36M. My problems started about 2 months ago. A little back story I work as a contractor outside and always lifting equipment at awkward angles and heavy things. I was doing a job for 3 days straight during 100+ degree weather and ended up getting heat exhaustion. It was very stressful and I was just burned out. That’s when the urge to urinate started and it has slowly progressed to other symptoms. I also suffer from huge health anxiety. I take Effexor to help deal with that. I have seen 2 urologists done all the tests. Everything is negative. No Antibiotics. Both urologists think it’s pelvic floor dysfunction. I am going to pelvic floor PT tomorrow.
Symptoms:
urge to urinate most of the day.
pubic bone area feels super tight or like cramped after I urinate. And the tightness feels like it’s pulling on bellybutton.
tight belt makes it worse?
feeling of pee stuck in tip of penis after peeing
pain/burning in the whole area after ejaculation
always lower back pain that’s super tight
doing a Kegel feels like a contracting a muscle that is overworked and slow
dull testicle pain sometimes
quick sharp spasm in perineum randomly
bowel movements been in consistent since this started
What helps:
walking makes it feel good
stretching I think
beer can be relaxing and make the urge go away.
drinking lots of water
sleeping. Nothing bothers me at all when I’m sleeping. No symptoms when I wake up. Slowly come on through the day.
sexual arousal makes urge go away? That’s a weird one.
What doesn’t work-
anxiety
sitting long amounts
stress
ab workouts
cycling
maybe coffee? Jury’s still out on this one.
Gonna try
PT
quercetin
magnesium
vitamin d
cutting out nicotine
consistent stretching
seeing a therapist for my anxiety next week.
being more active
not giving a fuck
I’m at my wits end worrying about this. It’s affecting my job, sex with my fiancé, everything is great in my life until this popped up. Any advice is welcome. Thank you!
Regarding heat stroke, it's never been mentioned on this site before as an etiology, but since one of the classic symptoms of heat stroke is cramping, stiff muscles, or muscle spasms, it's possible that your anxious disposition made your pelvic floor susceptible to being pushed over the edge into UCPPS, and the precipitating factor was the heat stroke, which made the underlying condition apparent
Let's see what happens with the PT (hopefully one trained in the Wise-Anderson protocol)

Did PT. We have a whole treatment plan to aggressively attack this. We first started with lower abdominal dry needling. All the needles went in easily except one muscle was like going through hard clay, which is no good. I felt good urinary urgency relief for a few hours and then flared up. The last few days I just had sore and tight lower abdominal muscles, but no urgency at all!
Today, though the tightness in the abdominals went away and the urgency is back. Which is so weird. When I do urinate the abdominal muscles feel super tight, AFTER.
So muscle tightness and soreness in lower abdominals, no urgency.
No tightness, I have urgency. Weird right? Any thoughts?
Anyways we are going internal next week and more dry needling on top. Will update.
Pain from pubic area is gone. The urgency went from bladder to urgency feeling in tip of penis. This is almost gone too. Only comes around after I have a bowel movement.
What I’ve done:
PT with electro dry needling in lower back and sacral nerve stimulation
trigger point release of a ligament that is connected to pudendal nerve.
quercetin. Used approved brand theralogix. This made my flow insane after 2 weeks of taking it.
Aloe Vera pills. Made bowel movements better
squatty potty
lots of walking being more active and less sitting.
pelvic stretches
distraction and better mindset working on my anxiety.
Not out of the woods yet but it’s getting much better. I just have to be consistent.
