CMT23, Burner2k,
I'm impressed we have very similiar symptoms.
I'm impressed we have very similiar symptoms.
Age: 30 | Onset Age: 19| Symptoms: Urinary frequency, Urinary urgency, constant 24/7 sensation in the penis (in the tip mainly - burning/pressure/discomfort/"wetness"), Nocturia, discomfort and pressure in the pelvic region radiating to the abdomen and becoming severe as time passes since last urination (resolved in 2014 by myofascial release), Stream velocity is somewhat slow and prolonged with an average velocity of ~13cc/min (and max 18cc/min) found in flowmetry test when bladder is filled with 500cc at age 25 (I always feel like I need to press my abdomen to urinate, improved later on when using alpha blockers)| Helped By: especially MYOFASCIAL RELEASE (especially in the areas of hips and abdomen) - generally resolved my abdominal aches, but penile symptoms remained the same| Worsened By: Coffee and possibly some other food as well| Other comments: Quercetin and acupuncture helped me no more than a placebo effect. Age 25-26: Diagnosed with indirect inguinal hernia and medium hydrocele at the same side. After operation many of the acute symptoms disappeared, but the chronic urinary and pelvic symptoms remained much the same.

Re: No pain - just constant sensation of needing to urinate
Yep taking all those meds at once. Don't feel much except for more tired than usual. There was a 3 or 4 week period where the Imipramine and Deanxit gave me huge relief. Then it just kind of stopped. I upped the dose without success. I will come off the Baclofen now as I don't like the way it makes me feel kind of weak and fatigued and I don't think it's doing much. I went more than a decade without any meds at all, but at this point I am kind of up for anything that is sensical and has a chance of helping.Burner2k wrote:Dude, are you taking all those meds daily? Or did u try those meds at various times at the dosage mentioned above? I can attest that Baclofen 10mg 1x day helped me initially a lot. A few months down the line, Baclofen (same dosage) + hot baths would do the trick. Like I mentioned in one of my posts above, a course of anti-biotics + anti-histamines really abated my symptoms; enough to make me get off all kinds of meds. For the last month, symptoms are back (dunno what the trigger was) and although they are not as bad as when it started, its still depressing. I have been celibate now for more than 9 weeks...so definitely that was not the trigger. I drink coffee once in two weeks and keep spicy levels in food to mild. I gave up liquor back in late 2009 and haven't had a drop since then. I exercise (brisk walk) almost daily and do stretching of quads, calves, hams & lower abdomen after exercise. I am truly confused as what triggers a flare up. May be its OAB...My PT here (who is good PT for other conditions other than this) thinks I need to strengthen my core. Should I give that a shot as well?
Are you doing the Wise/Anderson protocol? If so, for how long have you been doing it and do you follow it religiously? I have been at it for 14 months now. Sometimes I think I feel better and sometimes I don't. The internal trigger points have definitely quieted down as it takes much, much more pressure to replicate my symptoms now. (Area high and adjacent to the prostate mostly).
As far as strengthening your core, I don't know if this is counter to what the protocol is trying to achieve. Doesn't Dr. Wise say to avoid sit ups and the like?
It can't hurt to try an overactive bladder med. I may try the same one that you are looking at. I just know that with Vesicare didn't work for me in the sense that the non-stop feeling of wanting to urinate was still there - even if yes, I did actually have to urinate less often. It also took a tremendous amount of effort to get my urine stream started, which was quite disconcerting.
Anyway, keep us updated and I'll do the same.
Age: 39 | Onset Age: 17 | Symptoms: Constant feeling of needing to urinate felt in the tip of penis. Frequency. Urgency. None of the typical pain associated with CPPS. | Helped By: Valium, Klonopin, and very hot baths, followed by lying down still/doing relaxation. Also made almost symptom free for a few seconds following a bowel movement. | Worsened By: stress, anxiety, skipping meals, lack of sleep, extended sitting, coffee, alcohol. NO other pain or discomfort although after ejaculation symptoms will usually get worse unless I lie still for 20 minutes or so. | Other comments: Symptoms began at the moment of ejaculation, disappeared after a few months and then returned a few months later and have not gone away since.

