Pain increase after defecation
For one year I am suffering of pelvic pain. At the beginnig the prostate was hurting and I took antibiotics without any result. No bacteria has ever been found and all the exams seem to exclude any prostatitis infection or inflammation. Docs (i have seen all the best in Italy...) say there is no evidence of urinary desease. But I can't sit anymore (or the pain turns to be terrible) and this is ruining my life, private and professional. Furthermore, since a few months a new problem have arisen: after defecation the pain increase so much: It starts with pain in the urethra, below the testicles, then pain in the low back (sacrum) and pain in the testicles. The pain couse tachycardia and for a couple hours I feel very bad. Does anybody has experice of such a symptoms? Thanks in advance for any info and or suggestions.

Hi Matt,
at the beginnig it semt to start with hard stools, but now it is always and products for softening stools do not help. You are the first I have ever heard who has a similar symptom. A proctologyst told me it could be an inflammated urethra that couse pain after the passage of stools. The big issue is that in the past this was and exception, now it is nearly everyday, and I start to feel panick just to think of defecating....
Ciao Mario
Ps: yes, tachycardia is rapid heartbeats

Hi Mario,
I also have very bad pain after a bowel movement, either hard or soft, and this in turn causes very bad sitting pains, the pain can last for up to 6 to 7 hours. The only thing that has helped me so far is valium, I know it is not a cure , but it reduces my pain by 30% to 40%.
Valium is a powerfull muscule relaxent. Seems like the muscles/nerves contract and go into spasms after a bowel movement. I am going to try stretching exercises, and also see a physiotherapist who specialises in Palvic Dysfunction.

Ciao Eugene,
our cases seems to be very similar. I do also have dramatic sittin pain and Valium hepls. I do also take bacoflen: none of them solve the problem but help to survive. I agree that most probably muscles spasm is the cause. but the hipotesis of the inflamed urethra is also realistic. If the Uethra is sore, stools passage can hurt.
Furthermore I recently had a urine and sperm test with PCR methodology and gonococcus has been found after DNA amplification. Now my confusion is total. I was sure to have chronic prostatitis / chronic pelvic pain syndrome but now there is a risk of uretristis coused by that bacteria (even if my uroligist says that it is just a case of contamination or 'false positive').
Now I am waiting for new test, then if bacteria are definitely exluded I will try Botox....
i need to find a solution.... life can't be like this!!!

Hi Mario,
I agree this disease is very confusing, and you will hear many conflicting views, tests may show positive or negitive, if you have very precise tests like broth culture , it may be so sensitive thet it may pick up bacterica normally in the UT . I think that in some cases a bacterial infection over time may turn to a non bacterical prostatitis due to damage to mucles and nerve tissue.

Pain after bowel movement is related to pudendal nerve entrapement
(I think) as well as chronic prostatitis / chronic pelvic pain syndrome although I normaly got releif after b.movement.
Many don't buy the idea, but if you do buy then that is an 'option'.
Looking at first may seem all to easy (just decompress the nerve and that's it) but it ain't.
The whole thing is little sun some rain and a lot of fog.
Good luck.

On a almost similiar note.......
I don't have the chronic pain/spasm that many on here seem to have. My symptoms are mainly urinary (frequency, nocturia, occasional weak flow) and bowel. Since this started, I have frequent bloating and gas. It seems when my urinary symptoms 'lighten up', my bowel symptoms increase.
Can someone explain how my inflamed prostate is causing bowel problems as well? I am confused.

Most of the theories show the opposite: the colon inflammation may provoke prostate inflammation, not the other way round. Personally, I suffered for many years of irritable bowel syndrome. Then, suddendly, without any clear reason a prostatitis emerged and turned cronic (as the irritable bowel syndrome).
I have an older brother who is a doc, and he soffered in the past for same illness (irritable bowels, then prostatitis) and he is convinced that the 2 are linked and that bowel deseases are one of the most frequent causes of prostatitis. Cosequently the cure is diet, life stile and... time. He says ATB are just poison for people with bowel inflammation + prostatitis.
Good luck

I have this symptom, but usually only when I'm on decongestants, or if I've just passed a hard stool. It's a throbbing pain in the perineum.caiomario wrote:after defecation the pain increase so much

That's a spasm. Try this next time: get immediately into a hot bath or apply a hot water bottle or heating pad to the area. Works like a charm when it rarely happens to me.

There is a well-known and already described reflex for that.caiomario wrote:Ciao Eugene,

Well, probably your urologist is right. There are 28 species of Neisseriae (the genus to which Neisseria gonorrhoeae (gonococcus) belongs. So, a positive PCR for "gonococcus" might be just a positive PCR for a non-pathogenic Neisseria. The PCR manufacturers themselves suggest that the diagnosis is not conclusive until you also have :Furthermore I recently had a urine and sperm test with PCR methodology and gonococcus has been found after DNA amplification. either 1) a positive culture for gonococcus or 2) a positive amplification test different from PCR [/quote]
Not quite sure this is your best option. What if Botox makes it worse (there have been anecdotal cases of that)? I think you need to make a very well-informed decision before going for Botox. For example, at the recent AUA Meeting in Chicago, there was a discussion on what kind of receptacle Botox is delivered from to the patient (glass or polypropylene). Seems like choosing one or the other can undermine the success of the procedure. Now I am waiting for new test, then if bacteria are definitely exluded I will try Botox....

Your inflamed prostate sends a signal to the spinal cord that something is wrong. Since there is an intersection between the bowel and prostate signals (they share a common innervation area) the return signal is sent to both the prostate and the bowel (gut)Northstar wrote:On a almost similiar note.......

Thank you Dr Dimitrakov for you reply.
I have appreciated the info and I will follow ur suggestions regarding cultures.
Unfortunately I have already had the Botox injection (today) and now I am so scared...
I was strongly reassured that there is no risk and no side effects. I was told that the only risk is that the Botox doesnt work (20%-25% of the cases) and that the improvent may disappear after 2 months (I had it done in a public hospital in Itlay and I paid nothing... so there was no interest... no profit behind the advice)
I definitely dont need to face worse simptoms: I have had enough pain in the last 12 months...
