MB,What are your symptoms other than burning? So far I feel better , I am on day number 3 since the injection.

My PT told me that if the muscle doesn't relax within three injections of Botox, no more. I hope that this is the only one I need!
Age:30 | Onset Age:27 Symptoms: Golfball sensation, cramping in the rectum, inflamed/tight levator ani, occasional frequent urination, mild constipation, occasional burning testicular pain, IBS. Helped By: Exercise, Stretching (Yoga) specifically a deep lunge or pidgeon, running, beer, alcohol, relaxing,deep breathing Worsened By: Sitting for long periods (even w/ doughnut cushion), Stress, tight pants, caffeine, coffee in particular Tests: Pelvic MRI, Colonoscopy, Anal Monometry, STD, Pudendal Nerve Block. Botox into obturator internis, levator ani, and piriformis. Meds Xanax, Phenibut (The phenibut works awesome, better than the Xanax in my opinion)

Any more change in symptoms since injection?

Its been two weeks since I had the Botox injection. I would say that I have noticed about a 50% improvement so far. I noticed that its much easier to get erections and I had a lot of wet dreams the first week after the injection. Plus I noticed my erections were harder (sorry if this is a bit personal but I think its important information) I think this happened because my pelvic muscles were finally able to relax and the pudendal nerve wasn't being irritated. When I do a squat , I dont feel that extreme tightness I used to feel. The levator hasnt cramped up too bad either.I still have the sensation like I am sitting on something like a cushion bilaterally but its better than before. Hopefully this can be worked out with my PT. My PT told me she felt that there was a lot less inflammation and less tightness when she did the internal work. I am going to continue to do stretches and get better! Will keep you guys posted in another couple weeks.

Started: Spring 2003; high urinary frequency and pain associated with bladder filling; urinary hesitancy; pubic/prostate/perineal discomfort; Helped by: trigger point therapy, Afrin nasal spray, Cymbalta, hydrocodone (small doses), distraction. Makes worse: sex.

Thanks . This is *real* good news. I spent the whole day wondering if I will ever get out of this morass. This gives me something to look forward to

Wondeful news...This just re-inforces the fact that tight muscles play a major role in CP/CPPS. If we can relax the muscles any way we can then go for it. Keep us posted on your progress. Great news to read and I'm glad you are doing the PT along with this method of relieving your discomfort/pain.

I encourage anxiety prone UCPPS people to consider L-Theanine
Age, 44 onset age 37 Feb 2006 Freq. need to urinate. Sensation of having to urinate soon after going. Perineum discomfort/burning/tightness, pubic area discomfort @ times,poor urine stream, post urine dripping/spray. All symptoms have improved with my protocol. At the worst I give it a 1 to 2 on irritation and discomfort and frequency. Helps: Elavil 5mg for anxiety and mast cell protection, (will only take it as needed) self internal PT as needed, stretching, walking, stairmaster cardio workout and light weights, reducing stress, moment to moment relaxation, deep breathing relaxation and using a Theracane. Makes worse: sitting for long periods, stress, over focusing on it. Currently 95%-98% recovered. Stay positive, relaxed and control your anxiety.

It's great to think that eventually, if caught early enough, one Botox injection could put an end to this. Fingers crossed.

Thanks for your recent note that the Botox provided you with 50% relief. Please answer the following:

1. Total amount of Botox used.
2. What were the injection sites;
3. Was the prostate injected?;
4. How were the injection sites chosen - trigger points?;

I am considering Botox soon. I appreciate your input.

It'd be nice, but I doubt it.
  • Botox is not a long lasting drug
  • The tensing habit is not broken by Botox
  • Anxiety is not controlled by Botox

In principal I don't see a difference between regular Botox injections and the amount of work involved in SP. For some men it may even be more convenient.

I don't think Botox is going to be an all-around cure for CPPS, but I hope that studies will demonstrate it to be an effective weapon in the anti-CPPS arsenal. Even though its effect directly on the muscles lasts 3-6 months, if it breaks the pain cycle during that time, it can potentially have a long-lasting effect, as it probably did with bti20. Let's hope Dr. Berger gets his study up and running again.

Sorry it took so long to reply back guys: After getting the Botox injection into the obturator internus , my PT noticed (when doing internal work) that the piriformis was where a lot of tightness and soreness was coming from. So I just got the pirifromis botoxed . Its been a week and a half since the piriformis injection and I don’t feel the same immediate relief as the obturator injection. In fact it flared me up pretty bad. Maybe I need to wait longer. I will keep you posted

On the positive side the levator feels normal , no cramping sensation anymore. On the negative side, the injection into the obturator has worn off a bit. Maybe down to 25-30% relief. I still have a bilateral cushion sensation when I sit. I believe the obturator is causing this . It is inflamed for some reason and won't calm down. Its kind of strange that I have right lower quadrant abdominal pain and my right obturator is a lot tighter than the left. I ran across this picture of how the appendix comes into contact with the obturator on the right side. Maybe its causing the obturator to spasm...I am getting a CT scan this week to see whats going on. I remember the night before my chronic prostatitis / chronic pelvic pain syndrome came on , my abdomen hurt really bad! I will keep you guys posted on everything.

An update: The Botox doesn't really seem to be doing much anymore. The levator and obturator are tightening up again. The triggerpoints are coming back. I am still seeing a PT every week for the internal therapy. Its kind of strange I got no relief from the piriformis Botox injection where I have active triggers but got significant relief from the obturator and levator injection where I also have active triggers??? If anything maybe the Botox has helped confirm which muscles are irritating the pudendal nerve more (or part of the pudendal nerve which is irritating the muscle)???

The CT report was fine, appendix was not inflamed. I still believe that there is something from my abdomen causing this??? Going to see the GI doc in a couple weeks to see if he has any more insight. My issue is a bilateral lump sensation (not a golf ball) when I sit which I believe is coming from the obturator mostly.

Can you please post how many units of Botox did you get? Also can you describe your procedure a little bit.
Different docs seem to do different things. Some inject into the trigger points , others into the belly of the muscle etc.
