I also started hitting the gym VERY hard back in August, so hard that I competed in two bodybuilding shows, so you know my intensity was up. I've CONTINUED to progress to the point that symptoms are less of the norm and a normal urinary flow for much of the day is the norm. I hope to continue, I think it was less focus on this damn thing for the most part and continuing my internal work and work on my trigger points in the abs.

Kind of freaking out...
Other than frequency and some intermittent low level burning, I have been feeling fairly good the last few days.
This evening around 9:30 I took a shower and while I was showering I urinated. Don't ask me why, but half way through I suddenly...clenched.... I guess is the best word to use. It was like suddenly bending a garden hose in half and stopping the water. It lasted a second and then I was able to continue to urinate with no problem and completed voiding.
However,even after completing voiding, I have had this horrible feeling like I still have urine in me, but there is nothing left. I was able to fall asleep for a couple of hours, but woke up at 12:00 with the same feeling. I tried to get myself distracted, but have gotten progressively more anxious. I'm starting to pace and really starting to feel out of control.
I've been dealing with my chronic prostatitis / chronic pelvic pain syndrome for over a year and I have never had this happen. This is a totally new feeling and it's so horrible to deal with.
Is this urethral-prostate reflux? Did my urine shoot up into my prostate?
What did I do?
How can I make this go away? It's so unbelievably uncomfortable.

I've merged most of your threads into one thread, this one, strobers. I think it is much more useful to readers to see one person's case in one thread than many threads all over the place. Please post subsequent questions about your case here.
To answer this question, you are suffering panic attacks and anxiety about your case that is typical, if you don't mind me saying so, of our more frequent posters. You need to make sure that the anxiety is controlled. This forum has a lot of advice on how to do that.
And clenching during urination is not a good idea, so SIT when you need to urinate (less urge to clench when sitting).

I am not doing well right now. If in fact I have refluxed urine back into my prostate, what do I do to get back where I was? Did I reflux because my muscles are tight? Or is it because I have some organic problem with my organs down there? I have had this thing over a year and have never had this happen. Please give me some concrete things to do or try.
From now on I'm just going to urinate sitting down and keep myself from urinating when I'm taking a shower.

I've had the feeling that I still have urine in my system after I go. It's like I've completely voided and have no bladder urge to go but I can still tell there is some in my urethra. It almost feels like a pea or something small is stuck in the urethra. I have found that if I massage around and just under my prostate right after going a small amount of urine maybe 3 drops will come out and the feeling is relieved. I've been doing this each time I go to the bathroom and the feeling has started to dissipate. It's like you have to start getting the muscles and the sphincter working properly again. I hope this helps.

Hesitancy Issue
Three days ago I posted that in the middle of voiding I suddenly clenched up involuntarily and seemed to have caused a flare. Since then I have gone to the restroom sitting down no matter what it is I have to do; 1 or 2. What is freaking me out is that every time I go to the restroom to void I'm clenching and I'm afraid I'm making this flare worse and worse. It seems that I'm refluxing urine every time I clench and I don't know how to make it stop. It's getting to the point that I'm getting scared of voiding.
I also have been experiencing these spasms in the area right above the base of my penis about 2 inches below my belly button. I can actually see that part of my body moving around. It does not hurt, it just spasms.
What the hell do I do to make this clenching/hesitancy stop when I void? I have never had this before. It's a brand new symptom as are the spasms. The only medication I'm taking is the Elavil. Everything else are supplements as written in my sig.
Your help is appreciated.

This is happening partially because of the interchange we had recently, which just goes to show how powerful our minds are in controlling our bodies!
The only other suggestion I have, apart from seeking medication for your significant anxiety symptoms, is to void while in a hot bath. This may control the spasming.

I agree with Webslave about voiding in a hot bath. When that's not available, it's good that you're sitting to void. My own little trick for hestitancy/urethral spasm is this: I find that if I use one of my hands to pinch myself (doesn't matter where) while I'm urinating, my urethral sphincter is less likely to choke up. It's like one of those folk cures for hiccups (i.e. drinking a glass of water very slowly, allowing the swallowing reflex to overpower the hiccup reflex). Sounds wacky, but it works pretty well for me.

