Having incurred over $100,000 in negative tests I can tell you that Webslave is probably 99% correct. You simply overgoosed your pelvic muscles and nerves and they have not recovered. I have been dealing with it for over 2 years and am not any closer to resolving it. I do believe the more you mess with it with injections, etc., the worse you may make it. However, I understand how counter-intuitive doing nothing may be. There probably should be another warning on Viagra . . .
Tiger, you say you're no better after 2 years. May I ask what treatments you've tried. Have you been to a good PT, done stretching & relaxation? Seems in that time frame some improvement should be noticed, unless the same triggering behaviors have remained.

I have had Botox in prostate and in pelvic floor; pelvic floor p.t. in accordance with Stanford/Wise-Anderson Protocol; stretching; TUIP do to flow issues; most recently a obturator internus block which alleviated all symptoms for about an hour; my pudendal nerve is inflamed on MR neurography.

Tiger, thank you for giving the details. Sounds like all signs point to PNE. Have you looked into Dr. Stanley Antolaks injections for PNE?

I believe sitting while masturbating is a major component in hitting the breaking point. It's adding so much extra stress on the pelvic floor which already being strained through arousal and climax. After I got this I immediately noticed how arousal while sitting would trigger a flare, whereas all other positions wouldn't. I think there should be a warning thread at the top stating, "DON'T MASTURBATE WHILE SITTING".

I can second the remark on injections: 4 years ago I took the step and had intraprostatic injections done under general anesthesia and it left me bedridden with pain for a week. These were performed via the perineum, so basically they are sticking a needle in already dysfunctional muscles.

Moreover, it took 3 weeks to clear up the blood from my semen and urine.

As for the pain: it did absolutely nothing.

Not surprising if you consider the fact that they inject antibiotics and xylocaine which means that if you don't suffer from chronic bacterial prostatitis, the effect will be less than zero. Why do I say less than zero: because I ended up in more pain than before and enjoyed bloody semen and urine for weeks on end.

Their only guess is that oral abx do not penetrate the prostate well enough so they literally pump your prostate full with Cipro or whatever to get rid of any germs.

Suppose you could even benefit from the xylocaine: as most people here can tell you, the effect wears off after a couple of hours.

I toyed with the idea of having denervation of the spermatic cord done. Luckily they first did a trial in which they injected the cords with xylocaine to mimic the effect of the operation. The result was mild relief for a couple of hours, but I could still feel pain at the base of the scrotum, probably referred, so I wisely decided to step away from any surgery.

I believe there are some instances where one could benefit from surgery to combat pain, but these would involve people with proven nerve damage after an accident or previous surgery. In the case of spermatic cord denervation it had a reasonably good effect on people with post-vasectomy pain.
I doubt that in our case any injection, surgery or any invasive procedure is the answer.

I thought injections are mainly diagnostic, and it depends who does them, and I suppose how your condition occurred eg by TRAUMA, I also read a article that the injection has to be about 1mm from the site that is causing the nerve to be irritated. Apparently some people have had success but only with the right doctor. I've had 3 and by 2 different doctors the second doctor, well this was his first pudendal nerve block they had no real effect, only the anaesthetic,
