Cystocopy? CT-scan of bladder? For newbie to uros

Post by Nyckelben »

Hello. Long-winded, as usual.. :)

Just wanted some general advice regarding going down the uro-route..heh, well, I've only done kidney-scan before, but as of the recent flareup (January) I resolved to try things until I hit dead ends, not before. (Or until they try to make me eat antibiotics without very good grounds :p )

Anyways, saw my uro again today, (1.5 years since last , or so), and explained about my current issues. (Constantly weak urine stream, 'sore buttocks' feeling when sitting(new phenomenon; used to be just a 'tight' feeling on one underside); pretty severe pain occasionally when bladder has been full for a while, and sometimes some pain in back) and he said:

'It sounds a bit like a condition called prostatitis, which means that the prostate is sore; though there is no good test for prostatitis, the conclusion can be drawn via deduction. The prostate sits right under the bladder so it is likely the reason for the pain therein. As for your sore buttocks and occasional back-ache, I have no idea what is causing them, but it likely isn't the prostatitis.'

'Sore' buttocks and underside of top part of legs when sitting is a symptom of CPPS, yes? I don't know what would have happened had I mentioned 'Pelvic Floor Dysfunction' though. Something told me that wouldn't have been an instant hit.

'We shall do a cystoscopy - with a flexible scope if you want, but most young men tend to feel some pain when it is used, as it is used without anasthaesia, so I recommend the rigid variant. Additionally, a CT scan of bladder (and possibly something else), and a full battery of blood tests'.

..Is there any risk of a permanent change of state after a cystoscopy? Like some sort of (additional) diffuse pain that can't be explained..? Is a CT scan relevant?

I realize most here have already looked past urologists and their conventional and largely unsuccessful treatment-plans, where such exist, but as I've never been checked for strictures for example, would the above tests be at all good to do?

Funny thing though; I recently came back from a week in India (vacation with family), and every symptom except the weak flow was ..well, less. Maybe because I got some sort of stomach-bug on the fourth day. It's hard to care about sore buttocks when you have to vomit violently every 20th minute :p
Age: 27 | Onset Age:25.5 | Symptoms: Intermittent: nocturia, hesitancy, ache in testicles, thighs, perineum, back. Shooting pain in lower abdomen. When sitting: tense/taut feeling in buttocks, underside of thighs.
Occasional dribbling. Occasional sharp pain in bladder area when bladder has been rather full for a while.
Constant: Somewhat increased frequency. Weak urine stream(less than 50%, change was sudden). Ever present, mostly not variable. Helped by: Not thinking about it. Made worse by: Drinking a lot of anything and then going to sleep (sometimes), sitting (muscle tightness) (sometimes)

Stomach bug and symptoms viewtopic.php?f=37&t=5321&p=29082
Age:33 | Onset Age:19 | Symptoms:burning urethra, deep sore stinging around the perineum, dribbling | Helped By:when flare starts deep relaxing over a toilet to let out dribbling urine, hot bath, rubbing a heat cream on the perineum, Tens with a pad on the perineum, running, medication (solpadol) for pain. Current treatment: PT. | Worsened By:sitting, stressing over it, ejaculation, sometimes alcohol but I risk it. Been feeling a lot better last 6 months about 85% with a few flares lasting about 5 days. No longer having PT. Stretching every other night and I believe time is also a big factor for me

CT scan - a lot of us end up having them, I did too... apart from the radiation exposure it is probably worthwhile to exclude other causes. I'd insist on the flexible cystoscopy myself, because if he finds something he needs to correct (eg a stricture) he can go back in with a rigid. Flexible cystoscope is less likely to cause a stricture in the first place.

Sore buttocks and leg undersides are slightly unusual sites for pain.

Thanks for your replies.

Is a flexible cystoscopy generally done with any sort of anaesthesia?

He mentioned older individuals generally do not feel a lot during this procedure, but that younger males usually express a sensation of direct pain.
I seem to remember that he mentioned that he was going to do a 'general anaesthesia', ie not localized, the first time he talked about this (for the rigid scope, this is). Does that usually mean being 'put under'? I should probably read more and try to ask more..talking to him is a bit like talking to a whirlwind though, especially if he thinks you sound long-winded and droning, which I can sometimes be accused of.

Still, he strikes me as sympathetic, and rather knowledgeable, if bound by a rather classic viewpoint when it comes to 'prostatitis'.
(And even if I draw parallels to how I hear many dentists in the UK work, i.e quantity)

Look, this has been discussed innumerable times before, so answers to this could be found with the search function. In short:
  • Some uros like to do a general anesthetic so they can operate on any strictures found. That's also why some like using a rigid cystoscope.
  • You do not need a general anesthetic, strictly speaking. I had a rigid cystoscope with only an anesthetic gel in the urethra — not too bad, but painful afterwards ("pissing razorblades" - you'll get that after a general anaesthetic too).
  • I don't know what your situation is, and whether or not you can change uros easily, but if you can change to a younger uro who'll use a flexible and a local anesthetic gel, go for it.
