Hi. I am new at this forum. Hopefully I am posting in the correct section and my story is not to long. Your feedback will be deeply appreciated.

A year and a half ago it started, a strong burning sensation in the genital area, pain in the perineum, pain in one testicle and very frequent penis discharge with bowel movements (no frequency or other discharges). I went to the urologist who made a quick urine test (no culture) and determined I had prostatitis and I could discard a UTI. For the pain in the testicle, recommended an ultrasound analysis which came back to be a cyst. Another symptom I have since then (haven't heard it from anybody else with CP and maybe is not related) is pain in both legs.

I went over a course of fluoroquinolone (spell?) antibiotic for 20 days without results and the burning sensation increased being unbelievable when driving or sitting for a long time. I was given Zithromax (4 days) together with the other antibiotic (10 more days) and help a little but didn't resolve, finally what stopped the pain was taking bath sitz. The leg pain continues, also the discharges (only with bowel movement not related to hardness of stools).

After a long bicycle ride (I used to be a mountain bike fan, seems I will have to forget about it) the symptoms came back two months ago, though a little different; pain the testicle (that stopped by it self), pain the perineum and penis pain (pain and burning sensation while sitting). Went to a new urologist who had me take a urine test, a urine culture and two specific cultures for Chlamydia and micoplasma. Everything negative. Prescribed Bactrim for 20 days, advil and bath sitz. Rapidly symptoms disappeared but 2 weeks from stopping the treatment; the penile pain came back again and have it right now.

Questions. For discarding bacterial prostatitis (or choosing the correct antibiotic) is it worth having a semen culture or a prostatic fluid culture? Can the leg pain be related?

Thanks a lot.

Do a search of the posts on this site about bike-riding and chronic prostatitis / chronic pelvic pain syndrome --->Pudendal Nerve entrapment. For example some recent posts:viewtopic.php?t=863

Also look at the CP.comsite for a lot of good info.

It sounds like your urologist ruled out bacterial prostatitis. The antibiotic Bactrim may have had an anti-inflammatory effect, and Advil is an anti-inflammatory. Again, the cp.com site has good info on diagnoses and treatments of CPPS. Take a look at neurological, stress, myofascial, and/or muscular aspects of CPPS. Ask your Doctor to give you a better diagnosis (e.g. nonbacterial chronic prostatitis/CPPS).