It's very lucky we have the internet now days and we can share and learn from each other's experiences! For the longest time I thought I was the only person in the world with this and I had no idea as to what could bring even temporary relief or reduction in symptoms - for me hot bath and relaxation and sometimes .5mg Klonopin or 5mg of valium.Wolfcub wrote:CMT23, Burner2k,

CMT23, you have already tried amitriptyline, if I am not wrong? When did you started and with what dosage?
I found 15 tablets of 10mg that my father will not use anymore. So I started having half-tablet every 2 days at night.
Does anybody know a place to buy it cheap online? I think in the Pharmacy, you need a prescription

Hi fobo. Yes tried amitriptylanie just 5mg at night. After a few days I felt incredibly sad and depressed and stopped. Imipramine with Deanxit really worked well for nearly a month and then wore off I guess. Not sure about getting it - in the US I guess you have to go to the doctor for all this, but here it's almost all available over the counter.

Re: No pain - just constant sensation of needing to urinate
Thanks. I think I will continue with it for some time once in 2 day. Not feeling any side effect, maybe a bit tired but I have not slept a lot at night. Usually around 5-6 hours and thats not good per-se as I know. Staying late at work and a lot of stress related to it. But can't complain much though, the symptoms seems quite bearable most of time these daysCMT23 wrote:Hi fobo. Yes tried amitriptylanie just 5mg at night. Full bowels - Reduction in symptoms
Does anyone else experience that when their bowels are full, their urinary symptoms reduce? I've experienced it again and again that when my bowels are full I feel almost no symptoms. Then after I go, I feel symptom free for 30 seconds or so. This is always followed by the sensation of needing to urinate, which hits me hard again. I find that I do actually have to urinate again within a few minutes and from there I just have the familiar urgency feeling in the tip of the penis even though I don't really have to go.
I wonder how this ties into the whole trigger point theory. Maybe the whole system is just malfunctioning, but I would like to hear a logical explanation for why full bowels somehow keep my symptoms at bay. Any ideas?

Re: No pain - just constant sensation of needing to urinate
I wasn't feeling good at all in the last 10 days, especially 2 days ago when I could not get back to sleep after urinating 2 times after 3:00. I don't know but apart from my condition, the thought that I could have developed a bladder infection came to me, I just felt the need to do at urine test more then a year after the last one. The results PH 5.5, Density 1030, White cells 1-2 for field, others all negative. I know I shouldn't expect anything from this approach anymore but in my mind I just felt the need to confirm it.

The sensation of full bowels out-competes the allodynia from the bladder for the attention of the nerves.why full bowels somehow keep my symptoms at bay. Any ideas?

Very interesting. It's amazing how different the symptoms feel before and after a bowel movement. It's to the point where I try to refrain from going to the toilet because I enjoy the extra time of almost feeling normal.webslave wrote:The sensation of full bowels out-competes the allodynia from the bladder for the attention of the nerves.

Re: No pain - just constant sensation of needing to urinate
Hey CMT23,
It's been a while since I've posted...but I wanted to jump in and say that I also have *exactly* the same "full-bowel-makes-all-my-urinary-symptoms-go-away" thing that you do. As a matter of fact, you're the only other human being on Earth who I've heard as the same thing. No other person with chronic prostatitis / chronic pelvic pain syndrome that I've met or talked to online says this happens to them. Nobody at the Wise-Anderson Protocol clinic that I went to had this thing, and even Dr. Wise himself seemed befuddled by it and he simply shrugged his shoulders and said he didn't know why that happened.
What's startling to me is the extreme amount of urinary relief that I get from having a full bowel. It's been that way since the first day I felt my chronic prostatitis / chronic pelvic pain syndrome symptoms. Having a full bowel actually makes me feel 99.9% normal. And like you, almost immediately after I have a bowel movement, the urinary symptoms return. I feel like this whole bowel thing is a very important clue as to what my chronic prostatitis / chronic pelvic pain syndrome is all about but I simply have no idea how it all fits, or how I can use this clue to help me do the right things to make myself feel better.
Now, as a quick update, I've been having more "good" days than "bad" days these last two months. Still not a single day that's been "symptom free" but on a scale of 1 to 10, there have been days where I'd say my symptoms are a "1". I've finally been able to return to about 80-90% of my sleeping quality...it's still not the way it used to be...but at least I can get about 6-7 hours a night sometime before waking up to pee.
To add more to the confusion, these last four weeks where I've been getting these greater number of "good" days...I've been doing absolute minimum amounts of the Wise-Anderson Protocol protocol...some times I do nothing for days at a time! No stretching, no relaxation, no internal work at all! As I matter of fact, I checked my calendar and the last day that I did any internal work was more than 20 days ago! I haven't done any relaxation sessions for weeks! And yet I still continue to feel "good". While I'm thankful that I feel good, it scares the crap out of me that it appears that the way I feel seems to be completely disconnected to whether or not I'm following the Wise-Anderson Protocol. Or maybe I've just loosened everything up and now I'm better? The only thing that I've maintained consistency with is that I take two Cystoprotek pills in the morning and two at night. And I do note that my current run of "good" days started almost exactly when I started taking this supplement.
So that being said, unfortunately I'm in a little more pain than usual today. (LOL you can always tell the days I'm in pain because I end up posting on this forum!) Personally, I think it's because I only got about four hours of sleep last night (had to do a short turnaround for work today) and like you I always feel exponentially worse when I don't get enough sleep.

