. I suffer from OCD, so I know how devastating it can be. I think you would be helped more by a psychiatrist than by a urologist.

[ I've accidentally locked myself out of my account... I've emailed webmaster at ucpps.men ]

You're right, MastCells, in terms of the rules about the forum's scope, so I apologize. Makaveli, you can feel free to PM me, then.

This isn't my forum so I must accept the posting requirements, tho personally I am convinced that their impracticalities outweigh the danger of loss of the forum's focus.

I'm certain I havent encouraged Makaveli to obsess over his physical symptoms. I suggested he seek treatment from a psychologist. But I can't see why a clearly unhealthy degree of concern over the minor symptoms he experiences means he shouldnt also be treated for the PFD with which he's been diagnosed by a qualified practitioner. Why should it be either/or?

Maybe I should add that in my (non-medical) opinion, his psychological symptoms seem more important and (probably) fundemental than his physical ones, and treating them should be a higher priority. Also that it would be in line with the views of leading chronic prostatitis / chronic pelvic pain syndrome mind David Wise to suppose it likely (tho of course not certain) that his PFD is the result of a high stress level, which has the potential to interfere with treatment of PFD or eventually lead to other problems, unless treated. In Makaveli's case it seems that seeing a psychologist will be necessary, and indeed he has already said he'll see one.

But can't you have chronic prostatitis if you have urinary frequency and other urinary symptoms but not pelvic pain? Not to mention the detection of white blood cells in your prostate fluid?
