Tip of penis sensitivity and pain

Male pelvic pain, prostatitis, IC

Has your penis-tip sensitivity to friction worsened so it bothers you even if you're just sitting?

Yes; it started as occasional sensitivity to friction, but I can always feel it now.

Tip of penis sensitivity and pain

Post by kevin »

Sensitivity in the head of the penis (the glans) is a subject that comes up frequently here. A number of people mention discomfort when the penis rubs against underwear, or during sexual activity. What I would like to know is this: after enough chafing/rubbing/irritation, does this symptom eventually turn into always-present pain? In other words, does it reach a point where it hurts/bothers you even if you're sitting still, with no friction?

If this sounds like what happened to you, I'm curious what pushed it over the edge: was it friction from your clothes, sex, or something else? I have this sensitivity to friction, and want to treat it wisely so it doesn't get worse. So far, keeping it covered with a bandage or adhesive has worked fairly well.

Feel free to respond or PM.
Started: Spring 2003; high urinary frequency and pain associated with bladder filling; urinary hesitancy; pubic/prostate/perineal discomfort; Helped by: trigger point therapy, Afrin nasal spray, Cymbalta, hydrocodone (small doses), distraction. Makes worse: sex.

Not medical advice. Consult your doctor.

Kevin-

I've had chronic prostatitis / chronic pelvic pain syndrome twice (once in my early twenties and now in my early thirties) Actually, probably never fully recovered from first attack so maybe now it is a reoccurrence.

The first time around my symptom was strictly tip of penis pain/irritation. It lasted for about 2 years in my early twenties and slowly went away over time. I vividly remember how uncomfortable that was. I would be so sore that I would go to the bathroom expecting to see blood in my underwear but nothing was there. My skin wasn't even red, but it felt like I had rubbed it raw. I found that by wearing extra tight underwear (NO boxers) I was able to get by with the discomfort. I was SO miserable back then, but I would trade my new symptoms in a heartbeat for my old symptoms back then.

So, I did recover from the tip of penis pain and was symptom free for about 10 years and then SURPRISE - it's back with new symptoms 10X worse then the first time.
Age:35 | Onset Age:31 | Symptoms:Frequent urination w/ high urine output each time, (good day 6 times - bad day 12 times) continuous sensation of having to urinate; burning, raw feeling throughout abdomen; Bowel patterns changed (increased); cramping | Helped By:External & Internal Massage, Hot Tub, Playing sports, stretching, alcohol

I think this has pretty much been the only PAIN I have had--but I had it in 2002 and now again in 2008-9.

It is weird discomfort: the penis feels sore sometimes. Othertimes it feels like there is sand trapped in the urethra. I also sometimes get the "cold" feeling on the tip, as if water has been poured on it. Then I go to look and it is dry.
Age: 38 | Onset Age: 32 | Symptoms: Frequency, the feeling of having to urinate right after voiding, pain in the penis—especially on the tip, dripping, urine retention, mild burning. | Helped By: Hot baths, exercise. | Worsened By: stress, caffeine, possibly diet.

Hi Kevin, if I'm remembering the first year of my chronic prostatitis / chronic pelvic pain syndrome correctly (it's a bit hazy at this point), I had glans sensitivity and urinary meatus pain, and started wearing compression shorts to keep my penis away from my clothes. Eventually, my tip of penis sensitivity and pain both subsided to the point where I can usually wear boxer briefs with little problem:

To try to answer your question: yes, if I recall correctly, my tip of penis sensitivity/pain bothered me even while sitting still at my worst point in that first year, but after wearing compression shorts for some time my tip of penis sensitivity/pain subsided. So, at least in my case, this period of elevated sensitivity/pain was not a point of no return. My condition did improve quite a bit with attention paid to limiting stimulus to the area, and passage of time (among other factors that may have contributed to my improvement).

PS: These days I don't find tight underclothes like compression shorts comfortable- boxer briefs are the most comfortable underclothes for me right now.

It's funny.
I've had that cold tip feeling on and off for many years before the CPPS.
Also - the occasional raw feeling in the tip of penis - for an hour or 2 - then - gone - completely.
Now - with full on chronic prostatitis / chronic pelvic pain syndrome - I mainly get it when I ejaculate or try to sit in a chair that isn't properly padded.
Interestingly - I located a trigger point in my back that refers pain - and the cold raw feeling to my penis - it also makes my knees weak and feet burn and itch.
I'm focusing on that right now and am working it for 15 seconds 3x a day.
I'm also getting compression shorts.
thanks...

Also: Radiating from hips down thighs, Urinary urgency even after voiding, lightning bolts of pain that shoot through my rectum, penis & perineum, muscle spasms in my bottocks, fear of sexual arousal or erection. | Helped By: Hot Water Therapy, RSA Breathing, Pacing more than sitting, Avoiding Negative People, Painting & Writing, Painkillers. | Worsened By: Wearing Pants, Anger, Panic, Depression, Stress, Vigorous Sex, Prolonged Sitting, Constipation, Abrupt Movements, Lifting/Carrying anything more than a Gallon of Milk, Forgetting to do the above things I'm helped by. *Ejaculation is a Mixed bag: It definitely relieves pressure & pain in my Abdomen (as well as a good emotional release) - But, causes bigtime burning in my penis & perineum for 10-45 minutes after.

Your symptoms sound so familiar. I have never found referring trigger points. I'd love some more detail on this trigger point you've found. All the best!

Sorry for the long delay in a response - the triggerpoints i'm working are near my tailbone - about one inch to the left and right of my spine.
Hope that is helpful.

Hi,

That's interesting that JoVoChigcago has come across trigger points down at the tail bone or coccyx, that's where I first physically felt that there was pain, before I too, felt the weak and itchy feeling at the knees. I will try and locate any possible trigger points there so. Like you, I did feel that 'cold' and 'wet' feeling just inside the urethra.

Did the release and massage of these trigger points cause these symptoms to decline at all?
Age: 25 Onset: 23. | Symptoms: Burning in urethra, urinary frequency/urgency, 'golf ball' in butt syndrome, muscle tension form pelvis to calves, constipation, IBS, testicular pain/inflam, bladder pain when nearly full, difficulty relaxing muscles (coccygeal, levator ani etc.), pain and muscles tension in anus/perineum. Helped by: quercetin, relaxation, baths, heat packs applied to perineum. Worsened by: Stress, driving, sitting, jogging, caffeine etc.
