external/internal sphincter muscle...

Post by bti20 »

I am still convinced that some sort of urethral obstruction is responsible for my (seemingly intransigent) pelvic muscle tension. Looking into the alternatives, I could previously find only (i) BPH - which (correct me if I'm wrong) is doubtful in the case of someone whose chronic prostatitis / chronic pelvic pain syndrome started at age 22; or (ii) urethral stricture caused by scarring after urethral swabs - something I am looking into.

But then I discovered this today:

"If the urethra is narrowed, it could be due to

- involuntary contraction of the external urethral sphincter muscle
- involuntary contraction of the internal urethral sphincter muscle....... "

(it goes on........ - sorry I have forgotten the link)

I have never heard these muscle groups mentioned specifically on this forum. It would be interesting to know what causes their 'involutary' contraction (apart from 'anxiety' - I frankly don't believe this to be of any greater significance than as an exacerbating factor which would apply to any medical problem - AHIP notwithstanding). Any thoughts? Thanks

Hi bti,

My urologist thinks this is exactly what's wrong with me - a spasming urinary sphincter (although I am not sure if it's internal or external.) I am not sure what would cause it to spasm. It would not be so bad except that when it spasms it makes me have to urinate, even when clearly my bladder is not full to capacity. Why does any muscle spasm? I am no doctor and know very little about how the body works. Is it because the nerves in the muscle are damaged and not working correctly, as in multiple sclerosis? If the nerves are the source of the problem, maybe drugs like neurontin would help. Also, it stands to reason that antispastic drugs should help. My doctor is trying me on such a drug, one called tizanidine. No help yet, but I've only been on it for three days. I am hoping this works. Which brings me to another question: what are the stats of men with chronic prostatitis / chronic pelvic pain syndrome on antispastic drugs? If there is a great degree of success, it seems it would validate the pelvic muscle tension theory of CPPS. If these drugs don't work, does that mean the problem is not pelvic muscle dysfunction? I am confused as you about all of this. And as far as BPH goes, I think you are right, you are too young to have it.

That is also where I feel my spasms and I've been helped by uroxatral and going to a PT. He found my sacral joint to be up and rotated forward. He told me muscles will spasm for many reasons. To protect something or due to stress of the surrounding pain. The prostate issues in many of us could have been caused by tight muscles, then the pain, causes those muscles to finally react, its a pain cycle. I'm using PT, uroxatral and a prostate massage to try to break the cycle, so far it might be working, I feel decent today.
Age:29 | Onset Age: 25.5 | Symptoms: Initially burning in prostate/perineum, burning in urethra, uncontrollable muscle contraction before urinating, dual ache in groin, left testicle pain, feeling of golf ball in rectum, soarness in rectum, and muscle spasms, now mainly very mild urinary tract inflammation, burning mildly after ejaculation, some days after physical activity involuntary mild muscle contraction before urinating | Helped By: .5 mg of Ativan, Aleve (naproxen), quercetin, alcohol in large quantities (vodka and soda), stretching, walking, internal massage the perineum area and levators. | Worsened By: sitting, weightraining, jogging, coffee, caffeine in general, alcohol bothered me at first, overdoing sex
