Trigger point - just started self treatment - need help!

Hi,

I've been trying to sort my CPPS for getting on 2 years now - it's a long road but I'll get there.
Got to about 70% feeling better but could never sustain more than 3/4 weeks without a flare (for no apparent reason it came back)

Anyway, over the last 2 months I've started to do mindfulness meditation, daily stretching and just recently massaging trigger points. The stretching has made me much more supple but has aggravated the symptoms a little along the way (main symptoms now sensation of needing to urinate, left groin pain and radiating left leg pain down to the big toe at times plus penis tip pain in the last few days). The main thing I've noticed from stretches is that the hip rotator muscles on my outer left bum area are severely tight and stop me from stretching the left hip significantly less than the right side that is quite supple.

Anyway, the trigger points I've found in the inner thigh, gluteals have not caused too much problem but the ones above my pubic bone (pyramadis I think) have got really sore after pressing the trigger points - the area feels bruised and I can feel thick leather like bands just under the skin when I do skin rolling. Treating this area has caused a really bad flare - it's even caused mild burning sensation and pain in the tip of the penis pain at I've never had before. Part of me wonders whether I've made things worse - I hardly slept last night the pain was that bad.

Anyway - could this this flare and tenderness in the pyramadis be a good sign? Any tips on how I should move forwards - leave it to settle back down or try gentle massage? Will the burning / penis tip pain subside?

I've been to my GP today who as put me on 10 mg amitriptyline and ibuprofen.
He won't refer me until I've tried it for a few weeks. (I asked to see a consultant physio)

Any advice or insight into my problem would be greatly appreciated.

It's been years since I had CPPS trigger points and pelvic pain, but I do still get bad trigger points in my back and shoulder from bouts of intense computer work.

I can tell you for sure that when I start treating these trigger points, they become even more painful. It sort of rises to a crescendo of pain over a period of days, with multiple treatments per day, and then suddenly resolves. YMMV, but this is a common pattern from what I've heard.

Thanks for your reply.

Do you go easier on the area but keep treating it until the pain recedes?

Do you know if there is anything else in-between the skin and pubic bone other than muscle?
I'm a bit concerned that I might be trying to skin roll / massage a tight band, that I think is muscle, but it's some sort of key nerve or something that I might damage.

Nerves are located pretty deep, so I wouldn't worry about that.

As for going easier on the area, I do the opposite. Not that this is the best way. Even though I run this site, I am not the go-to guy on trigger point treatment. I'm a fossil who got better a long time ago, before trigger points were linked to the condition. You're probably better off taking advice from people who have a better understanding of how to treat them.

I know that it is pretty old thread, but anyway: JammyDodger, did you resolve that trigger points? I am struggling with similar problems, mainly on my right side, there are some very painful spots in the muscle an inch above penis in right side (rectus abdominis? external oblique?) as well as directly in inguinal canal...this area I am avoiding to massage becuase these are pretty delicate structures, ilioinguinal nerve etc...webslave what do you think? is it safe to massage directly within inguinal canal?

Hi European,

After 6 years I’m more or less sorted and pain free most of the time.

After trying everything I can definitely confirm it’s all muscle / trigger point related. I kept at it and eventually the flares get less and less - it’s a long road but gradually things get better.

Finding painful spots is good in a way as this generally means they need to be released. My lower abdominal TPs took many weeks to shift, they got worse and more painful at first but they gradually eased off and are now normal.

For me the break through was the internal trigger points. I managed to find someone to help me with these (a specialist) and then I self treated using a dilator / therawand. Dilator worked best. Occasionally I get a flare and go straight to the internal TPs which brings things back to normal.

For me it’s the left side that seems to be the tight side with the most TPs.

Just go easy with delicate areas, listen to your body and back off a bit if things get too painful / sore. Unfortunately TPs won’t go on their own and in my case seemed to get worse, cause a flare, before things got better.

Jammy, many thanks for your reply. I am struggling with similar issues (pubic area, groin, adductors) but my problem side is the right side. Better periods (4-6 weeks) are interchanging with bad flare-ups lasting 2-3 weeks. CPPS is clearly the most vicious challenge I ever had in my life and brought me on the verge of total dispair and agony. But improvements and hope which I get from my PT, my girlfriend and places like this webpage kept me alive.
