New here looking for advice
Ok here is how it started for me. In July of this year I woke up one day with burning in the tip of my penis. After a few days of it I decided to go to my Dr. thinking I had a UTI. Dr had me give a urine sample but told me that it was rare in men and told me I probably had prostatitis and gave me 10 days of Cipro. After about 5 days the burning went away but I took the full course of Cipro. About a week later I was masturbating and as soon as I ejaculated the burning returned. So this time I went to a urologist who did a DRE and said its probably prostatitis and gave me 30 days of Cipro. Well 14 days in I could no longer take the side effects of the Cipro (major tendon pain) so I called the urologist and asked to be switched to a different antibiotic and he said Cipro is the only antibiotic that reaches the prostate. So I found a new urologist and she said probably prostatitis and gave me doxycycline and Rapaflo to relax the prostate. My only symptom was what I would call a slight discomfort in the penis on good days to pretty significant pain on some days. On the bad days I would take 800 milligrams of ibuprofen every 8 hours but that wasn't helping much so doc gave me Percocet. On bad days I take the Percocet and it seems to help.
So after the 30 days of the doxy my symptoms never went away but would wax and wane so my urologist did a cysto of my urethra and bladder and she said I had a very minor stricture and my bladder was fine. She said she also saw some inflammation and prescribed prednisone. I will say that I do have more good days then bad days and I have also had a few days in a row that I feel nothing.
Then this past weekend I had a little discomfort at the tip of penis nothing more then usual but when I woke up Sunday it felt like I had to pee but I couldn't. So I laid back down for about an hour and the pressure of having to pee was getting worse and worse. So about 2 hours later I told my fiancé that I would try and pee one more time if I couldn't she would have to take me to hospital. So on the last try I stood over the toilet for about 2 minutes and finally was able to go but stream was very very weak. As the day went on I would get the same pressure feeling and I was able to go but weak stream. So I want back to urologist on Tuesday and told her the story and that on most days I still have irritation so she gave me more Rapaflo and another antibiotic to take for a week and come back in a week.
So today is Wednesday and my stream is better but I still have irritation on the tip of penis. So my question is, does this sound like CPPS? I am 36 and live in Cincinnati and can't find anybody for internal physical therapy. Do you guys have any advice for me? Thanks

Re: New here looking for advice
Yes, that does sound like CPPS. Taking prednisone for more than a short time for chronic prostatitis / chronic pelvic pain syndrome is not advisable or indicated. You should probably be on an alpha-blocker like Rapaflo (silodosin), or another, long-term. Dump the antibiotics; you should not be taking them at all. See "Dr Shoskes List of Tips for Urologists Treating CPPS" here.
Be careful of Percocet (acetaminophen and oxycodone). Oxycodone is an opioid. Read warning about opioids:

As for a therapist, do a search of the forum using Cincinnati or your state; failing that I suggest the nearest expert therapist, Marilyn Freedman in NY.

Re: New here looking for advice
I take at least one or two narcotic pain killers every day. I usually take a vicodin in the morning and one in the evening. If i don't do something to address the pain, I can't function the way society expects me to. If I don't take pain medicine before bed I can't sleep and I am liable to wake up. If I don't take pain medicine when its bad in the morning I won't have enough energy or willpower to get around much during the day. Thats awfully depressing to hear that opoids are "bad" for CPPS. Narcotics, hot baths, playing classical piano music, and stretching are basically my protocol and they help a lot. Is it one of those things where if you stop taking the narcotics for a few weeks and then start up again its not as bad as far as mast cells are concerned?
Age:19 | Onset Age:19. July 7th, 2011. | Symptoms: burning testicular pain, burning perineal pain, poor range of motion in right hip flexor, post ejaculatory pain (usually), decreased libido, depression | Helped By: narcotic pain killers, stretching (not sure) | Worsened By: alcoholic beverages, mostly unsure what effects my symptoms for better or worse.

Re: New here looking for advice
Opioids are not a long term solution to this problem. chronic prostatitis / chronic pelvic pain syndrome and IC involve (inter alia) degranulating mast cells, and opioids also degranulate mast cells. It's that simple.
So unfortunately, using opioid pain killers is not the answer. I wish it were not so. Opioids are useful in breaking the vicious cycle of pain and neuronal windup (central pain sensitization), but I have seen too many men get hooked into using them and then make no further progress on getting better.
No simple solutions, as in many things in life.
It is what it is.

Thanks for the advise. I now have come down with a nasty head and chest cold. Any recommendations on a prostate friendly medicine to take.

Avoid decongestants. Bed rest is the best medicine.

Need a few answers
In my previous post I told my story but here is a recap my pain started in July of 2011 and I didn't get any relief until November. Since November I have been pretty much pain free except for a couple of days here and there. Now if I masturbate too often the pain comes back a little for a few days and goes away. I am currently on 25mg of Amitriptyline and 8 mg of Rapaflo (silodosin). My question, is my pain still there but the Amitriptyline masks it or am I almost cured. Should I leave everything the way it is and just enjoy being pain free or should I start weaning myself off the Amitriptyline and Rapaflo and see if pain returns.

Difficult to say. If your only strategy is those 2 drugs, the condition is likely to return once you stop them. You should try to enlarge the repertoire of measures you are taking to control the symptoms before stopping the medications.

What would you suggest? I have read headache in the pelvis. The closest physical therapist is Cleveland and that's just not an option at this time. What else can you suggest. So in your opinion the pain in my penis is still there but the drugs are masking the pain

Re: New here looking for advice
Well, stick with that in any event. Are you doing stretches, self internal massage, relaxation exercises (including using special music like that by Dr Jeffrey Thompson), meditation, etc?

Re: New here looking for advice
Your obstructive and irritative voiding symptoms need a 24/7 treatment. Please ask your GP for Celexa (anxiolytic dose) and Trazadone (nightly). I had similar stories to many here, and am better with these two medicines. This disorder is one of an exquisitely sensitive bladder neck, which cannot heal because of use (urinating), abuse (sex/masturbation), and stress related tension (mostly unconscious or wilfully uncontrollable). I firmly believe a chemical holiday is the only cure. Other therapy is only partially effective in my view. Don't just take my word for it...try it yourself. By the way, it doesn't have to be those two medicines. But the principle is the same. You need a holiday from your sympathetic nervous system in order to heal.
Age: 28 | Onset Age: 23 | Symptoms: Essentially resolved. Previously severe and unrelenting bladder neck pain; frequency, and burning during urination | Helped By: Trazadone, Celexa, Flomax, running | Worsened By: symptoms have stabilized for the most part, sometimes ejaculation makes them worse

Are you fit? Do you eat well? Are you under a lot of stress? Do you smoke/drink? Eat too many carbs? Sit all day? ETC...see this site's main pages. Do not underestimate your lifestyle and the response chronic prostatitis / chronic pelvic pain syndrome has to it.
Addressing your lifestyle will probably make things better.
Seems to me you are looking for a pill that does not exist. I am yet to find any medicine/supplement that helped me. Probably only help you in your head as chronic prostatitis / chronic pelvic pain syndrome can wax and wane all over the place.
I get good long runs were I hardly know I have this by: relaxation, diet and exercise. Many men like me are never really cured but it bothers us "very little" when we change the way we live.
The post ejaculation pain/sensitivity is a killer. To stop that symptoms I only do it before I sleep. For some odd reason doing that stops it. Probably via relaxation. You could try that.
Pills are not always the answer in chronic prostatitis / chronic pelvic pain syndrome sadly.
