Howdy - I think you'll see that many of us on this board have IBS or bowel symptoms. I've had mine for years - and definitely think it's related to all the tightening and holding in my pelvis over the years. I didn't even know that I was forcing out bowel movements for years - pushing those poor muscles and nerves to their breaking point. I also have diarrhoea where I'm rushing to the bathroom and trying to hold it in (more pelvic tightening). I have the penis tip pain as well now - and without a doubt both things are related. The question is whether it is a purely behavioral relationship (BM troubles = pelvic tightening) or whether the systems regulating the neuromusculature of both the GI and GU systems are out of whack for the same reasons. It's probably both and in some ways I've decided it doesn't matter - and the best way is to deal with both possibilities...
i.e.
Learn to relax those muscles with paradoxical relaxation, deep breathing, yoga, meditation, biofeedback, etc.
Have a good PT help lengthen the muscles you can reach and workout those triggerpoints
Get some good relaxation medication like Elavil or Remeron to help (btw low dose antidepressents are often prescribed for IBS as well as Pelvic Pain)
Deal with the whole system as one - ie acupuncture or ayurvedic medicinal options if your open to them
Check in with your diet - I'm working through the elimination diet that Webslave recommends right now - do a search for Dr. Brostoff's book on this site. Sometimes food intolerance is part of the problem here
LIGHT exercise as recommended by Dr. Wise/Tim Sawyer (I'm talking 20 minutes of walking sort of thing)
put a stool under your feet when you sit on the toilet - and DON"T PUSH - what comes out is what comes out - don't force anything out.
Also - if you're like me (and many on this board) - I'm a worrier - I went to a GI doc, got a full colonoscopy/endoscopy to rule out anything else so I could put my mind at ease. It helped me focus on the things I can control - diet, state of mind, meds, etc.
I hope this helps!
Age:29 | Onset Age:29 | Symptoms: ORIGINALLY - rectal pain, penis tip pain, perineal spasms, golf ball feeling in perineum, painful ejacualation ONGOING - rectal pain, penis tip hypersensitivity, urethral pain at beginning of urination, ejaculatory pain, burning skin around legs and buttox | Helped By: warm baths, NO catastrophic thinking, Physical Therapy; Stanford/Wise-Anderson Protocol; Mirtazipine (Remeron) for anxiety and sleep, dry needling of TPs (NOT injections), PAIN PSYCHOLOGIST (CBT and Commitment and Acceptance Therapy), diet changes - so far eggs are out doing an elimination diet now :) | Worsened By: ANXIETY, FEAR, STRESS, CATASTROPHIC THINKING,heavy weight lifting , sitting too long
This is exactly one of my major complaints - I can't seem to get rid of the hypersensitivity and the tip pain - but I KNOW it is better when I'm not focusing on it...As I keep reminding myself over and over (sometimes it works) this takes A LOT of time to get here, and thus a good amount of time to get it gone. I'm going to go see a pain psychologist finally to get some good CBT help on dealing with this in the mean time. My current therapist is mostly psychodynamic and is not a specialist in helping people deal with pain...maybe that would help you too??

Brettp wrote:Ok, my flare up is including the issue of pain in the tip of my penis. I have had this before, and had to stop wearing my "Hugh Heffner" style robe as it felt like it was chaffing the tip. It did go away though after some time, and some valium which I was on at about the rate of 15mg/day, I'm now doing 7.5-10, and was reduced all the way down to about 5 before this flare started! I was going great with my PT and other forms of treatment until this started. I was wondering if anyone has any successful ideas on how to make it go away!!!!! :)
I've written a few posts on this recently. Try a search for "tip", "hypersensitivity", "chafing", and related keywords.
Started: Spring 2003; high urinary frequency and pain associated with bladder filling; urinary hesitancy; pubic/prostate/perineal discomfort; Helped by: trigger point therapy, Afrin nasal spray, Cymbalta, hydrocodone (small doses), distraction. Makes worse: sex.
Strangely enough, I have been concentrating on relaxation, and I also had an orgasm last night, and I feel MUCH better today, not really any irritation that I can feel, however it is strange, there does seem to be some red skin on the meatus, I have had this for a while now, started right after the bachelor party. But, I feel good today aside from the fissures/hemorrhoids, and that I can stand for now!! :) A positive mindset is so incredibly crucial, its so easy to wallow in misery and it almost feels good too, but this is a life changing disorder, and I'm trying to change my outlook on everything, I have had some success so far. I never really wanted to change who I am, but this is forcing me to, as if some cosmic force has forced me to sit and reanalyze my life.
Pain makes people do awful things, and make poor decisions, and its important that we all realize how powerful pain is and how it can effect our decision making processes, much like a drug will. I think understanding that dynamic, in a way, will also help to make good decisions, and also help to realize the connections between thoughts and pain. Just an idea I had, but I think it is somewhat relevant given the way I have been feeling.

