Well she's specialized in pelvic physiotherapy, so she has a lot of experience with intra-rectal massage, on both sexes (males and females), and been giving intra-rectal massage (pelvic muscles and prostate such as treating Trps) for hundreds of men over the years.
Is it anatomically possible that scars (caused by trauma from different reasons such as hernia forming, infections, surgical procedures and so on) in the pelvic and even in the inguinal canal area (in males) to "pull" the area of the prostate and/or irritate structures in that area (such as fascia and nerves)? It sounds stange to me. It was also one of my first assumptions/hypothesis... I asked different doctors from different fields all over the years but no one had a definitive answer to that... That's strange as I'm a nurse by myself. I also could not find any evidence based research on that matter...

Much more likely is that you have trigger points in the levator ani muscles adjacent to the prostate
I asked her that, and she said no taut bands, trigger points and so in my pelvic muscles (and I felt she was checking them all, anterior and posterior ones). Very strange. On my next session I want to have some conversation with her for clarifications. Really strange. I'm confused.

I ended up delaying Elmiron because I moved and other life stuff got in the way. My IC symptoms are still minimal, and I'm continuing on hydroxyzine and Prelief. I'm happy to report that alcohol is less of a trigger now than it was 6~ months ago. My guess is it's because the inflammation processes have been dialed back for long enough by the antihistamines and urine neutralizer that the chronic pain cycle in my bladder has begun to abate. I still plan to go on Elmiron to see if I can reduce my symptoms even more.

Did you really manage to get rid of the urethral burning/aches? I'm so sceptical it's possible because I never heard of someone who succeeded. I have it all the time. Before the myofascial release I also had abdominal burning and aches most of the time... now only occasionally and less intense.

Yup. Urethral burning along with pain on bladder filling went away the day after starting hydroxyzine + Prelief + increased water intake.

Amazing, your anecdote is a source of hope for me. I haven't tried anti-histamines yet. I'll try to convince my family doctor to prescribe me hydroxyzine (I'll tell her there's some evidence to support its use in addition to a positive personal exeprience I heard from a first source).
Among drugs I tried so far only alfuzosin (for more than 6-9 months till recently) and quercetin (on-and-off for a few months each time till recently). I think they did no better than a placebo effect for me. However to be honest I think quercetin helped it to be less "burning"... but the sensations have always been there.
I guess it's time to give it a try. Do you still take Hydroxyzine? and for how long? Do you think it can achieve the same effect for you without Prelief? Thanks again.

I still take hydroxyzine. I've taken it for about 8 months now. Overall, the antihistamines have been more beneficial than Prelief, but Prelief is good if I'm going to have something really acidic like coffee; I've noticed it helps prevent flares.

Many men with chronic prostatitis / chronic pelvic pain syndrome have had pelvic CT scans, and MRI scans. If there were an association between chronic prostatitis / chronic pelvic pain syndrome and osteochondroma (bone tumors), we'd know by now.

A Little update, any feedback would be appreciated:
I'm dramatically better since April (when was my last third session of myofascial release) - my abdominal and pelvic aches (pressure/dull aches/burning sensations) reduced in frequency dramatically (instead of having them on a daily basis, almost all the time 24/7, they were reduced to once-twice per week or so). In the last 2 months I also stopped the Gluten in my diet almost entirely (initially for other unrelated reasons, by the way not necessarily has anything to do with Celiac Disease. Based on personal genetic tests, my risk of having or developing this disease - lifetime risk - is even lower than the average - 0.04% compared to 0.12% in the general population which is a risk factor of 0.31x - actually a "protective" factor) and the frequncy of my pelvic/abdominal symptoms is even lower (in recent two weeks I cannot recall them).
I have some evidence to support those interventions, especially the myofascial release, to be more effective than a placebo effect in my case (for example, in recent year since the intervention my NIH symptom index had been reduced from ~24 to ~15 (which considered to be clinically significant as being more than a 6-point reduction). Before that, I tried alfuzosin and quercetin - and those interventions barely lowered the score in 5-6 points (and after a few months the score got back a little higher, back to the mean - what I think could indicate nothing more than just a placebo effect).
Of course I have no clear proof to show those interventions were the (direct if any at all) cause of my dramatic improvement this year, but I do have supporting evidence.
The symptoms I have left with are the urethral/penile symptoms, which are 24/7 as always (aches such as pressure/burning mainly felt in the distal urethra/tip of penis). I'm thinking of trying histamine's receptor-blocking agents such as hydroxyzine, as I've heard a few stories of ppl relieving dramatically those penile symptoms by taking those agents).

My main symptoms I have are pain on bladder filling, mild burning and penis pain.
Have good weeks and even a day recently of no pain only to get symptoms coming back again for no apparent reason.
I know Caedar advocates its use.
What dosage you thinking of and for how long.
I've stayed way from any kind of meds so far - focus has been physical therapy.