Boulder, thanks for the tip. I'm going to try this. I think what is stressing me out the most is what is happening to me physically when this happens. Is urine actually shooting up into my prostate? Is that physically possible? That is what is making me anxious. I guess I need reassurance that this is just clenching/hesitancy/spasm and not a big deal. Yet, when it happens I feel this horrible feeling that I need to use the bathroom immediately afterward that sticks with me, but I know logically I'm empty. Part of me thinks I have some kind of a block (which logically I know is ridiculous) and the other part of me thinks that the clenching/hesitancy/spasm is causing me to retain urine, which is giving me the feeling.boulder wrote:I agree with Webslave about voiding in a hot bath. Anything that interferes with the smooth exit of urine can cause turbulence and the potential for reflux of urine into the prostatic ducts, which is why urethral strictures cause problems. But once a stricture is removed, the problem goes away and so do the symptoms. So stop doing it, and you're fine.

Hey man this was my worst symptom. EVERY TIME I tried to piss it would clamp down or spasm hard much like an orgasm spasm, I could feel the urine back up, then it would come out.
The ONLY thing that helped was getting internal work done. The main trigger point for this symptom seems to be around the tailbone. I have a tool now I use to hit this area I was missing it with my finger and I'm not sure Tim Sawyer hit it either. I use that purple massager with a tip on it, its really a dildo, lol....isn't this stuff FUN, lol. Anyways, seek out physical therapy and learn to hit this area.
How do I think I got this? I was a stomach clencher and held my stomach in and UP for years, this then pulls the diaphragm up and this the pelvic floor up as well. My ass was clenched up so tight, I suspect you may do the same thing since this was the most unnerving symptom I had and not many other guys seemed to have it. I had it every time I pissed now its very mild and usually not present 90% of the time.
At first it was really violent very similar to an orgasm spasm...not fun....

Scooby, when that happened to you, did it cause you to flare? Do you feel that the urine backed up and caused your prostate to get irritated?

- painful urination: I don't feel pain when I go
- slow urine stream: Once the stream gets going I'm O.K.
- decreased urine output: I'm not sure what this means actually.
- spraying of the urine stream: It seems to come out in one stream
- blood in the urine: Never
- abdominal pain: Never
- urethral discharge: I'm not sure what this means, but nothing abnormal seems to have come out of my penis since I started having IC/CPPS.

Going to try an additional approach
I'm not doing well, but I'm confident that I'm going to be O.K.
When I started dealing with IC/CPPS I realized quickly that how well I did or did not do would be dependent completely on me.
One thing I personally believe is that the key to get things under control is to treat both the bladder and the pelvic floor simultaniously. That's why I follow the protocol in HITP and use a lot of the protocols supplements that are used by people with IC.
The thing I am going to add is to deal directly with my prostate. I ordered a couple of supplements that I understand have helped a lot of guys with similar symptoms and I am going to start doing prostate drainage. I'm a little apprehensive about it, but with the help of my wife, I'm hoping that it is just another thing that will help me in my quest to get well.
Normally, I would start things off by ejaculating, but it's not possible. I really wish I could ejaculate, but if I'm in a flare now, I can only imagine what an orgasm will do to my pelvic area. I'm going to start off by taking a hot bath and then do some trial experimentation. The last time I went to see a uro, which was a couple of weeks ago, he said that my protrate was a little enlarged, but stated, "I don't think it's causing you any problems though." Quite possibly these new symptoms I'm developing are a direct result of my prostrate enlarging and pressing against my uretha. Possibly the lack of not ejaculating for six months might be affecting the prostrate. I don't know, but I do know that I need to address all the possibilities.

New Bothersome Symptom and Question
I went to see my PT again yesterday and she probed me pretty thoroughly. I mean it was a full bore exploration. Throughout the treatment I did not not feel any pain or tenderness. None whatsoever. She told me that she could not feel any trigger points anywhere. She checked everything externally too and although there were a couple of slightly tight places, touching them did not cause me any discomfort and she felt that they were pretty benign. If in fact I have CPPS, why don't I have trigger points anywhere or the kind of pain that I read about here on the forum? Except for a time when I felt a charlie horse like feeling in my left calf and up my leg a few times, I have never felt any pain anywhere other than the one I've always complained about; the raw, irritated, inflamed feeling between the base of my penis to my perineum, which waxes and wanes is always worse sitting down then standing up. That, and frequency with an almost constant annoying feeling of needing to go, is what I have been suffering pretty much every day since last Monday, April 15th. Before that I was at least able to sit without discomfort.
I continue to suffer with this clenching, but now I'm starting to feel like it's my anxiety that is causing me to clench. Maybe the first time it really was an involuntary reflex, but now as I sit down to void I think about not clenching and I can't relax. I hate even suggesting this - do you think there might be an issue with the Pudendal Nerve (not entrapment but an irritation)?