I also have this symptom and it does reduce greatly when you get on top of the tension and other pain. My 2 cents on it is that this is one of the many referred pains chronic prostatitis / chronic pelvic pain syndrome suffers put up with and it goes slower because of its self-reinforcing nature - we feel irritation at the tip, we draw away from it by tightening our pelvic floor (and yes, there's muscles in your penis) and so we exacerbate the irritation etc. Try spending your next weekend off at home with no pants on all day (do draw the curtains ) and see how it feels after 8 hours with nothing to recoil from. And even if it doesn't work, it's kinda fun!
Wow, that sounds awful lot like what I had (search for my prior post). After about 8 months I finally saw a GI specialist who found only something called "Pruritus Ani" or itchy butt, lol. This however, doesn't explain why there's mucus and what looks like bloody "pieces" though. I suspect something else is going on.
The treatment he prescribed does help, however. Basically, he told me not to wipe my butt but jumps in the shower to rinse off the rear than lightly tap it dry or use a hair dryer.
- Take care to NOT use soap to wash the area, it will remove the natural oil.
- Apply a little bit of 0.5% hydrocortisone cream to the area.
- Once you get better, use Desitin (Diaper rash cream) in the area to protect.
I am much better with that symptom now. HOwever, I still think something else is causing it and "Pruritus Ani" is just a effect of that weird stool (mucus with small reddish pieces) I get from time to time.
I would still see a doctor to take a look, at least take hemorrhoid off the table.
Age: 38 Onset: April 2006 Current | Symptoms: Update, doctor found no hemorroid but pruritus ani. Was doing very good for a while but lately are having more flares. | Symptoms: raw, irrated feeling, spams in balls and bladder area. Drugs: Omega 3 oil only and just started Prosta-Q in Feb 2008, currently not taking Prosta-Q. Treatment: relaxation, AHIP stretches, the book EVERYDAY ZEN, Worsened by: Coffee (not 100% sure)

Well they generally go away in a couple days, I'm not over 40, I just figured they were fissures. I have had them in the past, but they seem to come intermittently, or after large BM. The blood is bright red too, so I'm pretty sure it is one, I'm not to worried, i dont think I should be worried, what do you think?

I got rid of this problem by squatting several times a day. At first, I tried 3 series of 30, 3 times a day and now just need to squat 1 time per hour as a safety precaution. I believe squatting realigns your pelvic bones, ligament and muscles in a straight manner. My hypothesis is that a misalignment of your pelvis is causing most symptoms, including the pain at the tip of the penis.
If you try, squat completely until your inner thighs reaches your calves. Make sure also to do it by putting your weight on the front of your feet (i.e. that your heels must not touch the ground)
Good luck!
Age: 37 | Onset Age: 37 | Symptoms: variable but include: frequency to urinate, discomfort in tip of penis (including occasional tingling), pelvic pain, pain in left inner leg/left testis, heat and sweat between legs | Helped By: Q-Urol or prosta-Q, taurine, hot bath, not sitting long hours, red wine (high in Quercetin I think, which reduces voiding a lot), stretching legs, pidgeon pause | Worsened By: sitting long hours, sitting in car .
Now I am cured!! Search for my posts on Harachair and squatting. I believe the symptoms are all caused by a misalignment of the pelvic bones that strains the muscles.
Do everyone with irritated Tip has irritated sigmoid colon?
Age:37 | Onset Age:22 (CPPS followed by NSAIDS-induce IBS) Symptoms:Trapped gas/irritated tip of the Penis Helped By Cold water/heat compress: | Worsened By:Food that cause gas or constipation
Good question Perth, how do we know if we have sigmoid irritation?
Well it's all part of my theory(i will explain in detail eventually) of pelvic sensitization.
Now to respond to your question,it is tricky to know exactly. One diagnosis test is colonoscopy. However, most chronic prostatitis / chronic pelvic pain syndrome sigmoid colon may appear normal to the G.I.doctor eyes. I don't want to elaborate too much (on the colonoscopy) but Perth you would possibly have a IBS diagnosis already if you had sigmoid irritation.---> IBS=Spastic colon=Abnormal gas motility. However,it is certainly possible that a CPPSer has Tip irritation only so that's why I ask this question.I know CPPSers don't necessarily have IBS.
*I think if a sigmoid colon is close to a chronic prostatitis / chronic pelvic pain syndrome bladder base, you may win the lottery of CPPS+IBS.
