Pain below prostate

Post by ldh000 »

I have pain and discomfort just below the prostate. Also have to urinate often. Could this be a stricture?

Could be. See a uro and have a cystoscopy.

I had a urodynamics test done and every thing tested out ok. Doc said I have CPPS. The test was painful. Dread a scope.

This all started with I believe is an acute prostate inflammation 1. 5 years ago. Woke up with chills and sharp pain just below the navel. It got into a chronic state ever since. The uro gave me Flomax, several antibiotics, nothing works. Discomfort in scrotum, lower back, and perineum. Will this ever go away? When I have a full blown flare-up I go nuts. What should I do next when I see the uro in July for a followup? This is constant 24/7. Also urinate 10-12 times a day including 2 times at night.

If a had a stricture, would a urodynamics test pick up on that?

Flexoible cystoscopy is often used, not so bad.

But then:
AUA symptom index combined with maximum urine flow is an accurate, time-saving and cost-effective tool for predicting recurrent stricture in patients with a known urethral stricture. It can be used to direct decision making on further invasive studies and treatment

Greetings and welcome to the forum -

From the tone of your post I'm wondering if you have a lot of anxiety about this (which wouldn't be abnormal). Once something becomes chronic we can get into a cycle that just starts to perpetuate and feed off anxiety which leads to inflammation and tightness in our muscles from tensing.

I would say that you need to realize that things can and do get better. It takes time in many cases - hard to accept, but once you embrace that you will hopefully stop dwelling on what's wrong and what can you do to get better. Start to do everything you can that will contribute to better health - diet, exercise, rest, relaxation, elimination of stress.

Work with your Dr. (URO) and find out what he would recommend - do you need some mild medication? Would physical therapy for PFD be helpful? If you don't think your URO is in touch with your condition then find one that seems to understand about the whole chronic prostatitis / chronic pelvic pain syndrome / IC / PFD condition(s).

I went to a URO the other day for a retention scan (not my regular URO) and was talking to him about Pelvic Floor Dysfunction and what was going on - his response to me was that I have a very complex condition but nothing more beyond that. Thankfully - I have a neuro-urologist who seems to be much more in touch with this thing. His first 2 instructions to me were weight loss & anxiety management. He feels that is right now the most pressing issues. THe guy is brilliant and fully aware of what is going on in my body - and I think recognized that this is what needs to be done first.

Maybe consider trying the any accredited quercetin product as well. They offer a free bottle. The members and moderators on this board are very knowledgeable based on their experience. They can help a great deal - I know that I've benefited.

I know this is frustrating. However, there are many many many guys that no longer post here because things got better over time.

Also - fill out your signature so that we can know a little bit more about your condition.

Good luck and we're here for you.

Hi there,

Welcome to the boards. It looks like Webslave and elitnyski have offered you some great advice. We will never give you official medical direction, as we are not qualified and it would be very irresponsible of us. That said, we do have plenty of educated opinions!

It's possible that you have a stricture. This is a common urological entity, however, and they can usually get a good idea of the likelihood via symptom observation/whatever testing they decide to do. If your urologist doesn't think it's necessary to scope you, there's a good chance that it isn't.

Having said that, your peace of mind is also important. If you need to completely rule out stricture for your own sanity, then you should request a scope. If the urologist doesn't take you seriously, find another. A flexible cystoscope is still going to piss off your urethra for a while, but it's not nearly so traumatic as a rigid scope.

If your urologist rules out all relevant causes within his ability to diagnose and treat, then chronic prostatitis / chronic pelvic pain syndrome is probably a good diagnosis. Your symptoms are certainly in the neighborhood of many of our past and present members. The bad news is, it's a very unpleasant condition. The good news is, the majority of men can and do get better with the appropriate measures.

Regardless of what you have, elitnyski's advice about anxiety is very worthwhile. Anxiety is associated with a proven chemical and nervous system reaction, the end result of which is heightened pain. Distracted your mind, do relaxation, diaphragmatic breathing, whatever it takes. Just keep yourself calm.

When you are ready to proceed with the chronic prostatitis / chronic pelvic pain syndrome diagnosis, I suggest getting an evaluation of your pelvic floor muscles. Most guys here found that they had pelvic floor muscular dysfunction which was causing a good deal (if not all) of their symptoms. With effort and the guidance of a specialized physical therapist, this can be treated effectively. It's not an approach most urologists recognize, because the idea of treating muscles and anxiety for a seemingly urological problem is a big paradigm shift for them.

Can't hurt, but has a good chance of helping. Please keep reading, stay involved. This board is a great source of support.

As I stated before this CPPS/PROSTATITIS all started with pain below my navel and chills one night. It has been chronic ever since. The chill and the navel pain went away the next day, but all the chronic prostatitis / chronic pelvic pain syndrome symptoms remained 1.5 years later. Did any body else have a situation like this? Would be grateful for anybody's input on this.

My symptoms started after a bout of the flu...Got real dehydrated and could barely pass urine for a day. I actually remember that part of the flu...A few weeks latter I came down with this.

The flu was just the straw that broke the camel's back...All the years of pelvic tensing and then a night of violent vomit and the runs which ='s more pelvic violence ( pelvic myoneuropathy)....Voila!!!! CP/CPPS is born.... BTW...I had my flexible cystoscopy to rule other serious conditions out...Which really helped ease the anxiety....I gladly took the piss of fire for a few hours after the procedure so I can move on with my life...Talk to your urologist...I'm not a doctor.

I encourage anxiety prone UCPPS people to consider L-Theanine
Age, 44 onset age 37 Feb 2006 Freq. need to urinate. Sensation of having to urinate soon after going. Perineum discomfort/burning/tightness, pubic area discomfort @ times, poor urine stream, post urine dripping/spray. All symptoms have improved with my protocol. At the worst I give it a 1 to 2 on irritation and discomfort and frequency. Helps: Elavil 5mg for anxiety and mast cell protection, (will only take it as needed) self internal PT as needed, stretching, walking, stairmaster cardio workout and light weights, reducing stress, moment to moment relaxation, deep breathing relaxation and using a Theracane. Makes worse: sitting for long periods, stress, over focusing on it. Currently 95%-98% recovered. Stay positive, relaxed and control your anxiety.

My own chronic prostatitis / chronic pelvic pain syndrome started after a bout of shivering and a mild fever too. Never did work out what caused it. The immune system can cause this for a number of reasons, not only infection. Autoimmune diseases, for instance, can also feature feverishness.

It's not going to be useful for you to analyse the past like this. Move on.

Hi there,

A good portion of the prostate itself is smooth muscle. It wraps around the bladder neck, and the prostatic urethra passes through it. Just below that is the urethra, your genitals, and pelvic floor muscles. (Very simplistic description, of course.)
