Clues for tight testicles and early ejaculation?

So I've had this condition for almost a decade now, not gonna go into my life story, but I did want to ask for your expertise. Maybe I can walk away with some golden nugs.

While I do have some pain and a golfball sensation in the perineum, my largest issue seems to be:

1. The muscles of and around my penis contract massively. Penis becomes hard flaccid, balls become tight.
2. This causes early ejaculation.

When I have sex, my testicle sack is so tight that by the time I ejaculate, my testicles are actually coming up and out of the sack itself... it's frightening.

Years ago when I went hard treating it I was stretching, using the wand and using Flomax (tamsulosin) and that solved a lot of my symptoms. Everything went loose and I had delayed ejaculation to the point I just could not finish. So I know I can make this better. Something about the Flomax was interesting... selective alpha blocker, it just loosened everything up.

I meditate every couple of days at the moment and have started to get back into the pelvic wand so just wondering if anyone has any tips on which muscles modulate this type of tightness? Seeing if there are any clues or things I am not thinking of that might be able to give this some relief.
