Prostate massage is probably (definitley??) NOT the answer to your problems, and you should view this link if you are thinking of going down this route:
viewtopic.php?t=262

Unless you have bacterial prostatitis, the most common cause of abacterial 'prostaititis' has little to do with the prostate itself. Instead, spasm and inflammation of the pelvic floor muscles is the key cause of symptoms in around 90% of cases.

Read these links and look into getting a pelvic floor assessment:

viewtopic.php?f=37&t=248&p=1214

If you live in New York, Urologist Dr. David Kaufman would be agood choice:

Here is a recent success story from one of his patients:

THIS, not prostate massage, should be your first port of call.

Good luck.

Age:39. Age at onset:31. Symptoms prior to treatment: Golf ball in rectum, severe urinary frequency (2-3x/hr; 5-10x/night); weak stream; painful ejaculation; coccygeal pain; tip of penis pain; general pelvic pain on left; testicular pain; supra-pubic pain. Current | Symptoms: Urinary frequency 1x every 2-3 hrs and 1-2 x a night; mild pelvic pain on left hand side (all symptoms still improving!)
Helped by: Trigger point release; avoiding exercise; pelvic floor relaxation; Neurontin decreased bladder sensitivity somewhat. Worsened by: Exercise; frequent ejaculation; ibuprofen irritates bladder. Made no difference: Diet; biofeedback; quercetin; Steroid anti-inflammatories; Elavil.

****UPDATE*** I am now able to sit again at work all day, and can perform moderate aerobic exersise again for the first time in 8 years!!!

This is sage advice indeed, from one of our best posters. That thread is important to read.

However, I do know that prostate massage has helped some sufferers (for whatever reason). I would just warn RUNAROUND to stay away from the doctors who promote the "Manila Protocol" and to be very careful if he makes an attempt to do self-massage with a "wand."

I HAVE ONLY BEEN IN THERAPY FOR 1 WEEK. THEY STATED I AM "TIGHT", ALL OVER AFTER THEIR INITIAL ASSESSMENT. NEXT TIME I GO I WILL ASK IF THEY FOUND ANY TRIGGER POINTS, CAUSE I'm NOT SURE WHAT WAS GOIN ON AS I WAS A LITTLE NERVOUS. THEY WANT ME TO START DOIND BIOFEEDBACK AS PART OF THE PHYSICAL THERAPY. I TOLD THEM I WOULD DO ANYTHING, JUST CURE ME. I'm STILL CONFUSED IF I SHOULD INCORPORATE PROSTATE MASSAGE AS PART OF THE OVERALL PROGRAM.

You have been there 1 or max 2 times I assume. This is a slwo fix so it will take much longer time. But hang in there, this method helps a lot of people.

THANKS FOR THE WORDS OF WISDOM PASO. MUCH APPRECIATED. I WILL CONTINUE IN THERAPY, I WILL DO THE BIOFEEDBACK, AND I AM CURRENTLY ON MY 7TH WEEK OF LEVAQUIN, THE DR. SAID TO TAKE IT FOR AN 8 WEEK COURSE. (LAST EPISODE I TOOK IT FOR 4 MONTHS) WHEN DOES THE AVERAGE GUY START SEEING RESULTS WITH THE PHYSICAL THERAPY? I've READ HEADACHE IN THE PELVIS. BUT IN YOUR OPINION HOW LONG WILL IT TAKE BEFORE I START SEEING SOME RESULTS. ALSO WHAT IS YOUR OPINION ON THE PROSTATE MASSAGE? SHOULD I LOOK AROUND FOR SOMEONE TO DO IT, OR STICK WITH THE THERAPY? THANKS FOR THE INFO.......

This is a question that I'm constantly asked over on the ICN boards. "How long will it take for the physical therapy to work?" The problem is that we're all pretty unique with our own variant muscle issues. I, for example, have extremely tight muscles very low in the belly which are apparently compensating for very weak muscles higher up. In any case and to answer your question, I believe that if you have an underlying muscle problem, that you've got to stick with it for as long as it takes to get it resolved. If it's a trigger point, it may just take half a dozen sessions.. but if there's a much more intensive issue, then you could expect that it would take longer.

What's heartbreaking is when people stop too early. If the muscles are weak, they must be strengthened. If the muscles are tight, then they must be relaxed. Stopping and starting PT will only prolong the battle to normalcy and getting these muscles back into shape.

Hope this makes sense.

Dr Shoskes states that there are a subset of men who DO benefit from prostate massage, but that they have certain characteristics:
In general, I have not found antibiotics+massage to be helpful unless at least one of the following is found:

1) Positive cultures of EPS and failure of specific antibiotics
2) Large volume of fluid expressed on initial exam
3) Thick clumps in fluid (usually seminal vesicle contents with lots of dead sperm on microscopy)
4) Patient reports that initial massage gave symptomatic relief (at least temporarily)
viewtopic.php?t=103

It might be worth discussing this with who ever is treating you. I'd also agree that for Physiotherapy to work, you need to stick with it. One week in physical therapy is nothing. This takes a lot of time and invloves a combined effort on the part of the therapist and patient. I can tell you from experience that if you do have muscle spasm & trigger points, then recovery is possible although it can be a long and hard road - BUT it is worth it!

Good luck.

Question to Jill,

You know a great deal about IC and I was wondering if you agree that a lot of IC people will benefit greatly from PT work, both internal and external. The PT I have visited have 3 patients with IC that is free of symptoms after years with IC. Whats your take on PT work? Also have women with IC started trying the "Relaxation" method that helps a lot of people with CPPS? Have you seen any IC women that have undergone treatment per the "stanford" protocol?

PT QUESTION?

Post by RUNAROUND »

GOT A QUESTION ABOUT PHYSICAL THERAPY. HOW MANY DAYS A WEEK SHOULD I GO? IS ONCE ENOUGH? I WAS GIVEN STRETCHING EXCERSISES TO DO. HAS ANYONE ELSE HAD TO DO THE SAME. I can't AFFORD TO GO TO STANFORD FOR THE PROTOCOL, I JUST WANNA MAKE SURE I'm DOING THE RIGHT HTING GOING TO THIS PHYSICAL THERAPY. HOW MANY MONTHS OF THERAPY DOES THE AVERAGE PROSTATITIS SUFFERER START TO SEE SOME RELIEF? I KNOW I HAVE A LONG ROAD BUT WAS WONDERING WHEN I CAN ENJOY LIFE AGAIN?

I went once a week at the start, followed by once every 2 weeks after a couple of months. In terms of how long of takes to feel better, I think this is a very individual thing. For me, my urinary symptoms improved after 3 sessions, but my pain took much longer (3 months) to improve significantly. However, I know another patient who went to the same physio whose pain improved much faster than his urinary symptoms. Have another read of the AHIP book - I would certainly give it a few months of sincere effort.
