Well, I hope it is a false positive. The prospect of a multi-drug resistant NG infection doesn’t sound compelling.
Since I took the antibiotics, I feel my symptoms actually got worse. Not sure how that can be. I have some dull aching in the testicles again.
Can this be chronic epididymitis that was caused by an initial infection? I remember uro #4 mentioned something about tubes that can get inflamed. He said it can cause discharge (and I think I saw it mentioned on a website about epididymitis). Would an MRI be able to find that out?

1. You raised the possibility that all previous tests could be false negatives due to below detectable levels of bacteria. However, knowing that PCR tests are very sensitive, if this is the case I wonder whether such low levels of bacteria is enough to cause visible and persistent symptoms; and
2. If there was low level NG infection present, then the follow up treatment should easily clear the infection (in which case there should be a noticeable improvement in your symptoms) - how are you feeling now?

I read about N. Meningitis but it should respond to the same treatment as NG.

He also, based on what I have read; has not mentioned swelling or painful urination which are key to Uretheitis.

Thank you for your replies. I seriously hope you’re right. I’m going through a pretty dark phase of panicking, worrying about a resistant Gonnorhea. I barely sleep and eat, and let’s not talk about work...
It doesn’t help that the culture results take longer and longer and the doc only keeps telling me that he’s in touch with that lab specializing in Gonnorhea infections but that he didn’t receive anything yet. Really don’t know why it would take so long; it’s 10 days now since they took the swab. I’m wondering if that means bad news. A few days ago he wrote “it doesn’t look like Gonnorhea” but since then he became pretty a bit unresponsive.
I keep thinking about why all the tests could have been false negatives; that I never really filled the container half full (because I thought I shouldn’t dilute the first void urine with mid stream urine), or that on some occasions a swab was taken before giving the urine sample.. but it wouldn’t change the facts of course.

I was able to get a hold of one of the girls mentioned in my post and convince her to also do a test. That should provide some reassurance hopefully. After that I hope I’ll be able to move on from this.
And I definitely notice the connection between frequency and panicking now. I can go like every 15 minutes when I’m going through a phase of anxiety. Very interesting to observe.

So the culture was negative so far; According to ID#3 there’s no NG, no CT, no TV. They’re still checking for fungi but so far it’s also not showing anything.
Results say:
1. NG culture: negative
2. Coloration:
- Some gram-positive cocci
- PI-epithel
- No leukocytes

Spot on, seems he was a bit pissed today when I told him Mgen cannot be cultured but only be tested through PCR. He’s a good doc; he runs an ID clinic with his gay partner and it’s always packed with half the local gay community it seams. I appreciate his help providing remote support via WhatsApp.

I’m really wondering what this is then, how to diagnose and then treat it. I realize I’m not the typical CPPS patient — no Prostate issues, no golf ball feeling, no excruciating pain, but penile discharge. I also don’t think I’m the typical IC patient — no bladder pain, but penile discharge. The only thing in common seems to be the feeling of frequency and pressure / itch / tickle / burn in the penis.
There must be some sort of inflammation or irritation going on, but what’s causing it and how do I treat it? Could this be some sort of Reiter’s Syndrome type of reaction, even though I don’t have any symptoms of arthritis or conjunctivitis? I doubt it can be viral (and urethritis caused by virus are rare anyways) because it was clearly reacting to Azithromyzin both times.

Tell yourself that you have a small problem with your urethra, but that even though it's irritating, it's not important. We all have things go wrong with us as we go through life, little things go wrong, nuts and bolts fall off, things wear out, shit happens, injuries happen. That's life. Just accept that something has gone wrong with your body, but that it's like that little dent in your new car — very annoying but it's best just to accept it and think about other things. Chances are if you can accept it, that your brain will say: Okay, then I'll stop putting attention on that area. When that happens, it often slowly starts to go away, or completely stops being a problem. This has worked for me with a number of problems, including tinnitus.

Some good news after all. Girl#2 sent me her results and she tested negative for all 7 STI including Gonorrhea. I understand she took the same antibiotics as I did (I initially informed both girls about the infection, and to not make them panic I told them that I just took those pills from the pharmacy that made everything go away, which I thought at that point; in hindsight, having learned more about the different infections, resistance, and correct antibiotics use, I realise it’s a bad idea, especially given how easily anyone can get antibiotics at the pharmacy here), except this last round of IV shot and pills for the assumed Gonnorhea. This gives me some reassurance that the infection must be cured; I mean, I cannot see a way how one person would be cured and the other person not when both are taking the same antibiotics for the same infection.
I also went to the hospital again where I live and did two cultures on two different days and a NAAT test specifically for Gonnorhea; all came back negative.
So I can focus now on what else could be the cause of my persistent symptoms. At this moment, the most likely explanation I have is:
1. The initial infection was probably Gonorrhea. Researching what ID#3 said, he seems to be right that the short incubation time can only mean Gonorrhea. The fact that it reacted to Azithromyzin but not Doxycycline supports that; ie., cannot be Chlamydia. It cannot be Trich because girl#2 didn’t take Metronidazole, and it probably wasn’t one of the Mycoplasma and Ureaplasma because those are more difficult to treat and have longer incubation times.
2. The fact I didn’t treat it correctly for almost three weeks (when I was traveling and only had Doxycycline) caused either a hell of inflammation/irritation (I hope) or worse, scar tissue that’s causing a stricture. I suspect it could be the latter; especially since this isn’t the first STI I had (I think I had 12 STI over the last 10 years).
So if things don’t get better within the next couple of weeks or months, I guess I have to get a cystoscopy.
I got up to last night to pee and didn’t flush, and in the morning I could see some foamy bubbles staying on top of the urine. This makes me worry whether a potential stricture could be causing me an UTI (which I wouldn’t even notice because my symptoms of discharge and frequency are chronic).
Overall my symptoms are manageable at the moment, especially when I keep my water intake moderate and drink evenly spaced throughout the day. I still have to get up to pee during the night, but overall the frequency is at a level that is bearable. Most guys here must be suffering a lot more than I do. What keeps freaking me out is the discharge.
I’m trying to get massages regularly now, especially focusing on the upper legs region where I understand (according to the HITP book) trigger points are. I’ll also try to not masturbate for some weeks to not irritate things further.
