Proctalgia Fugax (fleeting anal pain or spasm)

Post by webslave »

A number of members have complained of having an extremely painful cramp in the anal sphincter that comes and goes unpredictably. It can be provoked by a number of things, such as passing a bowel movement, diarrhea, straining at stool, stress, or intercourse. It can wake you from sleep.

Episodes are generally brief in nature, ranging from seconds up to 30 minutes, with each painful episode resolving spontaneously. The exceptional patient may have an episode that lasts for days. Most patients have several episodes per year; it is the rare patient who has 2 or more episodes per month.

Treatment:
  1. A warm sitz bath
  2. Sitting on a hard ball (such as a lacrosse ball). This works by expanding and stretching the spasming anal sphincter in the same way that you stretch a calf muscle when you get a cramp there.
Point (2) above is very effective, so if you are prone to these attacks keep such a ball at work and at home.

My view:

I have suffered from this for several years, and my current protocol is:
  • Heating pad to anal area (or failing that, a hot bath). If you have more than one heating pad, put one on the front of your body too, just above the penis.
  • Slow, deep diaphragmatic breathing, deep enough to feel pressure in the anal area.
  • Insert a finger into the anus and pull the anal sphincter, which is the ring-shaped muscle in spasm, sideways. This is similar to pulling the toes of your foot towards you when stopping a calf spasm ("charley horse" spasm). This can sometimes stop the spasm almost instantly.

I have had this quite a lot lately. It is mostly felt while moving but I can also get it while sitting down. It's like flashes of pain in the rectum. Combined with pain in the perineum it's forcing me to stay in bed with a heat pad most evenings now. Taking tramadol eases it a little bit. It's usually more prominent in the evening/night than in the morning/daytime for me.

I've had this on rare occasions even before my chronic pelvic pain, starting in my late teens. It's a intense ache and will even wake me up out of a very deep sleep and I have to walk around for a half a hour sometimes, even more painful than CPPS
