Quite strange, but during the last two weeks I had most of the days when I felt no abdominal aches at all, just the constant burning and tenderness in my tip of penis. Usually it tended to come back 1-3 hours after I go to sleep but in the last days only after 5-6 hours or so. I hope it will sustain itself. I've been taking alfuzosin (10mgX1/d) for more than 4 months and saw palmetto (450mgX2-4/d) for a month. Could have they been helping me with the pain?

This way or another, I plan to visit my urologist next month, any maybe also a psychiatrist, to consider another options (drug therapy - antidepressants/muscle relaxants/anticonvulsants - and other therapies).

I'll keep you updated.

Alpha-blockers like alfuzosin (Uroxatral) can take a few months to kick in, so that could be what's helping. Doubt that the saw palmetto is helping. It's not useful for our condition, according to most reports.

An update:

Stopped the Saw Palmetto as it seemed to made no difference. However, just recently I've started taking Quercetin in addition to alpha-1 blocker which I've already been taking (alfuzosin) on a daily basis and been noticing a further improvement in symptoms - abdominal aches gone completely, my sleeping is much less interrupted (~1-3 times per 7-8 hours of sleep compared to 3-6 times before drug therapy) and now when I press along the penis/urethra and perineal area it feels ALMOST NORMAL compared to having that strong irritating burning/stabbing sensations before taking Quercetin. (effect started to be felt within a week since onset). It seems to help me more than a placebo effect. My (almost) only symptom now is that penile constant discomfort (very "light" burning/irritating sensation). Maybe it could wear off with time... In a month I'll try to reassess using the NIH-Score,

A quick question, Wolfcub. The symptoms you list in your signature and how you described them in jcacciat's thread instantly makes me think of interstitial cystitis / painful bladder syndrome. Have you been checked out for IC/PBS or tried taking an antihistamine and/or Elmiron?

Have you been checked out for IC/PBS or tried taking an antihistamine and/or Elmiron?
I asked the urologist who diagnosed me if it can also be attributed to IC/PBS... he said no. I didn't try those drugs.

Did he give a reason why it couldn't be attributed to IC/PBS? Nocturia, frequency, and pain on bladder filling are all prominent symptoms in IC, as is acidic foods/drinks (e.g., coffee, as stated in your signature) making your symptoms worse.

a little update: I'm not sure if it's significant enough yet, but in the last two weeks I changed the Quercetin dose from 500mg per day to ~1gr per day, and I tend to feel less abdominal aches and less irritation in the tip of penis and perineal/urethra area (yet the latter is always there).
Is it possible Quercetin helping me too and I didn't reach its optimal effect for me?

Lately I've been thinking to try acupunctuire in addition, and another drugs such as SSRIs/SNRIs... My fantasy is still to get rid of that annoying sensation in the tip of penis.

I have no other symptoms (my bowel habits and function for example are just fine, even excellent). About food - the only thing I found affecting is Caffeine... I also tested many other things such as citric acid and spicy food... those seemed to have no effect on my symptoms.

On quercetin: If you aren't taking quercetin with an absorption enhancer (bromelain, etc.), you may need a higher dose than normal to get the full effects, which is what you're seeing here.

On acupuncture and SSRI/SNRIs: I believe acupuncture is best for regional neuropathic pain. I'm not necessarily sure it would do much for you based on your symptoms, but I'm not a doctor. On the drugs, have you tried a low dose of a tricyclic (amitriptyline or nortriptyline)? Those are the first-line antidepressant treatments.

On IC/symptomatology: Bowel habits/functions aren't direct symptoms of IC, just bladder/urethral habits/function/pain. (Of course, as in any pain/chronic condition, things can overlap.) I would very seriously consider IC as an underlying cause, especially with the pain at the tip of your penis (urethra?) and the caffeine sensitivity. My primary chronic prostatitis / chronic pelvic pain syndrome doctor diagnosed me with IC as one underlying factor because of urethral pain along with serious changes in bladder habits with alcohol and caffeine. Now that I'm taking hydroxyzine and Prelief tablets, those symptoms have gone away almost completely, right in line with the IC diagnosis. I'll be going on Elmiron in a month to further relieve this issue.

I'm very happy for you, and amazed, that your urethral symptoms are relieved! It also gives me hope.

By the way, in addition to the abdomen I have trigger points in the areas under my both armpits (region of Subscapularis) and on the sides of the rib cage... When it hurts it feels much like the perineal and penile pain... I wonder if this is all linked somehow.

I've been feeling like I'm on the edge of developing fibromyalgia or some kind of myofascial pain syndrome...

Now found this report:
The likelihood of developing fibromyalgia and chronic fatigue syndrome increases over time in patients with urologic chronic pelvic pain syndromes (UCPPS), but only men seem to experience worsening of their urologic symptoms the longer the symptoms exist, according to analyses of the baseline characteristics of the Multidisciplinary Approach to Pelvic Pain (MAPP) study cohort.
