Dr Wise Clinic

Post by Sherradin »

Hi I am new to the forum but have spent years researching my condition. I have been told it is irritable bladder, IC, and PNE. I am in Australia and am female, A type, banking, three small kids.

Well thanks to your site I am off to Dr Wises Clinic next month Dec. and look forward to any help they can give me. I spoke to a woman in Pennsylvania who did the course and who was demented with the pain and is now pain free. It took her two years and to say she hates urologists now is an understatement.

I have been through a pain clinic. But have improved 50% with meditation, yoga, swimming and a very rigorous diet from where I was two years ago. This is when I started reading the HITP

The posts have helped me a lot so thank you. I know the site is for males but as a female it has helped make a big decision as some of the posts symptoms were so similar. Just exchange the penis for the urethra and all the burning urination symptoms are the same. Our PTs in Aust are not familiar with trigger point work so I hope I can get something from the trip. How cold will it be in Dec in California?
Watch out for me on the road it will be my first time driving on the 'other' side.
CPP since 2005. Prior to CPP always overly fit and active. I am female. Had two natural births: singleton 1998 and twins 2000. 2002 emergency back surgery - L5S1 herniation. Then recurring UTIs. Usual antibiotic overload. Then constant debilitating burning bladder and reaction to many foods. Australian Pain Clinic 2007. Turning point was Dec 2009 Attended Wise Clinic in Santa Rosa USA.
Was helped by strict diet but now eating normally after years of restricted diet - wonderful. Helped by: stretching, relaxation, yoga, trigger point, warm baths. Worsened by: stress, sitting, abdominal or glute exercises and salicylates
Medication: Now off all pain clinic meds no more Endone or Elavil only Lyrica 50 mg as Dec 2010 just reherniated L5S1disc and had discectomy. Its taken years but I feel I am over it.

Welcome,

Northern California can be very cold in December... :-D

Glad you are going there to get the proper treatment for your condition. There is an IC site that I know Webslave can guide you to...I don't remember the site. You are right though our conditions are rooted I believe from the same thing...

Stress
Diet
Genetics...

Best...

I encourage anxiety prone UCPPS people to consider L-Theanine
Age, 44 onset age 37 Feb 2006 Freq. need to urinate. Sensation of having to urinate soon after going. Perineum discomfort/burning/tightness, pubic area discomfort @ times, poor urine stream, post urine dripping/spray. All symptoms have improved with my protocol. At the worst I give it a 1 to 2 on irritation and discomfort and frequency. Helps: Elavil 5mg for anxiety and mast cell protection, (will only take it as needed) self internal PT as needed, stretching, walking, stairmaster cardio workout and light weights, reducing stress, moment to moment relaxation, deep breathing relaxation and using a Theracane. Makes worse: sitting for long periods, stress, over focusing on it. Currently 95%-98% recovered. Stay positive, relaxed and control your anxiety.

Hi there,

Welcome to the boards, Sheridan. You're correct that our members are primarily male, but we've had a woman or two over the years! You are certainly welcome to read and post. I believe the community Carl's referring to is the IC Network, if you'd also like to check that out.

Doctor Wise offers no guarantees (who can?), but I've yet to hear a complaint from anyone who has attended the clinic. They have a female section in the more recent editions of A Headache In the Pelvis, as well, so you can be assured of their interest in female patients.

On a side note, Jim, I'm really glad that you went to the clinic. It's great that you seem to be enjoying yourself and benefiting from the experience. Please do keep us informed about how things go!
I am not a physician. This is not medical advice. Consult your doctor!

Age: 26 Onset Age: 17 Symptoms: Shooting, nerve-like pains throughout the penis, which abruptly hit and leave. Testicular pain, perineum pain, burning/irritative urination, extended pain after ejaculation. Occasionally, some allodynia or ache in the coccyx/sacrum/thigh/buttocks/legs. Diagnosis: Pelvic floor dysfunction, degenerated lumbar disk, and mildly herniated lumbar disk. Helped By: Physical therapy, pain management doctor, hot baths, therapy pool, stretching regimen, breathing exercises, relaxation, distraction. Worsened By: Arousal/ejaculation (worst), constipation, panicking/obsessing, other triggers depend upon current symptoms. Tests/Prior Treatments: Too many antibiotics to count, multiple urine tests (all normal), testicular ultrasound (normal), bladder and renal ultrasound (normal), lumbar and pelvic MRI with and w/o contrast (revealed disk problems), Elavil 25mg (caused retention), Flomax 0.4mg.

Thanks everybody. I know there is another Australian there with you this month that is a male. Wish him luck we both go to the same Prof V in Sydney who is so supportive and although an endogynecologist has been to see Dr Weiss in SF and is keen to know if we get any help from the clinic.

It is funny to read the board and see so many good outcomes. My fifth edition is very well thumbed. I have googled hard to find a poor one but I dont want to get my hopes up. I am just glad there is a clinic like this available. I feel very lucky to be going.

The IC Network is great but they like the pain clinics talk about medication and living with it. I think my body can heal I just have to help it and if it turns out to be mainly stress well great I know what to do.

What seems to be common to me with some of the guys on this page is the fanatical exercise regime. I had two twin pregnancies and was back at the gym doing abs and butts etc..very flat stomach obsession etc.

Image

I am like this woman and have spent my life squeezing my abs in...after reading the Trigger Point work book it started to make more sense..I have been on such a strict diet for years but that isn't the cause now I think it is neuromuscular but I need the experts to confirm it.

