Hi Guys, I had chronic prostatitis/CPPS some years back which is still there but urinary symptoms are mild. But since I had it my rectum became overly tight which in consequence caused an anal fissure during bowel movements. It has got to the point now where my rectum/sphincter muscle is constantly in spasm and the fissure will not heal. I have had Botox injections which didn't work unfortunately, so now I'm debating whether to go for a full sphincterotomy where they cut the muscle to stop the spasms. Has anyone on here had any problems with this. I feel like I'm gonna be stuck with this pain from the fissure for the rest of my life. Any advice appreciated.

This is a medical question unrelated to CPPS. I suggest you take advice from a surgeon, perhaps with a second opinion.
There are conservative treatments for anal fissure, but I'm sure you have researched that and tried all avenues.
A lateral internal sphincterotomy is supposed to be a safe and effective procedure for treating a stubborn anal fissure, so you seem to be getting good advice.
