Very interesting study, the second one.

The main problems I see are that the ESWT needs to be targeted at intrarectal trigger points, so they have to be identified first, a skill in itself. Then there's the fact that a few sessions with a ESWT therapist will not cure a patient, only begin the path to a cure. The treatment needs to continue for quite a while, so a cost factor comes into play. Perhaps each patient can have his own ESWT module for home treatment?

webslave,

I suppose those are 'problems'. However, those problems are not unique to shockwave therapy, but every therapy type in existence (the reliance on a trained professional with skills to identify and treat a condition, the need for more than one treatment, etc.).

The goal is to affect the condition to such a degree that it can be managed effectively with more traditional, non-invasive methods such as physical therapy, self management (stretches, baths, nutraceuticals, stress management, etc. etc.). Most patients require varying numbers of treatments at varying durations (usually in the order of 5-8 sessions) before almost complete resolution is achieved. After that, patients are given all the typical advice and management strategies to maintain their improvements.

It likely wouldn't be practical for patients to have ESWT machines at home, unless they had a qualified professional to operate and apply the ESWT (not to mention the cost and other impracticalities, such as attempting to shockwave one's own perineum).

The shockwave is but one approach, and any successful treatment protocol sees it as such; simply one ingredient in a complex recipe. It is not a stand alone, but it certainly an important ingredient.

I'd be interested in some patient reports. If you have successful patients, perhaps they can join up and share?

Absolutely. Some patients are reluctant to share their experience. This is in part due to the sensitive nature of the condition. Other reasons given include: the desire to "move on" with one's life after resolution (due to the long-term psychological and social impact that has accompanied the condition), and also the number of emails required of them by prospective patients (responding to everyone becomes quite a burden).

PM me to speak directly to prior patients (or email at [email protected])

Private emails are not possible through this forum.

So the reasons you give are not really valid.
PM me to speak directly to prior patients (or email at [email protected])
We don't allow private PMing here. Question: you are Kirk Andrew, a chiropractor in Cornwall, Canada. Are you here to recruit new patients? That's fine if you are, but please be open and straightforward about your intentions. Thanks.

Great.

They aren't 'my' reasons, but the personal wishes of some patients, so I'm not one to judge their validity (upholding privacy is paramount to testimonials).

Emailing isn't allowed? I'll suggest your forum, and if anyone wishes to pay to join, perhaps they'll post. Otherwise, if prospective patients want accounts of prior patient experiences, they can email me.

Not sure what the confusion is about? I thought it would be pretty clear from my handle "pelvicpainspecialist", my email "drkandrew", and from my posts.

Not sure why you'd feel labelling 'education' as 'recruiting' is appropriate. I'm simply sharing some promising information and research (upon which I am achieving results).

People can make up their own mind.

Thanks.

Yes, I worked it out. It would have been better to introduce yourself in an absolutely clear way. But never mind, let's move on ...

Unfortunately, we've had people join up before for purely commercial reasons, and if they're selling snake oil it's not tolerated. Chiropractic is borderline, IMO. If you wish to educate, fine, and if you are canvassing for patients, that's fine too, as long as you are clear about it.

Please note that I am personally inimical to chiropractic theory in general, so you will need a thick skin. Not willing to expand on this now; do a search here if you want to read more.

We are attempting to provide solutions for patients who have suffered with this condition for years with little relief and now valid answers.

Also, we are in the process of conducting research in the form of case reports, which will eventually involve more powerful studies as we see more patients.

To which 'theory' are you referring, exactly?

You're allowed your opinion, of course.

I'm quite inimical to many aspects of psychology and psychiatry, to be honest. Same goes with many aspects of allopathic medicine (especially in reference to treating Chronic Prostatitis).

Magnetolaser and enzyme therapy in combined treatment of patients with chronic bacterial prostatitis

Avdoshin, V.P., Andriukhin, M.I., Mikhaǐlenkov, T.G.

Abstract
A comparative trial of efficacy of magnetolaser therapy, enzyme treatment with longidase and their combination in chronic prostatitis has shown that these methods significantly relieve symptoms, normalize laboratory, urodynamic and immunological indices, reduce structural changes in prostatic parenchyma. Combination of the methods provides a persistent remission for 12-month follow-up.