In case it disappears, I'll quote it here:
Which Bond Girl is more likely to become Incontinent?

The majority of women who present with incontinence and prolapse in my clinic exhibit a muscle pattern like Ursula Andress on the right. They have learned to over engage Rectus Abdominis and External Obliques, causing narrowing or hollowing of the abdomen. This narrowing increases the intra abdominal pressure down onto the pelvic floor.

Thompson et al (2006) showed the incontinent women in their study were unable to perform a voluntary pelvic floor muscle (PFM) contraction using a localized muscle strategy, instead activating all the muscles of the abdomino-pelvic cavity.

Smith, Coppieters and Hodges (2007) looked at pelvic floor activation in continent and incontinent women. EMG activity of the pelvic floor, abdominal, erector spinae and deltoid muscles was recorded during rapid shoulder flexion/extension. In the continent women, pelvic floor EMG increased before that of the movement initiating deltoid. In the incontinent women the deltoid contracted prior to the pelvic floor muscles.

In many incontinent women, the pelvic floor EMG was observed to decrease before the postural activation, yet their intensity of pelvic floor contraction was greater than the continent women. A situation of too little too early, and too much too late!

The authors state that this muscle pattern has obvious negative consequences for the pelvic floor and lumbar spine.

How often do you hear fitness instructors tell their clients to draw their navel back to their spine? How many women have learned to draw back at the waist to flatten their stomach? Enough years of practicing this action may turn it into a reinforced habit.

McGill (2004) warns that narrowing the abdomen may de-stabilize the spine due to the reduced circumference (base of support) of the muscular platform.

Many women with incontinence (and lumbar dysfunction) display inco-ordination of their pelvic floor and core muscles aggravated by the centralization of abdominal tension around their waist and chest wall.

The challenge is to identify women with this narrowing pattern, and teach them to release the tension and allow the muscles to fire in a correct sequence and appropriate intensity. This takes time and individual training from a skilled pelvic floor physiotherapist to ensure the optimal outcome. It is not unusual for some clients to take two or more sessions with me to breakdown their habit and relearn the correct motor pattern.

To answer the question, if Ursula with the narrowed waist added heavy lifting, chronic coughing, large babies, inappropriate abdominal exercises or straining during bowel opening then she would be the girl most likely. James would never have seen this coming but once again 'M' would have been a step ahead.

It is such a small world I am sure Dr Wise reads this as well as knows half of the people before they arrive at the clinic. I have read your posts so I feel like I know you already. I am looking forward to it but I was just down at the beach swimming laps and it is so lovely and warm here that I might find the cold a shock.

I like you am much improved but still deal with it every day and it impacts everything I eat and do..so any more improvement I can get will be great. See you there.

A little bit different than here then

I'm coming over by myself - it's going to be very strange without my family. I used to be away a lot with work in the past but the most I've been away since we've had the boys (4 and 2) is a couple of nights. At least I'll get some decent sleeps :-D

Me too.. husband being a good man and minding the kids (boy 11 twin girls 9). Not only the decent sleeps but the shopping might be fun. I have planned a few other nights in Sonoma and San Fran before I come back - especially if I feel good from all the relaxation shopping will be fun. I just hope my expectations are not too high..and they do find a trigger point. I have all the relaxation handbooks and trigger point work book so I am hoping the hands on gives me the skills to get out of pain and be a bit more fun to live with. Strict diet only drinking water for the last two years has made me a healthy but dull girl.

Likewise on the Triggerpoint - I'm really hoping Tim can find something I've been missing that's causing my perineum to constantly be dysfunctional. A good bit of advice I got from someone that went to the clinic before was to take a marker pen with you and get Tim to mark the external triggerpoint spots, then take a photo of yourself, makes it easier to self treat when you come away from the clinic.

I'm also a big water drinker - mainly down to the tramadol giving my headaches if I drink any booze. One drink these days and I'm flying - very different to the vast quantities drunk during student life :jester:

I'm not spending that much extra time over there - flying into SF on Tues 8th, staying in SF that night, having a day on the 9th looking round SF before grabbing a hire car in the afternoon and driving up to the Flamingo. Flying back on the 16th.

The marker pen is good advise. I will bring a camera as well because I fluff around finding nothing except tight muscles at the mom.

I used to love a red..haven't drunk for so long because it burns my bladder that I don't even miss it. Except when I am at parties all the AA people come and talk to me thinking I will want to share too.

I fly 9th see DR Smith morning of the 10th Tim that afternoon at Flamingo and home 18th. So hoping the relaxation might put me in a good place for the last few days. I am getting my hire car at the air port..and heading straight down staying at Best Western in Sonoma...wish me luck with the car it should be an adventure.

I've got Dr Smith on the morning of the 10th as well, at 10am, then have Tim at 9am on Friday 11th. I'm staying at the Flamingo so looking to drive down from Santa Rosa to Sonoma in the morning. Yes, the driving will be fun :) Need to make sure I get a sat nav to make life easier.

Sounds like we will be meeting at Dr Smiths office. Poor man talking to us lot about pelvic agonies all morning. I lashed out and ate three pieces of sushi for the first time in 2years last night. Boy am I suffering for it today even with an Endone. Its the vinegar in it. Do you think we are meant to cut down our drugs before we get there so we can feel the pain?

I have booked a sat nav in my car but dont know how I will go with the american accent.
