I’ve been suffering from prostatitis/CPPS for over 2 years now. My most significant problem is 24/7 painfull/irritated tip of penis. Other symptoms are burning piss and ejeculation (depends on how irritated the tip was to begin with) and perineum pain when sitting a lot, drinking coffee, drinking alchohol, drinking cold water and eating spicy or sour food. It’s a nightmare,
I’ve tried multiple sets of antibiotic, pelvic physical therapy, prostate massage, lots of supplements and viatamines, yoga, losing weight, changing diet. Almost everything I’ve read online. I live in a country that is not very advance on this subject (Israel) so besically the medical world abandanded me and has nothing to tell me.
Because of the pain in my tip I have no sex life with my wife and it’s destroying our marriage (we have two daughters too) and we are in a crisis with no intimacy and only the hardship of day to day life. I am deeply afraid we would get a divorce if I don’t beat this shit or can start having sex again.
Please please if anybody has any idea how to do this?
Other people have a painful tip as well, how do you have sex with your wives without pain?
If she leaves me I don’t know what I would do, I might harm myself.

If it helps, the Doctors in the US don't know too much also, so I'm not sure how much living in Israel plays into it. Other than that, I share the same pain and fears as you and we aren't alone. Sex is a challenge, I kid you not, but if your wife really loves you, she should understand the situation and help be part of the solution. Maybe she is willing and its you with the hangup over this? Idk... It truly is easy to feel isolated and alone about it. I can't lie and tell you I'm fine about it. But this is probably the best place to get info and help. One thing is for sure, antibiotics aren't the answer.

do you have any information on how/ why your symptoms may have begun? The extreme pain in your penis tip is a little puzzling, is it the actual opening to the urethra? Or is the entire head of the penis? do you notice any physical abnormalities near the tip? Is the pain internal or external?

As far as sex, i would use lots of lubrication, extra friction would not be good for an irritated penis tip.

The pain is mostly inside the urethra close to the opening and specifically across the bottom vein of the penis. It feels like a nerve just over sensitive to the touch with the center of the sensation just bellow the head. It’s connected to discomfort in the perineum but opposite to most what I read, irritation on the penis causes irritation in the Perineum and not vise versa.
It’s also redder when the pain is stronger, looks unwell sometimes. The redness is also not symmetrical as there are areas on the penis tip that seem to be more red then others.
I have no idea how this happend. Two maybe:
1. I used my own saliva as lubricant on my penis when I was sick.
2. I used expired (year after expiration date) lubricant on my penis while having sex with my wife.
Both happens the day before it started.
Any ideas would be much welcomed.
How many other people have similar pain??

Forget about the saliva or expired lubricant causing it, not possible, at least long term. Is perineum pain/ tip pain your only symptom? No urinary/ ejaculation force issues?

Burning sensation when urinating or ejeculating but degree connected to how much irritated is the penis tip.
No other problems regarding urinating. Ejaculation seems less strong, with semen not shooting out as before and more spilling out. Erections are strong as always.
Sounds familiar to anyone?

Do you find yourself tensing your pelvic floor muscle involuntarily? This is the muscle you would tense when urinating to squeeze the last of it out.

No I don’t. Urination is good, just burning sensation. It’s mostly burning when I start pissing and when I finish. In between it’s regular.... what is this ???

When you start and stop, the bladder sphincter is working.

So what does that mean? I have a problem with the bladder?

Potentially, yes, or prostate/bladder neck. Cystoscopy may help to define
