So which test is more diagnostic, and the most accurate, the EMG or the PNMLT? Does it depend on how good or experienced the doctor is, or or can the neurologist stuff it up, or is it best to have both tests ??

I should probably take a moment to point out that EMG testing is typically used to detect muscular conditions. However, a nerve conduction study (the test I mentioned where a pulse is monitored from one end of the nerve to another) is often performed on the same visit.
EMG and NCS are both well established medical tests, and as I said, are often performed in tandem. However, many doctors (including some of the so-called PNE experts) claim that the PNMLT is worthless. Seems to be quite a bit of peculiar conflict there.

Age: 26 Onset Age: 17 Symptoms: Shooting, nerve-like pains throughout the penis, which abruptly hit and leave. Testicular pain, perineum pain, burning/irritative urination, extended pain after ejaculation. Occasionally, some allodynia or ache in the coccyx/sacrum/thigh/buttocks/legs. Diagnosis: Pelvic floor dysfunction, degenerated lumbar disk, and mildly herniated lumbar disk. Helped By: Physical therapy, pain management doctor, hot baths, therapy pool, stretching regimen, breathing exercises, relaxation, distraction. Worsened By: Arousal/ejaculation (worst), constipation, panicking/obsessing, other triggers depend upon current symptoms. Tests/Prior Treatments: Too many antibiotics to count, multiple urine tests (all normal), testicular ultrasound (normal), bladder and renal ultrasound (normal), lumbar and pelvic MRI with and w/o contrast (revealed disk problems), Elavil 25mg (caused retention), Flomax 0.4mg.

I Flunked the EMG Test!
I had the EMG test last Friday, 1/2, to see if there is nerve damage in my anal/pelvic area. When I got stuck all around the anus with needles and then had current run through, I did OK, not pleasant, but I handled it and there was no nerve damage showing around the anus. All the anal muscles functioned well also.
Then I got stuck with a rather long needle in the right buttock cheek, like between the sit bone and the anus (ischial?). I never felt so much pain in my life and the test had to stop. I couldn't handle it!
So, I get no offical results for that area. The Dr. who tested me even pushed with her thumb on the area between the sit bone and the anus and I felt a lot of pain, and more of it on my left one!
So then the Dr. had me go immediatley to a spinal MRI, and it showed a couple of bulging disks in my lower back, but nothing that could be referring the kind of pain I'm in, according to the Dr.
The Dr. doesn't think I have a Pudendal nerve problem because of how well the test went in my anal area but I wonder if she really knows (not that I want Pudendal nerve issues).
So, next I go to back to my Colon and Rectal Dr. with these results for advice, but I'm truly feeling desperate.
Before I can even think about what to do next, I need to get out of the horrible pain I'm in. Does anyone know of a good pain manaement Dr. in Dallas Tx. that understands medications? I haven't been able to find one yet. I really would appreciate any help!

I too had an LIS procedure for a chronic fissure, but my problems started before that.
You didn't "flunk" the EMG--you had some pain (I failed exam would mean no feeling at all!)
The bulging disks may be causing rectal and pelvic spasms. Have you noticed any? Are you overweight?

I'm not overweight at all, but I am out of shape. I just can't exercise. I am in too much pain. I can't tell if I am having rectal spasms. I am in constant pain. The Dr. didn't think I was in spasm.

Do you think a nerve could have been damaged during the LIS procedure?

Sounds like everything is severely aggravated.
I had the long needles and electric shocks in my feet and legs to test for neuropathy.
It caused a flareup of neuropathy for about 3 days.
So perhaps this is similar.
And yes- you didn't flunk the test.
If you didn't feel anything - then you'd really have something to worry about.
Give it some time to simmer down.

Yeah - I was on Levaquin for over a year and before that Cipro.
I know - my Wife read it can cause Neuropathy - which is why I stopped taking it.
An Ex Uro wanted me on a "management" dose - even though it wasn't doing anything good for me.
I felt pretty sick and lethargic the entire time I was on it.
Bad stuff.

Criminal is right.
I mainly stopped going to this guy because my Daughters friend was a secretary for the urology group and a common "office joke" was to call people like me "GOMO's" (Get Out Of My Office).
I've found out since then that this is a common "in-joke'" they learn in Medical School to refer to Neurotics or Hypochondriacs.
And the standard practice is to dole out a prescription and get rid of you as soon as possible.
Things like that can really shake your faith in the medical profession if not humanity as a whole.
Before I found this message board - I didn't know where to turn anymore.
