Hi Jay,

I've been told by fellow posters that not all PTs know what they're doing if the treatment didn't work or recreate my symptoms. Something to do with fingers being short...

It wasn't a supposition of my own in going into physical therapy but was told to me after about 10 out of 20 or so sessions where I was still in lots of pain and symptoms where not recreated or exacerbated upon palpation of trigger points of which the PT was never really clear if I had any just pressed internally and externally in the points where TPs should have alleviated my testicular pain.

In regards to that doctors comment, well, I guess he can say what he said as he's a hernia specialist. That's all he does is hernias, day in, day out as does the MD from Germany that I saw that I mentioned in this thread.
Age:37 | Onset Age: 35 | Symptoms: pain in testicles that comes and goes that also switches sides-trying to find a pattern as to "why" this happens/Rectal Burn at the 6 O'clock position at my anus which tends to flare around the periphery of my anus typically after defication but no perineum pain-no "golf ball" pain. Pain in testicles worsens as I sit but not always...Pain is not always present but depresses me. Pain used to be at a very high level but has subsided in time to a low level but has not gone away. Pain used to flare after sex but not as much as before although it still like playing russian roulette./
/Lack of SleepHelped By: I haven't found anything that has helped to my knowledge. Tried many many sessions of PT, accupuncture and chiropractic care but still have not found steady relief that takes me back to a pre CPPS state.

Yes, this is an opinion I've read from many sufferers, including Doctor Wise. The issue is that some trigger points can be located very deep inside. Men tend to have longer finger reach than women, allowing them to better access difficult areas, and/or they can apply a superior amount of pressure to TrPs. It's also possible that you PT simply wasn't good at it.

I think it would at least be worth a shot for you to see a specialized PT before going under the knife, unless your ultrasound test clearly reveals a hernia. Of course, I am just a layman and your doctor's advice should come first.
I am not a physician. This is not medical advice. Consult your doctor!

Age: 26 Onset Age: 17 Symptoms: Shooting, nerve-like pains throughout the penis, which abruptly hit and leave. Testicular pain, perineum pain, burning/irritative urination, extended pain after ejaculation. Occasionally, some allodynia or ache in the coccyx/sacrum/thigh/buttocks/legs. Diagnosis: Pelvic floor dysfunction, degenerated lumbar disk, and mildly herniated lumbar disk. Helped By: Physical therapy, pain management doctor, hot baths, therapy pool, stretching regimen, breathing exercises, relaxation, distraction. Worsened By: Arousal/ejaculation (worst), constipation, panicking/obsessing, other triggers depend upon current symptoms. If a top hernia doc has identified hernias, I would have them treated in whatever way he recommends. Sources of trouble in the pelvis, like hernias, can be the primary cause of chronic prostatitis / chronic pelvic pain syndrome (as can surgery, trauma, infections, IBS, and chronic tensing, inter alia). Get them cleared up, Rufus.

Saw another uro today.
Said I have a left sided inguinal hernia FOR CERTAIN but the right side was inconclusive.
He also palpated my epididymis on both sides.

Whats funny is that originally the left side would be much much more painful upon palpation on the right side didn't hurt as much.

Now it seems the left side hardly hurt while the right side was really bad.

I left the doctors office with a diagnosis of chronic epi and hernia.
He didn't find a varicocele.

So between the diagnosis of prostatitis based on a high white blood cell count in my prostatic fluid, the bilateral varicoceles (grade 2 on left and subclinical on right) and a hernia diagnosis...well I just don't know what to think.

Give me some credit as I've gone almost 3 years without having considered going under the knife and tried all sorts of non surgical therapies and supplements but I've made an appointment to get the hernias fixed.

I scared to do it but I read that hernias can cause scrotal pain.

Any recommendations?

G,

In the end, if it were me and a doctor confimed a physical abnormality, like your hernia etc...Then I would consider the surgery. Who knows, you could of caused the damage when you picked up your Wife that 1 time and this needs to be taken care of first so that the rest of the pelvic myoneuropathy can be slowly resolved through all the other methods mentioned here.

Cheers...Carlo... :)

I encourage anxiety prone UCPPS people to consider L-Theanine
Age, 44 onset age 37 Feb 2006 Freq. need to urinate. Sensation of having to urinate soon after going. Perineum discomfort/burning/tightness, pubic area discomfort @ times, poor urine stream, post urine dripping/spray. All symptoms have improved with my protocol. At the worst I give it a 1 to 2 on irritation and discomfort and frequency. Helps: Elavil 5mg for anxiety and mast cell protection, (will only take it as needed) self internal PT as needed, stretching, walking, stairmaster cardio workout and light weights, reducing stress, moment to moment relaxation, deep breathing relaxation and using a Theracane. Makes worse: sitting for long periods, stress, over focusing on it. Currently 95%-98% recovered. Stay positive, relaxed and control your anxiety.

Were it me, I think if there are clearly defined abnormalities that can be corrected by surgery then I would take that option. If nothing else it'll remove one potential source of the problem.
Age 56: Onset 2006 and bouts on and off since then. See posts for details.