Efficacy of low-intensity laser radiation and antibacterial therapy in the treatment of chronic prostatitis in the presence of sexually transmitted infections

Neǐmark, A.I., Khrianin, A.A., Safina, O.N., Neǐmark, B.A., Kondrat'eva, I.S.

Abstract
We studied 94 patients with chronic prostatitis (CP) in combination with urogenital chlamydiasis. The patients were divided into three groups. Group 1 consisted of 32 patients with bacterial prostatitis and sexually transmitted infection (STI). They were treated with fromilid in a dose 500 mg twice a day. Group 2 (n = 27) received also low-intensity laser radiation (LILR) on the prostatic gland. Group 3 patients (n = 35) with abacterial prostatitis were given fromilid (500 mg twice a day). We studied prostatic hemodynamics with color doppler mapping. A specific feature of prostatic vascularisation in CP and STI versus healthy subjects is heterogeneous decline of vessels density in ischemic zones with parallel decrease in these vessels diameter. A peripheral prostatic zone in CP patients with STI was characterized by lower vascularisation than central one. This deteriorates the course of the disease. The results of the study show that adjuvant LILR in CP patients with STI raises efficacy of therapy by 11%. Investigation of prostate vascularisation and hemodynamics of its vessels in CP patients with STI using transrectal ultrasonography and dopplerography provide detailed information about prostatic structure allowing for lesion zones. This facilitates choice of an optimal complex treatment with application of LELR in peripheral inflammation of the prostate.

Abstract
The therapeutic effectiveness of laser-electromagnetism combined therapy in chronic prostatitis is studied. Four patients were treated by irradiating the prostate's correlative parts and acupoints with 650nm semiconductor laser and pulse electromagnetism respectively. Three of them were recovery and only one was ineffective. This demonstrates the feasibility of using laser-combined electromagnetism for assistant treatment on chronic prostatitis. A further study on combined therapy of chronic prostatitis using 810nm, 650nm semiconductor laser and pulse electromagnetic wave is also introduced.

Pryima, O.B., Lysyk, O.S., Pidlisets'ka, M.M., Klymko, I.F., Svizhyns'kyi, I.M., Fetsiak, I.V., Huzychak, O.I., Bilyns'kyi, O.L.

Abstract
In this paper, a therapeutic clinical series-based plan is submitted of a combined pathogenetically substantiated treatment of patients with chronic prostatitis. Sine-modulated flows were utilized as were centimetric waves, phonophoresis with methyluracil ointment to prostate according to the authors' own methods, as well as laser therapy. Positive results were obtained. Significant improvement of the consistency of the gland as well as indices of spermatogramme and microscopy of the prostate juice were recordable in 75%) of the patients. In summary, the proposed therapeutic alternative for patients with chronic prostatitis can be recommended for use along with other treatment options.

A CLINIC REPORT ON 119 CASES OF PROSTATITIS TREATFD BY LASER ACUPUNCTURE

You Shizheng Zhang Shilin (Yantai Shan Hospital,Yantai) Li Shirui (Antiepidemic Station of Zhi Fu district,Yantai City)

Laser acupuncture produced remarkable effects on 119 cases of prostatitis. All the patients were from our clinic. Most of them were young or middle- aged. Patients were divided into two groups, laser acupuncture group (therapeu- tic group) and laser rectum irradiation (control group). For the therapeutic group, the helium-neon laser acupuncture and moxibustion needle was inserted into the prostate through the perineum, and for the control group, fiber optics was placed in the glass stick and inserted into the hole of rectum to irradiate indirectly the prostate.Patents in the two groups were all treated 20 min- utes per a day for four times as a course. The cure rate was 53.4%,in the threapeutic group including 88 cases and in the control group of 31 cases, the cure rate was 22.60%,the difference being very significant (p0.01).The total effective rate of the therapeutic group was 98.9% and that of the control group was 83.9%.The difference was also very significant.(p0.01). In the therapeutic group the prostate was reduced 0.449cm in average, and 0.108cm in the control group. There exists a very significant difference (p0.01).