Symptoms: An almost constant feeling of the need to urinate all the time. Pain in the penis, at the tip, and "behind" the penis inside the pelvis. Also urethral pain after ejaculation. No rectal pain or symptoms at all.
Helped By: Hot baths, Dr. Wise's relaxation tapes. Internal trigger point work every two days with the Wise-Anderson Protocol wand.
Worsened By: Ejaculation (changes the pain to a burning sensation most times) Alcohol increases urinary frequency greatly. Falling "off the wagon" and not following the Wise-Anderson Protocol protocol.

Re: No pain - just constant sensation of needing to urinate
Hi Yoink,
Good to hear from you. Very interesting that we both get such huge relief from full bowels. I also agree that this is a huge clue, but I've been to so many Urologists who just shrug their shoulders at this. Webslave said earlier that the nerves to the bowels take preference over the nerves to the urinary system. I don't know if that's it or not. It's so frustrating as it seems like it should be something obvious that could lead to the magic bullet to curing this thing.
I still do the internal work three times a week. I do the relaxation twice a day maybe 80% of the time. I've been doing the protocol for 15 months. I have good moments and some good full days, but I'm not cured at all. This is always especially apparent when I am in situations where I am stuck and can't get to a bathroom - in taxis, meetings, etc. I know I can put far, far more pressure on the muscles adjacent to the prostate than when I first started, but I am disappointed that this hasn't resulted in a huge reduction of symptoms. Another poster here was suggesting that you must go way further in past the prostate to reach other muscles. This seems like a very, very important point. I don't understand why the discussion hasn't taken over the forum. Basically he is saying that if you just go up to the sides of the top of the prostate that you won't get better and that the protocol is hugely mistaken in this regards. I have thought about buying a very long tool, but am weary about randomly poking around deep inside myself! Anyway, I wish more of a discussion would start on this subject.
In the last year I have tried so many meds including:
Xatral
Flomax
Neurontin
Lyrica
Baclofen
Tofranil
Elavil
Deanxit
Atarax
Tramadol
So far it seems that nothing works as far as providing consistent relief. That is very discouraging. All I know is that when I start to feel really bad, I can take Valium or Klonopin, take a hot bath, lie down and I will feel some relief. Sleeping a full 8 hours and exercising is also important. I do almost always sleep all night, getting up once or not at all. If I am feeling really bad, I take Klonopin to knock myself out and my body usually resets to the usual level of discomfort the next morning.
So I continue the protocol hoping that I am one of those guys that require 2 or 3 years to get better. I do have to admit that after 15 months, some doubt has crept into my mind. Despite the fact that the internal muscles can take a lot more pressure, I am still experiencing a lot of the same symptoms. I can say that it hasn't been excruciating like it was in the weeks leading up to the clinic, but it is still a huge problem in my life.

Re: No pain - just constant sensation of needing to urinate
CMT23, sorry to hear that. I'm quite in the same situation...

I still believe that there has to be a daily med that provides some relief. I know Valium helps, but don't want to take that daily due to tolerance and addiction. My long term plan has always been the Wise-Anderson Protocol, but I thought in the meantime that I would try to get some relief from meds. It's extremely frustrating that so far nothing seems to have really helped. Each time I start a new med it involves research and then slowly upping the dose over a number of weeks or month. Each time thus far, the results have been disappointing. I've still got another month to go on Flomax til I reach 90 days, but I don't have a lot of hope at this point.Wolfcub wrote:CMT23, sorry to hear that. I'm quite in the same situation...
There's not too many more meds that I can think of to try. I am still taking Tofranil. Maybe I will switch that to another antidepressant.
Have you tried all the meds on my list? Thus far Tofranil with Deanxit has given me the most relief. But it only lasted 3 or 4 weeks and even after upping the dose, that relief disappeared.
Anyway, nothing we can do, but keep playing the hand that we have been dealt.
